1.       Submit a permit to enter the lab signed by the supervisor and the head of the lab

2.       Submitting a simlab to the laboratory technician

3.       For non-pharmacy students, please bring a letter from dept of origin

4.       Submit an overtime letter if working after 4 pm

5.       Fill in the simlab every time you borrow the tools and pick up materials

Regulation for researchers

1.       You must fill in the logbook while using the tool

2.       The tools are returned in a clean condition and in the original place

3.       Must replace the tools if damaged

4.       Must wear personal protective equipment (laboratory coat, mask, and gloves)

5.       Read the rules before used the laboratory instruments

6.       Clean the area before leaving the lab

Examination participants are PSPA (Profesional Pharmacy Programme) students who meet the following criteria:

 

a.      Active student of Professional Pharmacy Programme Batch 40, whose names are registered at UIIgateway.

b.      Have completed all academic activities at PSPA.

c.      Have collected all internship reports.

 

1.   Participants of OSCE Examination are obliged
to attend a briefing scheduled on D-1 before examination

2.   Briefing attendance on D-1 is one of requirements to join the examination. Absences of briefing may result in a failure for the examination

 

1.      Examination will be divided into Session 1 (08.00 a.m -12.00 p.m) and Session 2 (01.00 pm – 05.00 pm)

2.      The participants of Session 1 should stand by at 06.00 a.m
3. The participants of Session 2 should stand by at 11.00 a.m.

1.       Students must follow the assistance material according to a predetermined schedul

2.       Late students are not allowed to participate in the laboratory practice

3.       Students must follow the pretest as a requirement for following the laboratory practice

4.       Students follow the pretest in accordance with the rules of each laboratory practice that is followed

5.       Students are allowed to has a laboratory practice in another schedule, following these conditions:

a.       At the prior time students were ill (proven by a letter from a doctor), Family affairs (family funeral, students marriage, going on a pilgrimage, or giving birth) as evidenced by a letter

b.       Students do not pass the pretest (in ths case they need to pay fee). Students need to report to the lecturer if they can not attends the laboratory practice 

6.       During the laboratory practice activities, the students are obliged to:

a.         Respect for the lecturer, laboratory assistant, and staffs

b.         Comply with laboratory practice rules and regulations properly

c.          Being earnest in following the laboratory practice, not joking around during the laboratory practice

d.         Keep the laboratory equipment and facilities clean. The students are obliged to return the tools used during the laboratory practice in a complete, clean and dry condition. The students who destroys the laboratory equipment are obliged to replace it with the same type and quality.

 

1.         Students who repeat the laboratory practice are required to take all laboratory practice topics

2.         Repeat students are allowed to take part in the final exam if all laboratory practice subjects have been followed.

3.         Students are required to obey the rules and procedures of laboratory practice

4.         Students are obliged to make provisional reports and fill the worksheet in accordance with rules.

5.         Students are obliged to follow the final exam of the laboratory practice and must meet the requirements to join the final exam

6.         Students who do not participate in the laboratory practice 2 times in a row without clear reasons are considered to have resigned.
