Recommend:
-ensure all the workers trained and aware of WAH hazard.
-supervisor ensures implement FPP on site and stand by supervision for high risk work activity.
-Ensure all workers worn they are fall protection equipment and secure correctly.
-follow safe work procedure during scaffold erection or dismantling.

Hazard - Falling from truck and injury to body.
Recommendation - Use of proper PPE and equipment eg. a ladder.

6) Toilet area at site.
Hazard - begin struck by moving vehicel or equipment.
Recommendation - Portable toilet should be well  barricaged therefore moving vehicels or equipments would not sturck.

7) Excavator oerations.
Hazard - engulfment of worker cauing suffocatation and death.
Recommendation - During excavator operations a banksman and signalman should be guilding the excavator operatorer.

8) Forklift operatorer carrying out an unauthorised operation of lifting a worker with forklift.
Hazard - Fall from heights and major injury.
Recommendations - Such operator should be sent for refresher course and repeat offenders must given a penalty. Supervisors must brief operator on SWP of operating a forklift.

Recommedations - Workers must use proper PPE and body harness when working on scaffold platforms. Supervisors should breif workers on SWP .
