Ask more about the patient's life: what are her strengths? relationships with peers and family? support system? mental health history? <br><br>Task 2b: (3 mins) PAs would be stabilizing the patient after the gunshot wound and assisting in surgery and post-operative wound and medication care. PT would also help with wound care, passive movement in the hospital, maintaining movement that she has, pressure relief. Once out of surgery, PT would also help with spinal cord injury education, using equipment, fitting braces for legs in collaboration with P&amp;O for stabilization. Use of closed loop feedback to ensure message was received appropriately so can be addressed. <br><br><strong>Task 7 (5 mins): </strong><br>Information most appropriate to note with the team includes (one topic not addressed in this summary): Stated patient is going home, but didn't note how well patient is able to use the washroom. Didn't discuss what future treatment for the future. For spasticity, didn't state goals on how it was gonna be treated. Nice that the d/c talked about psychosocial aspects. Nothing much was noted on GC. Disabilities on ADL's were a bit vague, what AD was used? For community participation, what specific activities are accounted for? For outpatient therapy, will insurance cover for the therapy? Overall financial coverage is important to discuss d/c, but not all were listed on the d/c note.&nbsp; <br><br><strong>Task 6 (4 mins): <br>To improve communication/collaboration, not talking over each other, active listening, each discipline was trying to fulfill their own agenda, build up on each other's patient factors. Everyone had their own goal, just not very cohesive. Should discuss high priority items and work together to help the patient. <br></strong><br><strong>Task 5 (1min): <br>Factors that could affect the transition&nbsp; are complexity of care, differences in culture, lack of respectful atmosphere, and potential new providers and teams<br><br>Task 4 (5 mins): <br>Genetic counseling would serve useful due to the uncle's death, but could be bad timing because don't want to scare patient. Factors to consider are performance of ADL's: like using the washroom, attending school, and ability to have a child. Counseling and social work would aid in helping the patient. PT's could help in transfers and be as functional and optimal as possible once patient is D/C from hospital. <br><br>Task 3 (4 mins): <br>What sport the patient plays. Question for goals? Help aid in progression. Prior level of function: related to hobbies. Roles or responsibilities at home?
