3.  Assume you were passing by when this took place. Later when the patient  leaves, you are going to encourage the patient to report the incident. Inform the patient of the consequences of that ethical and legal issue. Tell them it is confidential because their business need to be private and nobody else needs to hear about it You can file for a lawsuit and they can also loss they business relationship because people won't trust them again with their personal business.
