This is a checklist created by a teacher to help with her students who need a little extra care and attention. It does well to allow a teacher not just to check a box for behavior, but also give an area for comments and feedback.

Student Self Evaluation

I thought this was very interesting. It is a student self evaluation. It seems it would be useful for the upper part of my Elementary school as students can look inward and really try and judge how they did.

I really like this checklist as it has an area that reads, "How will I celebrate if the goal is met?" I think it would be good for students not just to create a goal for behavior, but to also think of how to celebrate doing well. In addition it allows for teacher and student feedback.

Checklist with Class Rules

I think this one is good for younger students than I have in Elementary, but I put it here because I really liked the way it had the classroom rules in parentheses under the descriptions. This can help student better understand those rules and what they mean.

Student Reflection

This worksheet isn't a checklist, instead it is a reflection sheet. Students will need to think more about their behavior to fill it out and will need to reflect affect on the class their behavior had. I also really like that the student gets to come up with their own action plan to change. I think allowing the student to have this chance to take control can help them in the future.

Action Plan with Why Checklist for Students

I really like that this action plan has an area for why I acted this way, and in that area there is a checklist. As some of my students have a hard time expressing themselves when frustrated, especially in a second language, I think this could be useful in my classroom.

I created a simple daily checklist that allows the teacher or student to put in multiple behaviors and mark them on a day of the week basis. This page also has an area for the parents and teacher to send feedback or ask questions of one another.
The second page is an in class action plan. If a student is having an issue they can move to the quite corner I have set up to calm down. Once calmed down they can complete the action plan to think about why they are acting out and the appropriate actions they can use in the future.
