Reliance on technology for changes in patient assessment.

Program dependent on the parents/caregivers' engagement in the child care for data input and compliance. Programs reliant on individual data entry leave room for error (McGonigle & Mastrian, 2016).

Ability to perform physical exam and listen to breath/cardiac sounds limited. This remains one of the biggest drawbacks with telemedicine in general (McGonigle & Mastrian, 2016).

There is an obvious learning curve for assessment skills via video/teleconference.

Following corrective surgery for HLHS, families are discharged home with a Microsoft Tablet. Patients are still considered to be very high acuity and are at high risk for complications and decompensation. Multiple times a day the family inputs the patient's vital signs into the app. Data is then stored and analyzed in the cloud. The app allows for the care team to be instantly alerted when data is inputed and if there are abnormal findings. The app also allows for video assessments and video conferencing with the care team as needed.

The Cardiac High Acuity Monitoring Program (CHAMP) is an innovative form of telemedicine that allows families to return home with their child sooner than has ever previously been possible (Ray, 2016). The application is advanced in the area of telemedicine since it incorporates three different domains of telehealth medicine: live videoconferencing, store-and forward capabilties, and remote patient monitoring capabilities (Telehealth Resources Center, 2016). The app allows for post-operative cardiac infants to be discharged from the hospital to home, but still recieve close monitoring.

I think this is a great advancement in technology and care delivery for babies with HLHS. Cardiac infants that have previously required constant hospital stays and visits can now return home with families. I think the advancment gives hope to families with critically ill infants to lead a more normal life with the reassurance of continued monitoring and easily accessible resorces when needed.
