Effective communication is very crucial for patient safety and quality care in health care center environment. We are facing communication problems between patient to nurse, manager to nurse, nurse to nurse during shift report, transferring patient to one floor to other or discharging patient from acute care facility to skilled nursing facility.  Nursing faculty that teach in undergraduate and graduate nursing program plays an important role in preparing nurses communicate effectively.

The nurse has to assess barriers to communication in order to improve, change, and redesign the health care system and meet standards of professional nursing practice. 

A study conducted by John Pill and co-researchers during the year of 2013 reported that the criterion on the basis of which communication skills must be judged for its degree of effectiveness involves four parameters namely:

• Approach towards a patient and his/her care,
  
• Mannerisms presented while interacting with a patient and handling his/her case,
  
• The methods or tools selected and adopted to promote quality interactions with the patient, and
  
• The various conventional aspects of a communicating process.

• language, 

• developmental level, 

• medical condition/disabilities, 

• learning styles, 

• psycho-social, 

• literacy, financial and 

• cultural factors

A study conducted by the Canadian Nurses Association projected the possible causative factors that can lead to loop holes in communication by a Nurse. Four factors were found to be reasons for this:

• Variations in conveying information and reports
  
• Absence of patient assignments

• To provide feedback and follow-ups punctually
