         <description><![CDATA[<div>1. Interprofessional education arose due to the challenges encountered as early as the 1900s. The most concerning among the issues raised in the reports of the Institute of Medicine (IOM) was the increasing number of poorly equipped and educated healthcare professionals. A new educational paradigm was thus established, in which from learning models that used to focus on the independent care for patients, students in the various healthcare professions were now being taught to work collaboratively with other disciplines as well. To optimize a better health system performance, the Institute for Healthcare Improvement developed the "Triple Aim," consisting of three dimensions namely, (a) improving the experience of patient care, (b) improving the health of the population, and (c) reducing the per capita cost of healthcare. Along with an effective and efficient interprofessional communication and collaboration, team-based and patient-centered care are all essential in meeting the goals of the "Triple Aim."<br><br>2. The interprofessional education comprises of four core competencies which I believe I could apply both personally and professionally in my life. The first competency centers on one's moral and ethical values. Interacting and/or working with people of other professions entails mutual respect and acceptance for one's similarities and differences. The second competency is the knowledge and awareness of one's roles and responsibilities as well as that of other professions in order to appropriately address the needs of patients and the rest of the population towards the advancement of the society as a whole. Interprofessional communication with patients, families, communities, and professionals in healthcare and other fields is also essential, and such must be done in a responsive and responsible manner. The last competency encapsulates the importance of being able to work in a team. The value of relationship-building as well as the principles of team dynamics are applied in order to deliver the best care that our patients deserve. When teamwork in any setting is effective, communication, collaboration, and the combined efforts of the group will always make for a better outcome.<br><br>3. Physical therapists are called 'movement experts,' since we are responsible for the treatment and rehabilitation of patients who were diagnosed with a medical condition or have incurred any injury that has limited their abilities to move and function in their daily lives. During the initial examination, a physical therapist makes a diagnosis for the patient based on their level of function in performing tasks, including their basic and instrumental activities of daily living. Based on the observations of the patient and the results of the examination, an individualized care plan is developed and implemented. Throughout the sessions, the patient's progress is evaluated, and the interventions to be performed on and/or by the patient are adjusted accordingly until the goals set during the first meeting are attained, and the patient is already cleared for discharge.</div><div><br>4. Interprofessional education places an emphasis on teamwork and cooperation. For one to consider a collaboration a successful one, every individual must not only be aware of their tasks, duties and responsibilities as a member of a certain organization or at the workplace, but that of their other colleagues as well. Role clarity is an essential precursor of productivity; when everyone knows what they need to do and what are expected of them, the organization's overall work experience increases immensely. The individual also tend to become more efficient and effective in work, thereby improving trust among colleagues and avoiding possible conflicts brought about by stress and burnouts.</div>]]></description>

         <description><![CDATA[<div>1.&nbsp; &nbsp; &nbsp; It is imperative to understand that no man is an island. Each of us has a specialty that we can use to contribute to society, but challenges don’t come with one configuration. Thus, collaborating with others is inevitable to fulfill the gaps that are needed to solve issues which in turn can also help us develop our skills as we learn from each other. With regards to “Patient-Centered Care”, treating patients should not solely focus on the disease itself but should also put attention on the patient’s wants and views on the respective plan of intervention. In addition to this, family members should be viewed as participants in improving the health condition of the patient in which the health care professionals guide them on ways to do so.</div><div><em>2.&nbsp; &nbsp; &nbsp; The learnings I’ve mentioned are not limited to professional settings but can also be utilized in one’s personal life. Being able to collaborate with other people to reach a common goal is surely an edge whether in solving personal problems or professional challenges. Also, having an open mind on others’ beliefs and perspectives, in general, can generate new knowledge and healthy interactions. This wisdom can later come in handy in achieving success.&nbsp;</em></div><div>3.&nbsp; &nbsp; &nbsp; As a future Physical Therapist, my role and responsibilities include being able to provide preventive care to the public to preserve the health and wellness of the whole population. The profession also comprises improving the functional capability of patients with neurological conditions, musculoskeletal conditions, as well as those who have lung and heart issues. In doing the abovementioned roles and responsibilities, having a “heart” is the most essential value to hold. Empathy should always be embedded in oneself to live a harmonious life.</div><div>4.&nbsp; &nbsp; &nbsp; <em>Having a great understanding of each role of the team members will prevent overlapping of the tasks that need to be done and will promote efficient flow of the process. This is where having a proper plan takes place. Great collaboration comes from being good in each of your roles. This will prevent any inconveniencies in the execution of the plan. It is also good to note that there is no “big” or “small” role. All contributions hold equal values since it was all essential in attaining the goal of the team.<br></em><br></div>]]></description>

         <description><![CDATA[<div><br>1) I learned that collaborative learning is a side-effect of effectively collaboration among members of different field. When we understand our roles and the boundaries of our area of expertise, we will be able to learn a thing or two from our colleagues and apply these learnings to our own practices. As for “Patient-Centered Care”, I learned that despite the differences in roles among healthcare professionals, the common denominator is, and always is, the patient and their well-being. Although we may have different areas of expertise, all of our clinical knowledge comes together with the common goal of achieving proper healthcare for our patients.</div><div><br></div><div>2) I learned the importance of knowing where the boundaries of your area of expertise are. This allows us to have clear cut roles and responsibilities since without these boundaries, we’ll all end up getting in each other’s ways. Only when we know these boundaries can we understand when to listen to our colleagues and when to speak.&nbsp;</div><div><br></div><div>3) As an aspiring Occupational Therapist, my role is to re-build the different skills and functions my patient lost due to their injuries or condition. My role is to re-teach my patients how to perform their everyday activities once again or perhaps provide adaptations so that they can still perform the activities they find valuable despite their disabilities.</div><div><br></div><div>4) As mentioned earlier, not having clear roles among team members will lead to us getting in each other’s ways. This can cause conflicts to arise as it’s not only unprofessional to overstep your boundaries but it is also disrespectful towards your colleagues who clearly have the knowledge and expertise in the area you’re trying to step into.</div><div><br><br></div>]]></description>

         <description><![CDATA[<div>1.)&nbsp; Providing the needed medical care entails a holistic approach that does not only involve the health-related advice and opinions of healthcare professionals but also the perspective of the patient, family members and the community. In this way, the concept of patient centered care is practiced. At the same time, the healthcare team should also learn how to properly communicate with each other in relation to the general well-being and direction of the care for a patient.&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;&nbsp;<br>2.)&nbsp; I could use the values of proper communication and teamwork in whatever problems that could arise in my personal or professional life. Simply being honest in voicing out my concerns or learning how to work on a task with others could not only strengthen my relationships but also get more high-quality work done. &nbsp;<br>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;<br>&nbsp;3.)&nbsp; As a future physical therapist, my role generally involves providing a treatment plan to restore patients to a more functional level or prevent further disability. There are also examination procedures to be done in order to better assess the condition of a patient. Although PTs are not really mental healthcare experts, we were also taught the basics on how to help with the mental health of our patients as this could affect the various treatments.&nbsp; &nbsp; &nbsp; &nbsp;&nbsp;<br>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;&nbsp;<br>4.) &nbsp; Having a clear picture of one’s role in a team prevents possible misunderstandings and accidents from happening. One of the ultimate goals, of course, is not to cause further injury to a patient.          <description><![CDATA[<div>1. It is necessary for us students to understand the importance of collaborating professionally and healthily among different fields to deliver the utmost care towards the client/patient. To be able to hone our skills in the future and make it work as one unit will not only be able to deliver the quality treatment/care that the patient deserves, but the smooth and efficient manner of managing the patient as well. With all of this in mind, the key phrase is and should always be "toward the patient, because at the end of the day, as future healthcare professionals, our priority is the patient and to be able to use the collaborative knowledge and the different values and principles innate to each individual, comes a well-rounded, patient centered and top quality management.&nbsp;<br><br>2. On the professional level, the learnings that I have acquired can be used whenever I am handling patients inside or maybe even outside either my place of employment, clinic, or the hospital. This is to ensure that I am not only representing myself as a future PT but also the name and reputation of the hospital/clinic that I am bearing name. Meanwhile, on a personal level, these learnings can be applied whenever I am in a situation wherein my skills as a healthcare professional are needed, along with the values that I have learned as a student. With this, I can still cater to those close to me and still be able to provide the same work ethic as if they were patients in a clinic.<br><br>3. PTs are responsible for multiple events and scenarios that happen inside the clinic such as taking a history and physical exam, treating different scopes of problems such as musculoskeletal, neurological, etc. However, it also expands beyond the professional limitations by having the compassion, empathy and other values towards the patient. This ensures that the management that I can give as a PT does not only necessitate the technicalities of the profession but also the human side as well.<br><br>4. A clear distinction without discrimination should always be practiced within any healthcare organization. A distinction should be enforced as for those involved in the organization are well aware of the different areas of expertise that each field has to offer. This now can be helpful in a patient centered manner of care wherein different professions can collaborate in giving the best care towards a patient.</div>]]></description>

         <description><![CDATA[<div>1. I have learned that collaborative learning is a method of enhancing learning by working in groups. Groups of two or more people collaborate to solve problems, complete tasks, or learn new concepts. Collaborative work isn’t something new in our lives. In fact, it is not surprising that the roles of different healthcare professionals frequently overlap. Management overlap is common among healthcare professionals, especially in the rehabilitation field. It was emphasized how important communication is between the team members. They must know their roles and goals as well as the other members’ of the team so that they can all work in harmony, thus avoiding redundancy of treatments. Having a deeper understanding of the case of a client will benefit not only the clinicians but also the client themselves. This will then lead us to providing a patient centered care. Individuals' specific health needs and desired health outcomes are the driving force behind all health care decisions and quality measurements in patient-centered care. Patients are partners with their health care providers, and providers treat patients not only clinically, but also emotionally, mentally, spiritually, socially, and financially.&nbsp;<br><br>2. Collaborative learning isn’t only limited to the school and professional setting. This can also be applied to our personal lives. Hearing other people’s perspectives and judgements isn’t always a negative thing. We may use these outside perspectives to make a better version of ourselves that would help in the improvement of our society. Knowing are roles and responsibilities also help us in defining boundaries and knowing our limits. We are not capable of knowing how all the things in this world functions. For this, we should be grateful that we can learn different insights from other people.<br><br>3. My roles and responsibilities as an aspiring Speech-Language Pathologist include preventing, assessing, diagnosing, and treating speech, language, social communication, cognitive-communication, and swallowing disorders in children and adults.<br><br>4. Clarity of roles is required for effective team performance. When team members understand what is expected of them, what aspects of their role are most important, how their performance contributes to team goals, and how their performance affects their common goal for the patient. As previously stated, it is not enough for each team member to understand his or her own role; each team member must also understand the roles of all other team members. This necessitates regular communication among team members to ensure that everyone understands their individual and collective roles.</div>]]></description>

Collaboration is a practice that I know bears importance but never actually realized <em>how</em> important it is in providing the highest quality of care possible for a patient, considering the resources accessible and available. With this, from the sessions and modules presented, learning collaboratively is one way to enhance teamwork and develop a better understanding and implementation of various techniques and performance in healthcare provision. Furthermore, having the patient partake in an active role in the therapy they receive is also emphasized wherein it’s not only the professionals that must work together but the patient themselves.<br><br>2. Interprofessional education and collaboration are concepts that I perceive I can apply both personally and professionally. Learning how to be a team member, performing my role actively, inclusively letting others share their thoughts and insights on the topic, developing competence in the field, and taking a proactive approach to situations that need it, are all characteristics that must be put to practice not only with being an OT but also as a person. Establishing a positive relationship and environment with the people I will be working with and as an individual, along with applying collaboration and effective communication, will result in a greater outcome.<br><br>3. Occupational therapists (OTs) utilize the therapeutic use of everyday tasks to treat the various aspects of a patient’s case—be it mental, physical, or developmental conditions. There are various techniques in the profession that can be used to maintain, improve, or rehabilitate a patient depending on their situation for them to participate and perform in their chosen occupations. Apart from this individual role, OTs also communicate effectively with the parents or caregivers of the patient, as well as with the professional rehabilitation team for plans, evaluations, etc. for a patient.&nbsp;<br><br>4. Knowing one’s role in the team is crucial to implementing an effective plan for the patients being handled. As such, having clarity among the members of the team regarding their responsibilities and obligations is also significant in increasing efficiency and effectiveness in the roles they take part. Moreover, it develops their accountability in various aspects as a professional, resulting in more enhanced team performance.&nbsp;</div>]]></description>

I learned the importance of interprofessional collaboration because it is an avenue for us to practice it in the real world, especially in the healthcare setting. We have to prioritize our future patients’ safety and this can be done by working efficiently and holistically with the involved team. We are dealing with the lives of other people so it is vital that we know how to take into account the opinions of others into having a production work ethic.&nbsp;</div><div><br></div><div>2. Using my newly acquired knowledge about interprofessional collaboration can be professionally seen in clinics, home-care settings, and hospitals. Right now, I can see how we will be working with other therapists, doctors, nurses, and other healthcare workers. I am not just representing my name as a future PT, but also the place and people I will be working with. On a personal level, we work with different people throughout our life. The saying “no man is an island” is a true statement since we learn how to connect with various people in different stages of our life.&nbsp;</div><div><br></div><div>3. My rules and responsibilities as a physical therapist include proper subjective and objective evaluation of the patient to properly assess and examine the findings in creating a tailored exercise or therapy program. We deal with conditions such as musculoskeletal, cardiopulmorenal, neurologic, etc. Although, we are also taught to put emphasis on proper patient care and empathy not just towards them, but to their families as well.</div><div><br></div><div>4. In a team, there is bound to be miscommunication among members due to the fact that we tend to forget important information. This is why clarity is crucial, especially in reinforcing and designating proper roles and responsibilities to lessen obscurity and confusion between members. Particularly in a healthcare setting, if we miss out on important facts, numerous problems such as misdiagnosis of the patient may occur due to miscommunication between workers.&nbsp;</div><div><br></div>]]></description>

What are the key insights I learned from the session on “Learning Collaboratively” and “Patient Centered Care”?&nbsp;</em></div><div><br></div><div>It was quite surprising to learn that 44, 000-98,000 americans die each year as a result of medical blunders caused by a lack ofcommunication among the healthcare workforce team.&nbsp; This has made me realize how important it is to understand one's roles and responsibilities as well as how they relate to an interprofessional collaboration.By doing this, they are able to recognize their own strengths and limitations which will allow them to coordinate with other members of the team in order to provide the patients with the best care possible. In terms of patient-centered care, it's essential to educate families and patients about the condition and how it may affect their daily lives in detail. They are also informed about the different therapeutic choices for their condition. They are also instructed on the healthy behaviors they should practice to prevent their illness from getting worse. I also discovered how critical it is to approach patients with compassion. It is very important in treatment since it gives the client confidence that their well-being is being cared for.</div><div><br><em>2. In which areas, personal or professionally, will I use what I learned?</em></div><div><br>Working with other professionals is inevitable for me as a future SLP professional who will operate in a variety of contexts. I can use what I've learned about working together in an interprofessional team to make sure the patient I'm taking care of receives high-quality care. When used in a professional setting, the lessons I've learned from this will aid in achieving the team's and the patient's objectives. On a more personal level, I'll be able to work with others effectively and connect with them, which will help me build strong relationships with them.</div><div><br><em>3. What are professional and/or non-professional roles and responsibilities in your chosen field?&nbsp;</em></div><div><br></div><div>As an upcoming Speech-Language Pathologist my professional role and responsibilities in my chosen profession are to be able to diagnose and treat communication disorders, hearing issues, feeding problems and swallowing difficulties. It is also my duty to become a supporting role to my patients, in doing this, it would comfort them that someone cares for their health and well-being.</div><div><br></div><div><em>4. What is the value of role clarity among members of the team?</em></div><div><br></div><div>In an interprofessional collaboration, it is vital that every member of the team understands each of their roles and responsibilities to effectively function as a healthcare team who can deliver a quality service where the patient's needs are addressed. By realizing their roles, they are able to work together efficiently. Through this, they are able to succeed in identifying and achieving the goals of the team and the client.          <description><![CDATA[<div>Looking back at the team dynamics that were practiced during the different tasks we’ve been doing throughout the semester, I learned that having open communication within the team is super important because it allowed us to generate new ideas that we later on pieced together to complete the different tasks we had to accomplish. It was also evident that the open communication among team members was practiced all throughout the task, meaning the communication didn’t stop when the planning phase was over. I noticed that even as we were doing the tasks we were delegated to do, we were still in constant communication with each other, checking in on each others’ progress and whether anyone needed assistance.&nbsp;</div><div><br></div><div>Using the <em>“What? So what? Now what? Reflection Model” </em>, I noticed that during the different activities, the majority of us were proactive and took initiative when it came to giving suggestions and new ideas. I personally see this as a good thing because it means that the team is comfortable with one another and we’re confident that everyone in the team will actively listen to each others’ ideas and suggestions. However, there are always things that can be improved on and one of the things I noticed needed improvement was the inclusivity of the group’s dynamic. Being a team consisting of different people, each with different personalities, I think we as a group could’ve been more inclusive towards the more soft-spoken members of the team since they may have been overlooked during the times wherein the majority of the members were busy talking about our ideas, such that we might’ve forgotten to ask for the insights of the other members.&nbsp;</div><div><br></div><div>After all the opportunities to practice and observe the importance of clear communication among team members, I believe that Reflective Practice is central to us Rehab Professionals because this allows us to use every experience we have “under our belts” to build on the knowledge we already have. It is said that one of the best ways to learn is through self-reflection and self-correction, so being able to analyze our own performances will allow us to get a better understanding of ourselves, our strengths, and our weaknesses. When it comes to my preferred model of reflective practice, I would say it’s the Gibbs’ Reflective Cycle because it doesn’t just take a look at the facts but rather it also allows you to look into your feelings, which is something that we tend to forget when acting professionally. Using this model, we’re free to reflect on your feelings, internalize, and then analyze them before we start thinking about rational adjustments we can implement for the succeeding sessions. In terms of applying this model in future practice, I will make it a habit to allot a set amount of time in between my therapy sessions in order to reflect on the session that had just transpired in order to find adjustments I can implement in the succeeding sessions. After every session, I will start off by reflecting on the subjective feelings I have regarding my performance as a professional. After which, I will start to look at the objective facts regarding how I performed as a therapist. Lastly, I will piece together my subjective feelings and the objective facts regarding my performance in order to properly analyze the situation holistically and rationally. Doing these will allow me to come up with solutions, adjustments, and action plans that I can apply in my next sessions.</div><div><br><br></div>]]></description>

         <description><![CDATA[<div>1. Teamwork and effective communication are vital factors that can lead the group to success. Teamwork and communication work hand in hand—a team, can work well together if each group member can voice their thoughts and opinions. Communication allows collaboration and builds a common purpose within the group, allowing them to reach their goals. In the IPE group, the liaison officer and members demonstrated effective communication and efficient teamwork through the previous tasks in the subject course but even more so during the synergy. While a liaison officer exists to initiate and facilitate group discussions, the rest of the members cooperated by accomplishing the parts assigned to them and not hesitating to speak up about their questions and concerns to the group. It was also ensured that every team member actively engaged in the conversation and consciously listened and understood their fellow group mates. However, I think one of the areas which needs to be improved in my IPE group is the initiation of tasks by the members. Most of the time, it is the usual people who would step up and volunteer for a group task or when a representative is needed from the team. For one, when the liaison officer is unavailable, I would try to facilitate the group during our meetings and distribute the tasks among ourselves. I was hoping to hear more from the other members since they are also rich with ideas and have so much potential to give and show to the group. One way to improve the situation is always to consider their thoughts and feelings and involve them more in the discussion. When a representative is needed, we could politely ask them if they want to volunteer for the position and still respect their decision if they decline.<br><br>2. Reflective practice is essential in the practice of our profession in the future. It encourages self-reflection and increases self-awareness, as one can look back at an experience that has occurred during their practice and think about the decisions that were made and actions that were taken, as well as their consequences. Among the models of reflective practice, my favorite is Gibb's "<em>Reflective Cycle</em>" (1998), which is an advancement from Kolb's model. It is designed as a continuous cycle for both a standalone or repeated experience. This model has six principal elements, namely, (a) a <strong>description</strong> of the situation in detail; (b) an exploration of <strong>feelings</strong> and thoughts regarding the experience; (c) an <strong>evaluation</strong> of how the situation was handled; (d) an <strong>analysis </strong>of the individual aspects of the event; (e) a <strong>conclusion</strong> of the investigation; and (f) the <strong>action plan</strong>. As a healthcare professional, from time to time, I actively engage in conversations and deal with patients, clients, colleagues, and people beyond the organization. In order to keep pace with the changing nature of my profession, reflection is a crucial aspect of personal and professional development. Through a structured form of reflection, I will be able to identify my role in the healthcare setting and have an opportunity to think about the effects of my behavior and actions. Specifically, I will be able to continuously reflect on how things may be improved for my patients and/or clients and do something to make things better for them.</div>]]></description>

         <description><![CDATA[<div>1. The team communicated very well since they engaged in open communication where each member has time to voice their opinions and concerns. In addition, varying perspectives were summated after constructive criticisms of each input which resulted in a holistic plan. Also, the team members were all aware of their roles and the problem to be addressed. Following this, reflections come to a place where all members identify which of their actions should be followed troughed and which should be developed. This can be further improved by recognizing own’s strengths and weaknesses, examining the factors significant to the event that transpired, integrating current knowledge and new knowledge, evaluating the generated idea in addressing the problem, and actual implementation when it reoccurs.<br><br>2. Reflection on practice is an essential part of continuous self-growth to have better responses to uncertain circumstances that might be encountered. The models of reflective practice somehow overlap with each other in terms of their elements, but for me, Gibbs’s reflective practice depicts the most holistic approach to the reflection process. The model starts with being aware of the event that took place. Then, it is followed by the acknowledgment of thoughts and feelings that arise from the problem to have a comprehensive evaluation of whether it was a good or bad experience. Next is where critical thinking takes place in which all the processes before this stage are taken into consideration to generate a conclusion in a general sense and individual level. Lastly, an action plan will be applied in any case a similar scenario arises while keeping in mind the things to be done differently and similarly. This can be implemented in my future practice by increasing my knowledge through stacking up experiences with different patients since this model emphasizes learning by doing principle. With this, reflection should always be manifested in practice to provide the best course of action in assessing and treating our future patients.&nbsp;</div>]]></description>

         <description><![CDATA[<div><em>1. </em>Having proper communication became essential in the successful completion of the various tasks given to us. One of the activities that I would cite as an example would be creating paper chains. In here, we were assigned to accomplish it without talking while only using our dominant hand. The first thing we established before the activity was assigning roles such as those that would cut the papers or the ones who would tape the papers. Once the activity started, I just assumed my role right away and helped one of my groupmates in tearing papers in halves. However, in the middle of it, we noticed that the papers were piling up faster than what our other groupmates can tape out of. So we thought of dual-tasking and proceeded to help them with taping the papers to create a long chain. With this, we were able to complete the task faster than before. Although our communication was limited, it was good that each member had the initiative and understanding of what should be done<br><em><br>&nbsp;2. </em>Reflection is something that we can do in our everyday lives to continually improve ourselves. It helps us with regards to our self-awareness. My favorite model of reflective practice would be the ERA model developed by Jasper (2013) which only has 3 components: experience, reflection, and action. Due to its simplicity, anyone could easily understand the gist of this concept. In my future practice, I am aware that not every experience would be a positive one. There are days where I could receive negative feedbacks from my colleagues or patients. Nonetheless, positive and negative feedbacks would still help me in improving my skills as a future healthcare professional. Knowing my strengths and weaknesses would give me an idea on how to act more appropriately the next time I encounter the same or a different situation.&nbsp;<br><br><br><br></div>]]></description>

         <description><![CDATA[<div>1. We were able to communicate through our meetings and sharing of ideas which paved the way for productive teamwork and collaboration. Especially, our means of communication were mostly virtual, which hindered us in a way since we were not able to connect on a deeper level due to only seeing each other through a screen. It was hard for us to make time since we came from different schedules, thus trying to continuously find a common ground through which we can talk to each other. The barriers that we faced made me realize that there are multiple things needed to have proper and effective communication. Time, respect, patience, attention, and open-mindedness are just some of the traits on top of my head that are essential for us to connect with each other. In reality, communication is a vital part of our life and the saying “no man is an island” is indeed correct because our life is based on meaning and connection with different people, aside from our group mates. Finding the balance between listening and talking with each other can also be a great way to have productive conversations since communication is a two-way street.&nbsp;</div><div><br></div><div>2. Reflection on oneself is a great way to learn about how we are and what to improve on. This is something we do every day to learn about the deeper meaning of what it is to live our lives and how we connect with the people around us. Professionally, we must also instill reflective practice so we can grow holistically and positively as we enter the real world as healthcare professionals. My favorite model of practice is Gibb’s reflective cycle since it highlights the person’s feelings and how we think about a certain situation. Retrospectively, it also makes us evaluate how our moral compasses are vital since we need to assess the situation of it being bad or good, and how it affects other people. We can see how this model is a cycle that each of us goes through and helps us thoroughly think about scenarios or situations that may arise. We can use this in safely evaluating our patients to make sure what we do is for their greater good and for their benefit. We are dealing with their lives so we must highlight the need of reflecting on our decisions to have a productive and fruitful relationship not just with our patients, but with our colleagues as well.</div>]]></description>

         <description><![CDATA[<div>1. For our team, proper communication and teamwork was essential in accomplishing the tasks that were given. During the first few weeks of working together, our group was still trying to figure out how we can work together despite our differences. During meetings, no one would usually initiate to share there ideas as they were too shy to share their opinions and views. We tried to ask each member their insight as we value our knowledge differences to provide a holistic approach. Once we got our rhythm, each member was able to take initiative and contribute based on their strengths and weaknesses; in the process, the team was able to move forward harmoniously as each member now knows their different roles.<br><br>2. Reflection allows me to recognize and appreciate positive experiences while also identifying ways to improve my practice and service delivery. It can also be helpful when I've had more difficult experiences, allowing me to process and learn from them. Reflection is an important part of learning new skills, but it also improves our ability to respond to challenges, make timely decisions, manage emotions, maintain productive relationships, and deal with stress.&nbsp;</div><div><br></div><div>The Gibb's model is one of the models with which I identify. It allows me to grow and develop as I gain a better understanding of my experiences and feelings as an SLP student, which will help me improve for my future career. This model will help me plan and implement a more effective intervention for my clients and their families, as well as strengthen my relationships with them and my colleagues. Furthermore, I can use this model to share my experiences and learnings with others so that they, too, can benefit from and learn from what I've encountered.</div><div><br></div>]]></description>

Communication and teamwork are vital in allowing a concise, clear, and complete exchange of ideas and insights, including the purpose and intention, and in having a balanced distribution of tasks within the group, increasing efficiency. However, it is not practiced often in doing a project or activity. The team, utilizing the concepts of collaboration and teamwork, communicated through different mediums, such as social media platforms with video and voice features, and sharing of documents. The liaison officer initiates the conversations on how to divide the tasks required for the group and the members simply share their ideas if they think there is a more efficient manner of dividing the tasks. Over the weeks the group has been working together, there are effective communication aspects that are practiced, including mutual respect, sharing of knowledge, frequency, consistency, and timeliness, which Kim et al. (2019) state as elements of effective communication. <br><br>However, openness appears to be a barrier, since I, personally, sometimes struggle in sharing my thoughts, especially when the majority of the group members have already agreed on a statement by another member. According to Kapur (2018), there are times group members are uninterested in communicating due to feelings of fear, mistrust, anger, or annoyance, arising within their minds. This then poses a potential issue for the organization and the effectiveness of communication within the group.<br><br>Ultimately, the group successfully accomplishes the requirements expected through the liaison officer, taking the lead in distributing tasks and allowing suggestions to be freely expressed. However, to improve the process, the group perhaps needs to become more open. I, on a personal level, also need to practice this openness more often to strengthen the team dynamics of the group. In future group works or tasks handed to us, I will be more transparent in what I think can be an option in accomplishing a task. <br><br>2. Reflecting on one’s practice, as a healthcare professional, is essential to the continuous improvement of the service we provide to clients and patients of different ages. This also allows us to identify and appreciate different experiences—positive and negative—to better understand various ways for development both in practice and theory. Reflections both constitute improvements in looking back on what we did and thinking ahead about what we can and must improve. Among the many models of reflective practice, my favorite would be Gibb’s Reflective Cycle, as this presents a simpler and clearer process of how we can reflect without leaving elements that are necessary. Having the different stages—Description, Feelings, Evaluation, Analysis, Conclusion, and Action Plan—form a cycle is one of my favorite features, as it emphasizes that reflection never ends, and continuous improvement is needed, especially for me, as I must develop to provide quality service delivery to my future clients. This model can be applied in my future practice as an aspiring occupational therapist, to recognize the aspects I lack in fulfilling my role in healthcare, to identify the consequences of my actions, to validate what I have felt upon looking back on the experience, and to form concrete actions on what I can do now and on a long-term basis to improve, not only as an individual but also as an essential team member in the healthcare setting and as a professional in the field of occupational therapy.<br><br><strong>References<br></strong>Kapur, R.          <description><![CDATA[<div>1.) The group collaborated to produce a comprehensive subjective assessment or interview for a patient with autism spectrum disorder. Due to the nature of the project, it is vital for the group's members to be able to communicate effectively with one another and to exemplify teamwork and cooperation to come up with a seamless and consistent set of strategies in tackling the interview with the patient. With how the group collaborated in this project, it gave an internal feedback within ourselves, as members of the group, in pondering the rights and wrongs that may have manifested during the project. Some members were quick to their feet and some needed an extra push, but eventually and ultimately, all were competent in their respective fields of study. Moving forward, the project provided a glimpse of how a rehab team would work together in assessing or managing a patient. Now with this newfound knowledge and experience, as a group, there can be more avenues in utilizing our skills in our own respective fields and come to work together in future cases, along with putting the utmost importance towards good teamwork and collaboration with one another. <br><br>2.) As a future health care professional, it is necessary to be in line with one's own capabilities as well as one's own faults and difficulties. This is important, especially when I find myself in the clinical setting in the future as these reflections can produce more self-awareness in my practice and what I can work or improve on to provide a more well-rounded care for the patient/s. Reading some of the models of reflective practice as well as from the internet, Gibb's Reflective Cycle (University of Edinburgh, 2020) is probably the one that piqued my interest. The model is comprehensive and detailed without having to be too complex for everyday use. It is broken down enough for me to see the individual aspects in my work/practice, but cohesive enough to make it simple enough for me to be able to mentally practice it in everyday use.<br><br><strong>References:<br></strong>University of Edinburgh.          <description><![CDATA[<ol><li>Collaborative learning is an approach where everyone functions efficiently and effectively. It is effective because, the weakness of an individual is being covered up by a strength of another individual in a team. And so, they are attaining the outcome they desire efficiently. Especially when everybody is speaking up and manifesting their leadership skills. Meanwhile, it could be related to Patient Centered Care because it is where all health professionals come up with a hollistic solution that benefits the patient’s well-being. Even if they have different areas of expertise.&nbsp;</li><li>I think that I can apply it both personally and professionally, because I believe that life involves endless understanding to people’s perspectives. We are taught about individual differences, therefore we must respect each other’s way of adapting in circumstances. But, if they are doing it the unethical way, then that’s the right time to step in and teach them the right way. With that said, we must also know when we should be accountable for our mistakes, and always be open to learning.</li><li>&nbsp;As future sports scientist, we are here to be responsible for prehabilitation of athletes or individuals. We reduce the risk of injuries by teaching them the proper biomechanics of their sports and general exercise. It is also our duty to regulate our body systems by promoting the benefits of exercise in our body such as reducing the risks of CVD, stroke, hypertension, etc.&nbsp;</li><li>Clarity of roles in a team will prevent a conflict between members, which can also affect the performance of the team towards their outcome. It will also be more productive since everyone can focus down to minor details of their own roles, and come up with safest solution for our clients. This will also establish the value of trust in a team, which is important in patient centered care.</li></ol><div><br></div>]]></description>

         <description><![CDATA[<div><em>1. Reflect on the importance of communication and teamwork in your own IPE group. How did the team communicate? What went well and how can it be improved? Use the What? So what? Now what? reflection model by Rolfe et al (2001).</em></div><div><br></div><div>Communication and teamwork is essential in any field. In our IPE group, both of these are greatly valued to be able to effectively perform as a team. In terms of communication, I think what sets our group apart is that everyone can share their own ideas on a certain topic. There is an unspoken rule of shared leadership between the members. Whenever we would communicate, we’d have goals in mind so that we would be able to share our opinions. Mutual respect is also evident when we would work together as it encourages diversity of our ideas. Because of this, the team is able to form cohesive and concise answers to the questions and we are able to learn from each other. The team was able to build relationships and put emphasis on trusting one another on certain topics that we have more background knowledge. For me, communication and teamwork within the team can be improved by also providing constructive feedback with each other. I noticed that we would often agree on each other’s statements and I think it would be great to also positively critique each other in order to know what part we can improve on ourselves.&nbsp;</div><div><br></div><div><em>2. Why should we reflect on practice? What’s your favorite model of reflective practice? Explain. How can you apply this model in your future practice?</em></div><div><em>&nbsp;</em></div><div>We should reflect on practice in order to identify what we can improve on in the future. Reflective practice would also help in gaining a deeper understanding of yourself and the people you work with. This provides an avenue for a professional to analyze situations and rethink their actions. Ultimately, it is a means for us to apply theory to professional practice and prevent mistakes from happening again. For me, I became fond of Gibb’s Reflective Cycle. This model emphasizes the need to reflect and think about an experience and to learn from that experience. I liked how it shows that reflective practice is a cycle and should never end, since we should always be learning from our past experiences. It involves guide questions that would help in my own reflection. This model can be applied in my future practice by following its 6 stages: description, feelings, evaluation, analysis, conclusion, and action plan. As future professionals, we should be able to describe what happened, identify what we felt and thought about the situation, evaluate the outcome whether it’s good or bad, use critical thinking in considering different perspectives, think of solutions, and consider what would be done differently next time.&nbsp;</div><div><br><br></div>]]></description>

         <description><![CDATA[<div>1. Communication and teamwork are vital when it comes to groupworks especially during the Synergy activity. Although I am the appointed Liaison Officer of the group, every member was accounted for and everyone provided suggestions, opinions and their own perspectives. We openly communicated with each other not only during the activities themselves but also even after the activities. We used communication and teamwork to find common ground and work towards a common goal we all want to achieve. Everyone’s opinions and ideas were taken into consideration before we formulated a plan for the activity. Although communication and teamwork was established within the group, it could have been improved if more members were willing to initiate and to step up especially during the activities. During activities, there were only a few people volunteering to become the group’s representative during the activities. This shows that due to us only meeting online and given that this is our first activity face to face, people were still getting to know each other and some people were shy and did not want to take on the responsibility of leading the team for an activity. With this, we would try to be more initiative so that during times where the liaison officer is not available or not adept with what is expected from the task, there would be people who would take the responsibility of leading the team.&nbsp;<br><br>2. Reflecting on practice is important to see what your actions or what has happened from a larger perspective. It is not enough to just do without analyzing and acknowledging past experiences. By doing so, it makes a person more responsible for the consequences of their actions, learn from their mistakes and grow as a person and/or as a professional. From the models discussed my favorite is Gibb’s Reflective cycle. This model, although similar to the other models, this consists of 6 parts namely Description, Feelings, Evaluation, Analysis, Conclusions and Action plan. These parts are a way for the person to describe the situation, check how he/she feels, evaluate the experience, analyze the situation to arrive at a conclusion and to prepare and adjust if the same or similar situation arise in the future. As a future Physical Therapist, I can apply this model in checking and evaluating my actions through the experiences I go through in my future practice. I would be able to objectively evaluate the situation so that I can act accordingly and be aware of my actions. With this I would be able to grow into a better Physical Therapist and as a person by reflecting on my actions, retaining what I did good and improving on aspects wherein I can do better.&nbsp;<br><br></div>]]></description>

         <description><![CDATA[<ol><li>At first, due to the online setting it was difficult to communicate with each other because not everyone can show their true selves in zoom meetings, because for me, I am afraid to be judged since we are like strangers to each other. And we all know that communication is important, so thank God that someone had to break the ice, and that was our leader. And so, as time passes by we are getting along better and better, especially when we had our first face to face. And for me, that was so effective because finally we had a chance to show them our true vibes, talked to each other personally, and I could say that the experience was so fun. And I think the best solution for effective communication is more frequent face to face meetings or activities just like that. Because we are getting to know each other more and their skills.&nbsp;</li><li>We should continuously reflect with our practice, because I believe that life is a trial and error process. And we have to reflect what are the things we could improve, so that we would be more efficiently next time. Hence, I think the best reflective practice Model for me is Gibb’s reflective cycle. Since it emphasizes that we should keep on reflecting on our experiences, and more importantly to keep on adapting with health innovations through time. And success is measured on how we adapt to these kind of innovations. And I also appreciate that this model is a mixture of objective and subjective perspective because it considers the feelings and emotions aside from the mere situation. Then it also makes us think critically about what happened, and come up with a concrete plan so that errors can be minimized and less stressful practices the next time.&nbsp;</li></ol><div><br></div>]]></description>

What are the key insights I learned from the session on “Learning Collaboratively” and “Patient Centered Care”?&nbsp;</em></div><div><br></div><div>Learning collaboratively means being able to accomplish a task with a pair or group of people. It is an approach that involves intellectual effort of the individuals involved. As we browse through the topics, I realized that collaborative learning provides many benefits that could help you develop into a better individual. These benefits include oral communication, higher level thinking, leadership, and self-management skills. This maximizes learning experiences all while improving self-esteem, confidence, social, and interpersonal skills. Additionally, collaborative learning would also aid in providing patient centered care since it focuses on respectful and responsive care that ensures appropriate clinical decision making. Patient centered care focuses on the patients having an active role in their own care, which would eventually aid in their recovery.&nbsp;</div><div><br></div><div><em>2. In which areas, personal or professionally, will I use what I learned?</em></div><div><br></div><div>I am able to use our learnings both personally and professionally in areas that includes collaboration, communication, and in ensuring optimum performance in taking care of ourselves and our future patients. Personally, I can apply this when approaching other people, especially our future patients. Keeping an open mind to others opinions would defnitely aid my social skills and increase my competency as a clinician. As a professional, I will be able to remember that we should always be respectful of our patients and realize that they are the biggest part in helping them make a full recovery from their conditions. These lessons taught me that as clinicians, we are just their guides, but it is up to the patients to take control of their recovery and we should always respect and consider that.&nbsp;</div><div><br></div><div><em>3. What are professional and/or non-professional roles and responsibilities in your chosen field?&nbsp;</em></div><div><br></div><div>As an aspiring Speech-Language Pathologist, we provide support and guidance to patients who have difficulties with communication and swallowing. As SLPs, we work to assess, prevent, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders. Additionally, we also serve as a guide to support our patients in doing the treatments at home. We lead them to our goals that we set for them and help them overcome or deal with their difficulties. As support, it is our responsibility as their clinicians that they should not be left behind with activities that other people normally do and reassure them that we are also doing our best for a full recovery. &nbsp;</div><div><br></div><div><em>4. What is the value of role clarity among members of the team?</em></div><div><br></div><div>In collaborative learning and interprofessional education, it is important that each members have clear goals and responsibilities. As a team working together on a patient, it is important to prevent overlap of goals to efficiently work together. The team should be able to maximize the patient’s time and effort into learning or relearning skills. Further, this would also help prevent conflicts within the team that may affect the progress of the patient. Conflicts may include miscommunication and misdiagnosis which would may result into much more bigger problems.&nbsp;</div><div><br><br></div>]]></description>

         <description><![CDATA[<div>1.&nbsp; Our team maximized on the use of teamwork and organization in our communication throughout the several activities for this course. Given that we can barely meet each other face to face, we were able to use online platforms to properly communicate with each other. The position of one liaison officer really helped the team in delegating tasks and the overall organization of the task management. Moreover, the team used open communication in dealing with future tasks and we were able to discuss them beforehand which really helped the team in preparing. In future tasks it will be of great importance to the team to further uphold open communication and to be more vocal about certain issues or problems such that everyone in the team can chip in and help in resolving these problems. &nbsp;</div><div>2. Reflective practice is essential in a professional’s arsenal in the workforce. Reflection provides a professional self-awareness regarding their performance and could be a great deal of help in improving on their strengths and weaknesses. Furthermore, it can also help a professional in reducing risks and problems for future tasks and further improving the quality of work. The What? So what? Now what? reflection model by Rolfe et al (2001) can really helpful for my practice as it simply dissects my actions and looks into what has transpired and what can be improved on in the future.&nbsp; What i particularly like about this model is the importance of implications of the present to the future. What is important in reflective practice is that we do not only look at the present but in how it can affect the future. Thus, with this model I am able to see my present actions and hopefully improve on them for the future.&nbsp;</div>]]></description>

         <description><![CDATA[<div>1) Communicating effectively in a team is crucial as this allows for open discussions consisting of respect and trust in each of the team members’ capabilities and strengths to complete the assigned tasks. In order to achieve the purpose, each member of the team should utilize their own set of skills and work hard to play the role that they believe suits them best. In return, this will produce good results such as being able to achieve the shared goals of the team and being able to finish the task before its due.<br>Thinking about it now, I know that there is open communication within the team. This allowed us to be united and to play our roles very well. In addition, this has drawn forth the concept of self-awareness. We became capable of examining our strengths and limitations which is why we can analyze the situation, identify the problem and give an appropriate judgement or solution.&nbsp; Although things are going smoothly I still think the way we communicate with one another can still be improved especially in encouraging engagement in specific activities since most are still shy or afraid to step up. <br><br>2) We should reflect on practice as this will allow for a better understanding of oneself such as one’s strengths and limitations, roles and responsibilities, and effect of one’s actions/behaviors on others. Although, the models provided in reflective practice has its similarities–-my most favorite is the Kolb’s Learning Cycle. I will never forget the line that I encountered when I was learning about this model,<em>"Learning is the process whereby knowledge is created through the transformation of experience" (Kolb, 1984, p. 38). </em>This model has made me realize that the experiences that we will have will either be good or bad but it will allow us to gain knowledge that will be crucial in becoming a good professional. I can apply this model in my future practice as an SLP through understanding that when I become an SLP in the future I’ll constantly encounter new experiences that will test my capabilities so I must always apply reflective observation in order to come up with a solution to the situation that I am in. Through this experience I should be able to practice coming up with new ideas/ modifications on the spot depending on the client’s needs and then I should experiment and use this new idea/ modified idea in dealing with my other patients.</div><div><br><br></div><div><br><br></div><div><br></div>]]></description>
