For example, one of the most critical uses of informatics in healthcare is the implementation of the Electronic Health Record. Many patients can now access this information in real time through a desktop computer or app on their cell phone. To prevent errors and make it easier to use, most entities are using a system that collaborates with other essential healthcare programs and equipment. In the hospital settings we are able to take a patient's blood glucose at the bedside and it will automatically upload to the patient's chart. This bridges the gap for error opportunity and also aids in preventing delays in care so that the information is recorded in realtime. The most important point to takeaway is that health informatics is apart of our everyday life and it is our responsible to continue to work on improving how all of this comes together so that we can be more efficient and have better patient outcomes.

The main takeaway explained how a piece of data only has meaning when it has background (or information) to provide context and meaning to what that data point means. After you gathered information, knowledge creates additional understanding with programs that provide algorithms with computers. For example. our charting system in the hospital will flag a patient at risk for sepsis when data is input such as blood pressure, temperature, labs, etc. The wisdom portion of technologies include the alerts it will provide to the medical staff to identify these patients and continue to monitor specific indicators that the computer has been programmed to notice.

Many, including myself, associate virtual health visits as the main use in telemedicine. With these devices, providers are able to have a better understanding of how their patients are doing via an objective standpoint such as blood glucose trends, heart rhythm/rate monitors, and blood pressure monitors. The EHR systems implemented in primary care work together with the third party programs to access all of these results within the patient chart after being reviewed by either a home health agency or a remote diagnostic testing facility. Personally, I have benefitted greatly from the education portion through telemedicine and the EHR. Even as a healthcare worker it is easy to get overwhelmed with the information given or instructions given with your primary care provider. Being able to go back to the provider's note and instruction as well as the listed 3rd party for further instruction is so helpful. There needs to be a better way to distinguish alarm significance. Another concern that exists are pop up alerts either on the EHR or pyxis. For example, when tylenol is removed there is an alert that shows the max dose for 24 hours. This creates another window for us to close out before taking the medication but it shows up for every dose of tylenol without incorporating specific patient information such as weight, dose, or last administration. Everyday we see the same messages pop up on our EHR/MAR when taking care of patients. It is easy to get lost in just ignoring messages and overriding warnings especially when we are busy (which is pretty much everyday). It is vital to patient safety for each of us to take these alerts seriously and take the time to review what is being presented to us. Alarm fatigue is another obstacle we face. All day long our monitors, IV pumps, and other machines make noise and it becomes background noise in our brains. Some of us go home and hear it in our sleep. Our healthcare system needs to do a better job of alerting to more serious concerns in a way to get our attention. The final A is Accountability. As providers we need to take responsibility for staying current in our practice with changes to guidelines, new published data, and new treatments. When looking at published data in healthcare, we know that information isn't technically current. Patient health information can be accessed by people who shouldn't be able to view them such as other healthcare workers not involved in the care as well as the potential of data breaching. Now that the information is electronically placed in a portal system there are potential for cyber hacking and exposure of health records. We now have a duty to protect our patients' information electronically. Are we ensuring patient privacy by closing their chart or logging out of the computer when we step away? Are we giving our password away to another user to access something because it is easier to do that than call the IT worker? Protecting patient information should be our number one goal.
