Update on 18 Feb briefing to all on Overseas Vacc/ FWMOM Care App/ ICP Booster/ STVP/ Dont turn away pts/ Covid Naive

Just to update the rest on what was briefed earlier.

1) Overseas Vaccination
Instead of asking if serology has been performed, ask if MOP have visited a clinic in Sg to get their vaccination records ingested.

Digitally verifiable overseas vaccination from selected countries does not require serology. But it does not mean that we can use MOP e-certs as evidence of vaccination. They will need to go to a clinic first.

Inform MOP to confirm with source/clinic when their records have been uploaded. May take up to a week for it to be reflected properly. If low loads and there's urgency to vaccinate, ask duty Dr to check.

2) FWMOMCare App for migrant workers. All their details ie: address, dob, covid infection status, discharge memo, vaccination or ART records are in there. Ask MW to show it to u if u require verification.

3) ICP Booster
Earlier today, we had an ICP MOP, presented with a memo from specialist to take booster. M-M-M+M. As MOP has completed the enhanced primary series, the 4th dose count as booster 0.25mls instead of 0.5mls. There may be more of this group coming in.

4) Feedback that our VC staff turned away MOP or directed to other VC. Do not turn away MOP unnecessarily. If unsure, check with OM first.

5) AG+ interval 90 days, minimum 28 days with <del>FA approval</del>. (Duty Manager approval) Official comms is <del>90</del> days please (28 days). If AG+ not found in Healthhub and 'Known Alerts', consider as Covid naive.

6) Short Term Visit Pass (STVP). Minimum interval is 60 days. If < 60, inform MOP to extend pass. Do not vaccinate.

Morning Team,

*[For Attention and Reminder]*

Yesterday one of the Vaccination Centre (not ours) had a vac error whereby a 74 yo non-immunocompromised MOP was being administered a 4th dose. Both the reg staff and vaccinator had *assumed* that the MOP had yet to take his B1. There are a few areas to highlight:

1. When reg staff could not register the MOP, *pls pay special attention to check thoroughly* and not to simply register the MOP manually;

2. When the Vaccinator could not order a B1 in the system, *pls check with the Nurse Manager/Doctor* before the needle goes into the skin. Do not ignore and proceed to jab without CLARIFYING as what was done cannot be undone. We have the duty to ensure MOPs’ safety; and

3. All reg staff and vaccinator are to *scroll down the page to view all the doses in the vaccination History*. Without scrolling down, staff only see Dose 1 and Dose 2 in the screen so *pls don’t assume.*

For your cooperation, pls.

Persons with moderate to severe immunocompromise are recommended to receive four doses of the mRNA COVID-19 vaccines, comprising a three-dose 3 enhanced primary series and a booster dose for the fourth dose. (see Annex A for Summary of Vaccination Recommendations). To avoid any doubt, there is currently no recommendation for a fifth vaccine dose in this group.

However, some immunocompromised persons may have followed the vaccination regime recommended for the general population (i.e. received the third dose as a booster dose after receiving the first two doses as part of the primary series).

Medical practitioners with patients who have followed this regime should remind these patients to receive a fourth dose (i.e. a second booster dose) of the vaccine at around five months after their third dose. Vaccination can be received either in a community vaccination site with a completed referral form or at the patients treating healthcare institution, if available. Please use the revised referral form in Annex B to refer the patients to community vaccination site.

Patients may walk-in to their community vaccination sites with the completed referral form to receive the additional doses.

The recommendation for immunocompromised persons to receive four doses applies to persons aged 12 years and above. Persons aged 5 to 11 years who are immunocompromised are recommended three doses as part of the enhanced primary series, but no booster dose is recommended for this age group at this time.

Refer to Annex A & B

This is an updates medical practitioners on the following revisions regarding the contraindications to receiving mRNA COVID-19 vaccines authorised for use under the Health Sciences Authority’s Pandemic Special Access Route

a. Persons with severe allergies (including anaphylaxis) to other vaccines can receive the mRNA vaccines. Local and international safety data now indicate more clearly that persons with severe allergies to other vaccines can be safely vaccinated with mRNA vaccines. There is no longer any requirement for such persons to undergo a review by a specialist to determine their suitability for vaccination.

b. Polysorbate allergy is removed as a contraindication to receiving a mRNA vaccine. The mRNA vaccines do not contain polysorbate. The initial precaution around polysorbate allergy (e.g. to other vaccines) was out of concern over possible cross-reactivity with polyethylene glycol (PEG) due to their structural similarities and case series based on skin test. To date, global evidence that anaphylaxis to the mRNA vaccines is predominantly caused by PEG (and cross-reactivity with polysorbate) has not been conclusive. International public health authorities, such as the US CDC, have moved away from citing polysorbate allergy as a contraindication.

c. Persons who are known to be allergic to polyethylene glycol (PEG), which is a component of mRNA vaccines, will however continue to not be suitable for the mRNA vaccines. This is encompassed in the existing contraindication for persons with allergy to mRNA vaccines and their components.
