         <description><![CDATA[<div>Jane is 35 years old working full time and taking care of her father Roger who is 74 since her mum Catherine, left fifteen years ago.  Roger has severe symptoms of dementia which are getting worse. Roger and Jane have always declined any assessment or support until now. Jane reports that Roger has become disorientated to time and place and is awake through the night time which affects her ability to sleep and function at work. Roger is at risk of falls especially down the stairs. Jane also advised that when she is at work she has come home to find Roger missing for hours and when he does return he is not always wearing weather appropriate clothes and sometimes he goes out on his bike. Jane holds Power of Attorney for Roger in terms of his health and welfare decisions as well as for his finances. However, Jane does not actually own the house it belongs to Roger. A home based care package was trialled but this was found not to have worked as Roger objected to different care workers arriving at different times. Jane removed the key to their house from the key safe so care workers could not get in. There was no scope for support from OACMHT through a medication review for possible anti- psychotic medication as Roger has a heart condition. The health care professionals (G.P. annd members of the OACMHT) feel that Roger should be placed in a care home. Roger has always objected to going into a care home and voices his objections to leaving his home. Jane has advised that she does not want Roger to go into a care home for many reasons but one of which is that she would have to leave the house as it would be sold to pay for fees for his care home placement......and she holds POA for health and welfare decision......<br>* Older Adult Community Mental Health Team (OACMHT)</div>]]></description>
