Executive Summary
Many students have complained about the  slow document delivery process of the Student Affairs Office. It has also been reported that the same office staff have been taking extended lunch breaks. Investigation has revealed evidence that confirms initial reports.

Introduction
This report will look at:
• student complaints
• employee break logs
• transcript request and delivery records
• recommendations for action

Findings
1. More than 30 student complaints have been recorded in the past semester alone in regards to the document request process.
2. Personnel have been abusing the leniency of the office manager and regularly taking 2 hour long lunch breaks.
3. The University website states that students should be able to receive their transcript withing 2-3 business days, but records show an average of 8 days from request to delivery.

Conclusion
Inefficiency in management and employee subversion of university regulations have lead to increased difficulty in students receiving valuable services and important documents in a timely manner.

Recommendations
It is therefore recommended that the Student Affairs Office manager is retrained on office management and that the staff is reminded of standard university protocols.

Complaint Investigation Report


Executive Summary

There has been complaints from students about mistakes in transcripts and papers taking long time to be recieved. It was also reported that some personnel take long breaks. We investigated these claims and found out that they are true.

Intro

This report will look at:
• Complaints about mistakes in student transcripts
• Complaints about recieving papers taking long time
• Personnel taking longer than limit lunch breaks


Findings

1. Upon further investigation it is determined that some mistakes were made in student transcripts by the office.
2. It has been observed that the process of receiving some important papers take longer than necessary time.
3. It has been detected that some personnel take longer lunch breaks than the legally allowed limit.


Conclusion

Some mistakes were made in student transcripts by the office in student transcripts, recieving important papers take long time and personnel takes long lunch breaks.


Recommendations

It is recommend that the office reviews the mistakes and rrewrites the transcripts, paper receiving process should be fastened and the personnel should be warned about adhering by the time limit rules.

   1. According to students complaints the office makes mistakes on the transcripts and they prepare transctipts in a very long time
   2. We have comfirmed that office personel takes longer lunch breaks than its leggaly allowed
Conclusion
   Office personel found guilty about transcripts and lunch breaks.
Recomandation
   We should prepare clear preparing guides for office personel to prepare transcripts correct and at the time also we should bring strict punishments about taking long lunch breaks.

Executive Summary
Students in Universal University have complained that the Student Affairs Office made mistakes in student transcripts and the process of receiving important papers took quite a long time. Also, students have reported that office personnel took the lunch break longer than the legally allowed limit.

Introduction
This report will cover these:
  • mistakes made in the student transcripts
  • late delivery of significant documents
  • the responsibilities of the office personnels

Findings
  1. Some mistakes such as wrong course entry were made in student transcripts. Office system caused this problem.
  2. Documents such as permission documents and result documents were delivered late.
  3. Office personnel took the lunch break more than 1 hour.

Conclusion

Office system should be fixed and office personnel should receive formal warning.
