Vaccination is an extremely powerful public health intervention that saves countless lives and promotes good health. It is second only to clean water, which is a basic human right. It is deeply concerning that some countries have experienced a decline in vaccination uptake, due in part to misguided fears about vaccine safety. When vaccines are successful in controlling disease, parents are unlikely to witness the devastating effects of vaccine-preventable infectious diseases firsthand. Unfortunately, this can lead to misplaced anxiety or suspicion about vaccines, which can override concerns about the disease itself. As a result, we often see a resurgence of infectious diseases when vaccination rates decline. To combat this, some countries have mandated vaccination, but the effectiveness of this approach varies greatly between countries. We must prioritize vaccination to ensure the health and safety of our communities.

There is always a risk of the unvaccinated child contracting the disease, depending on the type of disease, how easily transmissible it is, and its prevalence in the area. Parents should be aware that there is a definite risk of their child getting sick. Unvaccinated children are, for example, 22% to 35% more likely to contract measles than those kids who are vaccinated against it. Thus, a group of unvaccinated children with measles can put all other children they come into contact with at risk of contracting the disease. Although vaccines work extremely well, nothing is 100% effective. One child may have gotten all her immunizations, but perhaps she didn't get full immune protection from every dose. Another might be too young to be vaccinated or might have received her latest dose a bit late. On a community level, schools with higher numbers of exempt children have higher rates of disease. We've seen it in some religious communities. An example is an island off the coast of Seattle, Washington, where many children are unvaccinated; there, outbreaks of pertussis occur.

There's a simple argument in favor of mandating Covid-19 vaccinations. By getting vaccinated you reduce your risk of serious illness. Less serious illnesses mean fewer deaths and less pressure on hospitals.

Historically, immunization campaigns have seen huge success, eradicating smallpox and drastically reducing mortality levels in diseases such as polio and measles.

"We have good examples that show a direct causal relationship between requirements, getting very high vaccination rates, and protecting not just individuals but protecting communities," says Jason Schwartz, an associate professor in the History of Medicine at Yale University.

"Vaccines work, they work, we've got a large body of evidence to show that."

To help understand the issues at play, we sat down with some leading experts. We asked whether mandatory vaccination is the answer to minimizing the impact of diseases such as measles, diphtheria, and pertussis.

Dr. Julie Leask of the University of Sydney says regulation is useful but absolute mandates go too far. “We need requirements that encourage parents to get their children fully vaccinated,” she says. “But there need to be exemptions for people that don’t vaccinate. It should be harder to get an exemption than a vaccine.”

Regulations work, she notes, and help to push the vaccination rates up. However, hard-to-get exemptions are important to ‘firm but fair’ policies. Removing other barriers to vaccination, educating health providers, and providing strong information systems are also vital pieces of the puzzle.

It’s not all about childhood vaccination. Dr Leask also points to the risk of disease outbreaks resulting from low vaccination rates in adolescents and adults. This, she says, should be part of a multi-part approach to improving public health rather than expecting mandates alone to solve the problem.

Herd immunity provides the maximization of utility as the costs of vaccination for an individual are generally low (because of high vaccine safety) and are outweighed by resulting gains in reductions in disease incidence and preventative health. These advantages contribute to the well-being of the population and, in the long term, conserve resources and money by reducing the need to care for those infected with the disease.
