         <description><![CDATA[<p><strong>Initial Observation and Inaction</strong>: An 81-year-old DNR patient's oxygen saturation drops to 73%. The registered nurse (RN) applies a 100% non-rebreathing mask, but the saturation only rises to 80%. A senior nurse observes the RN and a student nurse looking puzzled at the patient’s bedside and offers assistance. When asked if the doctor has been informed, the registered nurse looks hesitant and reports that the patient is DNR.</p><p><br></p><p>The senior nurse, observing the RN and student’s struggle, proactively intervenes. Upon learning that the patient's oxygen saturation is 73% and only 80% with a non-rebreather, she immediately initiates a 'Call-Out' by asking about the patient's consciousness and quickly assessing the patient’s alertness and air entry.</p><p><br></p><p>She then directs the RN to contact the doctor, using an implied 'SBAR' (Situation, Background, Assessment, Recommendation) framework by focusing on the critical information (desaturation, current intervention, her assessment of air entry) and recommending increased oxygen and family notification.</p><p><br></p><p><strong>Clear and Empathetic Family Communication (Handoff &amp; Teach-Back)</strong>: The senior nurse takes charge of communicating with the family. She takes the phone from the confused helper and clearly explains the patient's condition, confirming that the doctor has been notified and that the situation is improving with increased oxygen, though still below normal. She reassures the family that the patient is responsive and promises to provide updates, asking if they would like to come to the hospital.</p><p> </p>]]></description>
