The Explosion of the Ariane 5 :
-Rocket explosion 40 seconds after lift off. Caused by software error. 64 bit floating point number relating to the horizontal velocity of the rocket with respect to the platform was converted to a 16 bit signed integer. The number was larger than 32,767, the largest integer storeable in a 16 bit signed integer, and thus the conversion failed. 

Fiat Chrysler(2017) -Recalled over a million trucks due to a software glitch that was linked to at least one deathly crash. The issue was caused by erroneous code that temporarily disabled airbags and seat belt functionality.

- the attack occurred after the deputy president clicked on a malicious link via email that caused keyloggers and other malware to be installed. The attack was kept as a secret before being finally unveiled in 2012.
 As a cause of that, the company’s privacy was heavily breached with files being accessed and email pilfering.

Irish Electronic voting Failure 2004   

This information technology failure is perhaps the most famous in irish polotics and is widely regarded as the biggest waste of the tax payers money ever recorded in irish history with the failure causing the irish economy 55 million euro. It was implemented by Taoiseach Bertie Ahern who wanted to make the irish voting system more efficient.

Reason For Failure: The implementation of the system was not well  recieved by the general public who worried that it would lead to a lack of transparency and also the elections could be open to manipulation if these electronic voting systems were used. It then transpired that the electronic voting machines could be easily hacked and manipulated. The system was never implemented and lead to 55 million euro of the tax payers money being wasted.

The Equifax social security hack
Equifax, one of the United States’ largest credit reporting agencies, announced that up to 143 million of their consumer records were stolen by hackers. Names, Social Security numbers, birth dates, and credit card numbers were all amongst the data stolen. Given that the population of the United States clocks in at 321 million, that means that approximately 50% of Americans could now find themselves in danger of identity theft or worse. Though the hack took place in May 2017, Equifax hid the story until early September, further outraging the public. As details of the hack have emerged, it quickly became clear that much of the damage done was a result of vast negligence on Equifax’s part.

Hawaii Sends Out a State-Wide False Alarm About a Missile Strike
In January 2018, the citizens of Hawaii were notified to take immediate cover in the face of an inbound ballistic missile strike. It turned out to be a false alarm, although it took over 30 minutes (and, presumably, several thousand heart attacks) before the alert was retracted. Investigations found that while the problem was largely due to human error, there were “troubling” design flaws in the Hawaii Emergency Management Agency’s alert origination software.

A giant agricultural co-operative, Tri Valley bought at least $6 million worth of ERP software and services from Oracle in 1996.  The reason of failure is  none of the software worked as promised and some of it could not be installed by Tri Valley. Oracle denied any wrong doing. Tri valley lost the court case and was left with a net loss of $30 million.

In 2018, doctors and hospital staff of the Wales NHS experienced a widespread computer failure that led to them being unable to access patient files. According to the National Cyber Security Centre, the failure was due to technical issues as opposed to a cyber attack yet it still caused wide disruption as GPs were unable to access blood and X-Ray results. It also caused a backlog as patients could not be contacted to cancel appointments, and notes could not be typed up and saved on NHS systems.

In March 2018, an autonomous Uber vehicle was involved in an incident with a 49-year-old woman in Arizona, who was walking at a crosswalk. The self-driving software installed on the vehicle had detected the person and the bicycle, but decided that "it didn't need to react right away". This indicates that the software was not tuned properly. As a result, the vehicle hit the woman, and the woman was later declared dead at a hospital. This incident appeared to be the first reported fatal crash involving a self-driving vehicle and a pedestrian in the US.

1983 Soviet nuclear false alarm incident

During the Cold War, the hostile relationship between the USA and the USSR resulted in both sides developing early warning systems for any impending nuclear attacks from the opposing nation. The Soviet strategy for an impending attack was a compulsory nuclear counter-attack. On September 26 1983, the Soviet's early warning system incorrectly detected a nuclear attack from the US. However, no nuclear counter-attack was conducted because the officer on duty correctly predicted the failure of the system and did not report the warnings to his superiors. 350 000 people had to be relocated. Area around had to be declared as inhabitable.
