Students will need to think more about their behavior to fill it out and will need to reflect affect on the class their behavior had. I also really like that the student gets to come up with their own action plan to change. This page also has an area for the parents and teacher to send feedback or ask questions of one another.
The second page is an in class action plan. If a student is having an issue they can move to the quite corner I have set up to calm down.
