In our district, students who have a 'D" or "F" average at the four to five week mark in the grading period will receive an interim report card to alert their parents of their child's progress. Due to the limited number of students receiving this snapshot of their grade midway through the grading period, this allows the student, parent/guardian, and teacher to all work together to help that student improve their grade before the final grade card is submitted to the district for the student's school record.
(Interim/Benchmark, Formative)

In our district, our grading policy is one where the majority of the points are required to come from summative assessments. This allows students to learn during the formative assessments and take in the feedback from previous learning to help them grow and learn before the summative assessment is given and graded. One way to gauge students' understanding is to assign homework to determine how well students can complete their work independent of the teacher. If students are able to complete without the use of their teacher, then those students are likely to be on-level or above level. Other students who have not completed the work or who have a difficult time completing the homework are a signal for the teacher that this student may need more instructional time or practice to become more self-confident with that particular skill.
(Classroom-focused, Formative)

For some teachers, it is critical for them to use an exit ticket after a certain day's lesson to determine how well the material was taught (and understood) that day. Also, an exit ticket is a way for teachers to assess if the students are beginning to understand concepts that were being introduced or reviewed for the class.
(Classroom-focused, Formative)
