         <description><![CDATA[<p>The transition cycle connects four phases about supporting students through change. </p><p>The first phase,<strong> imagine phase,</strong>  is learning about long term things that might create patterns, structures in place already, etc.</p><p>During the <strong>connect phase</strong>, this is when adults are connecting with students and creating that relationship. </p><p>The <strong>act phase</strong> is planning and making an action of how to help.</p><p>The <strong>assess phase</strong> is figuring out what you can do now, to help students through change. </p>]]></description>
