1.  How is the PAR level on the supply carts being set?
2.  How is the PAR level in each OR being decided upon?
3.  Who is responsible for stocking the supply carts and the OR rooms?
4.  When stocking if things are noticed to be over the PAR level, how is that remedied?
5.  What happens to the supplies when they are out of date?  
6. Are  the number of out dates being wasted tracked and how so?  
7.  Do the number of supplies that have out dated and been wasted affect the number the unit continues to order?

1. Reassess OR kits for items that often go unused and placed into the medical waste stream. 
2.  Keeping detailed records of supplies when they have outdated and not ordering supplies when they are not being used just because someone "may" need them.
3.  Educating staff.

1.  Are the lists of each of the physicians supplies kept up to date on a regular basis?
2.  Are the employees (Nurses and Techs) trained to know what and how much of an item to stock and the importance of not over stocking "just in case"?
3.  Would we run out of a certain supply if a physician decided to use something that haven't used in a long time because it was something they used to use?
4.  Is Central Supply notifying the unit when items are not being used on a regular basis when they see this during stocking of the carts?
5.  Communication between all staff (RN's, CST's, Physicians) is imperative to ensure the proper supplies are available without over spending and keeping items on hand that aren't used that often outdate and cost the unit more money which is then passed on to the consumer, insurance company etc.
