This is the first time that she has stood up to him, and is a pivotal moment in the play. Instead of putting on 'Paper doll' and dancing, a passive aggressive reaction, she is calling him out for what he is. A rat. 'Sewer' has connotations of dirt and trash, and that exemplifies Catherine's feelings for him. He has fallen so far, and this has created a conflict between them.
