(29 inequalities across the 5 different domains). 
A electronic spreadsheet that’s freely available to use and share, the results and ratings are returned in a HEF profile using a five-point Likert scale. Higher exposure is linked to health problems associated with premature death or multiple significant long term health problems. 
Benefit: easier to see the differences the nurse has made and to see what further improvements could be made.
