The name of the Sexually Transmitted Infection (STI), that I have chosen is Syphilis. Syphilis is a bacterial infection usually spread by sexual contact, which starts as a painless sore and can spread from person to person. It can cause serious health problems if left untreated and brushed off. Syphilis has many stages (primary, secondary, latent, and tertiary); each stage can have different signs and symptoms. In this project, the prevention as long as the signs and symptoms are shown, in order to educate, prevent, and bring awareness.

The pathogen that causes Infection

Like every other infection, Syphilis is caused by bacteria that develops into something harmful. The bacteria that causes Syphilis is Treponema pallidum. Treponema pallidum is a spiral-shaped bacterium that incubates in the body for up to 6 weeks, then produces a small pailesion called a chancre. A chancre is a sore that is painless but oozes pus and fluids that contain Syphilis. When the chancre is present the STI, Syphilis, is transmissible to others via the chancre. The formation of the chancre varies from gender. With males, the chancre forms on the penis but with females it develops within the vagina and can go unnoticed. The chancre will heal within 6 weeks, but the infection is still in the body.

The primary stage of Syphilis is the first signs of symptoms of this STI. There will be single sores or multiple, that is called chancre which was shown on the slide before. The chancre location is where syphilis has entered the body, and usually occurs on or around the: penis, vagina, anus, rectum, and lips or mouth. The sores (chancre) are painless, round, and usually firm since the sore is painless it lasts up to 6 weeks. After it is healed, it seems as if the infection has run its course but that was just the first stage. Without treatment in the primary stage, the infection will go into stage 2 which is the secondary stage.

During the second stage, you may have skin rashes or sores in your mouth, vagina, or anus. This stage usually starts with a rash in one or multiple parts of your body. The rash can show up when your primary sore (chancre) is healing or weeks after healing. The rash can be on your hands and the bottom of your feet with a rough, red, or brownish tint. Unlike other rashes, this one will not itch and at times is so faint it will go unnoticed. Other symptoms of the secondary stage are fever, swollen lymph nodes, sore throat, patchy hair loss, headaches, weight loss, muscle aches, and fatigue. The symptoms of the secondary stage will go away whether you receive treatment, but without the right treatment, it will develop into the latent and possibly the tertiary stage.

The latent (hidden) stage of Syphilis is a period when there are no visible signs or symptoms of the infection, and be as brief as a year but as long as 5-20 years. Without treatment, syphilis will stay in the body even after there are no signs or symptoms to be seen or felt. Since it is in the latent stage, many people may think that it has run its course or they just got lucky with it while it is the exact opposite. Without the proper treatment, syphilis can and will stay in your body for years laying dormant until it possibly develops in the tertiary stage. The tertiary stage will result in very harmful and life-threatening conditions.

Most people with untreated syphilis do not develop tertiary syphilis. Although, if it does develop, it can affect many different organ systems like the heart, blood vessels, and the brain and nervous system. Having tertiary syphilis is very serious and life-threatening and would appear 10-30 years after the infection began. With tertiary syphilis the third and final stage, the disease damages your internal organs and can result in death. About 15-30% of people infected with syphilis who do not get treated will develop tertiary syphilis.

Effects on Newborns

Congenital syphilis can cause problems for a newborn during pregnancy and after birth. During pregnancy, the effects on the fetus can be miscarriage, premature birth, stillbirth, and complications with the umbilical cord and placenta. After birth, the effects can be fever, problems with the spleen and liver, anemia, rashes on the mouth, genitals, anus, hands, and bottoms of feet, meningitis, and the most severe neonatal death. All of these depend on how long the mother was infected with syphilis while pregnant and when treatment was given. If treatment for the newborn is not given right away congenital syphilis can cause problems later down the road. Including vision problems, hearing, developmental delays, problems with the nervous system, bones, and joints, as well as paralyzed arms or seizures.

Without treatment of syphilis, the long-term effects are detrimental and can cause problems that can result in not only death but problems for children. Syphilis can cause damage throughout the body, and without treatment increases the risk of getting HIV infection. The long-term effects on the neurological system include a stroke, meningitis, hearing loss, vision problems, dementia, loss of pain and temperature sensations, sexual dysfunction in men, and bladder incontinence. The cardiovascular problems that may arise are bulging and swelling of the aorta and damage to heart valves. While most of these can be treated with medication, most that were mentioned cause greater problems from the effects of syphilis.

Syphilis can be found and diagnosed with blood tests and lumbar punctures to test cerebrospinal fluid. When diagnosed with syphilis and treated in its early stages (primary, secondary, and latent) the treatment is a single injection of penicillin. If one has had syphilis for more than a year, additional doses may be needed. Penicillin is an antibiotic medication that can kill syphilis, but if allergic there is penicillin desensitization. If a pregnant woman has syphilis and the newborn can contract it, will receive the same treatment. On the first day of treatment, symptoms may include fever, chills, nausea, achy pain, and a headache, these last no more than a day.

Since there is no vaccine for Syphilis, other than the treatment of penicillin, there are ways to prevent the spread and contraction of syphilis. The most certain way to present the infection is to not have sex, since it is a sexually transmitted disease. If sexual intercourse is taking place, the use of a latex condom can reduce and prevent the infection. Another way of avoiding and preventing the infection is if sexual intercourse is taking place and condoms are not being used, testing for STI is very important. If testing comes back positive for syphilis or other STIs, the partners within the last 3 months to a year should be notified and tested as well.
