         <description><![CDATA[<div>The tank's owner claimed that the tank holding the molasses was bombed by anarchists, but this was proved false. Ever since it was built, the tank leaked the substance and made strange noises when filled. The company, instead of replacing the tank, deiced to continue using it, as it would be ultimately cheaper. The cause of failure can be attributed to multiple people: the man who oversaw construction of the tank did not know how to read blueprints, the building of the tank itself was not inspected by professionals, and lastly, the U.S.I.A. decided to use the tank despite of the warning signs.</div>]]></description>

         <description><![CDATA[<div><br>This incident was preventable since warning signs and been seen since it was constructed, by only filling it with the right amount of molasses, repairing it, or making sure the right addition is made to ensure that it was built sturdy.No architects or engineers were ever consulted during the construction, and many corners were cut in order to complete the tank before the molasses lost its value due to prohibition. The company that owned it ignored the complaints about it by the people who lived in the surrounding areas.</div>]]></description>

         <description><![CDATA[<div>The cause of failure had been the poor construction and high stress the bridge had been under. The problem had first been spotted on the south arm in which two of the cords were bent. Instead of fixing the problem they continued with the project and more chords had been found bent which had then led to the collapse. The person responsible was Theodore Cooper, the American engineer behind this because of his lack of design, calculation and leadership.</div>]]></description>

         <description><![CDATA[<div>Black smoke was coming from the bottom right rocket booster which eventually evolved into a flame when the rocket took flight. The black smoke came from a leak in the rocket booster and this occurred due to a dysfunctional O-ring which lost its elasticity due to the extremely cold temperatures before takeoff. It was common knowledge that an O-ring didn't work properly in cold temperatures but this wasn't properly communicated between the engineers which lead to the catastrophic failure and death of the passengers on board.  </div>]]></description>

         <description><![CDATA[<div>A bridge titled the Quebec Bridge was under construction since 1887 to traverse between Sainte-Foy and Levis. During the winter the St Lawrence River would freeze up completely leaving its trade route cut off until the spring. The Quebec Bridge was feverishly created to subside the issue. However, it was on August 29, 1907, that a collapse happened. After the first failure, construction on a new bridge spearheaded by the government was made. Yet again, this bridge collapsed. September 11th, 1916 was the date of the 2nd incident.</div>]]></description>

         <description><![CDATA[<div>Tuesday, January 28,1986 At Cape Canaveral,Florida the spaceship challenger exploded  73 seconds after launching, the explosion happened while everyone was watching from t.v. Black smoke appeared of the right solid rocket booster. The smoke than disappeared and was replaced by flames. The flames grew and engulfed the fuel tank.&nbsp;</div>]]></description>

         <description><![CDATA[<div>It was definitely preventable. Theodore Cooper paid little attention to the bridge and constructed it with the cheapest possible materials. If they stuck with the original plans, and if he had actually been involved, then he would be able to see the errors and instructed on how to fix it. Sadly this didn't happen because of Cooper's neglect.</div>]]></description>

With the high hopes of having the shuttle be a reusable and reliable way to go to and from space for human transportation, the decision makers thought it nessecary for the shuttle to be able launch from anywhere at any time.</div>]]></description>
