         <description><![CDATA[<div>The Osteopath Feb/Mar 18.<br>Some interesting articles which have given me thoughts to consider TEXT NECK/PHALANGEAL SD which I'd like to research further and see if it is a viable option for a dissertation topic. I think it's especially relevant due to the increasing use in hand held devices and from a younger age.&nbsp;<br><br>Another thought from the same page is the article on PERSISTENT NEONATAL CRYING which I find very interesting and especially after the recent article PB commented had been published from research at the ESO, might be another possibility as area for dissertation research.&nbsp;</div>]]></description>

It bought back happy memories and was a valuable reminder of how far I've come since my own induction week 2 years ago.          <description><![CDATA[<div>Today I feel completely overwhelmed and not at all ready to go back to Uni on Monday. I do not feel ready to be responsible for patients and have no idea how I will treat them or manage their expectations, and not royally mess it up and make a complete fool of myself. It feels like I am hurtling towards the day when I am finally discovered as a fraud and that I do not belong here and have managed to get here by some collection of errors of other, not by my own merit at all. How did I achieve a 1:1 for the last 2 years? That is far better than any other academic achievement I have made and it simply does not feel real or like is it due to me...I feel like I have no idea what I am about to do and it is all I can do to quell the panic.<br><br>On the other hand (learning a la critique huh), I also just want to get on with it, get the first 2 weeks out the way which are bound to be dreadful and traumatic and then ease my way back into osteopathy. At the end of summer clinic in August I was not ready at all to stop, I felt like I was just getting started and really finding my groove I want to feel that way again.</div>]]></description>

         <description><![CDATA[<div>Feedback from the work was fair, and represented areas where I had removed information in order to meet the word count limitations. This shows that I am on the right path and that I need to be more succinct in order to fully discuss the points I want to raise so as not to lose my for incomplete work or work which is lacking scope/bredth.<br><br>The "minor errors with quote referencing" is an incorrect comment which I queried and followed up with a tutor (PB), I was assured that I had received full marks for referencing and that this mistake had been picked up during the moderation process. I was pleased and reassured by this.<br><br>Owing to the limitation of the number of peer reviewed studies we were permitted to reference I am not entirely sure how to achieve this final feedback comment, but I will continue to strive to find the most current and verifiable research to support (or not) any point I am trying to convey in future pieces of work.<br><br><em><sup>"Provide a fuller disucssion of osteopathic treatment to better inform patient. minor errors with quote referencing. A broader selection of peer reviewed studies is needed to show depth of knowledge and to show reading has been done in the statement area justifying and reinforcing the thought process."<br></sup></em><br></div>]]></description>

         <description><![CDATA[<div>I observed my colleague treating their first patient and I provided some feedback and noted my own reflections.&nbsp;<br>Feedback to the practitioner is something which my clinic group is going to implement on a regular basis and is a very useful feedback tool I observed the 4th years doing last year.&nbsp;<br>We are there to help each other and this is a helpful tool for us to use to reflect upon.<br><br>I also intend on starting my clinic journal shortly, however I am unsure of the medium to use as I want to make notes at the moment when I feel them so that they are pure, which for me means paper and pen....but for the reflective portfolio I'd like to make a 'word' document...but I can't get my laptop out in the clinic room. I think that I will ask some tutors what they suggest and go from there. I also want to make sure that my reflection is both helpful for myself and that I am using the tool correctly, is there a format which I can adopt to 'reflect' appropriately/more efficiently?&nbsp;</div>]]></description>

         <description><![CDATA[<div>Very interesting program - the power of placebo.<br><br>By taking all of the most up to date research on placebo they created the optimum environment for chronic pain sufferers.<br><br>Then, after proving that the placebo group improved, they told the group that the daily 'medication' was a placebo - patients were invited to carry on the trail and see what happened.<br><br>Even though people KNEW they were taking a placebo - for some of them their S&amp;S continued to improve!&nbsp;<br><br>That blows my mind - the power of the mind....it's so suggestive and can really shape our reality.&nbsp;<br><br>Important to remember this as I go on in the course and treating my patient, for me it's a 40 minute encounter - for them, something I say or do could change their life. <br><br><br>https://www.bbc.co.uk/iplayer/episode/b0bmblb8/horizon-2018-8-the-placebo-experiment-can-my-brain-cure-my-body</div>]]></description>

         <description><![CDATA[<div>Sciatica-like leg pain can be the main presenting symptom in patients with cervical cord compression. It is a false localizing presentation, which may lead to missed or delayed diagnosis, resulting in the wrong plan of management, especially in the presence of concurrent lumbar lesions. <br><br>Funicular leg pain is a rare presentation of cervical cord compression. It is a referred pain due to the irritation of the ascending spinothalamic tract. <br><br>Spinal cord compression occasionally causes pain that is referred to the lower back or lower extremities, which is well below the level of the lesion. <strong>Such pain is called “tract or funicular pain”</strong> and there have only been a few cases described in literature&nbsp;<br><br>In case 1, the patient was a diabetic who had a concurrent lumbar stenosis at L4–5. Apart from atrophy of the right upper extremity, he had no myelopathic signs to suggest cervical cord compression. His reflexes were not brisk and his leg pain, right ankle and big toe weakness could have been due to the lumbar stenosis. It was the right upper extremity atrophy that alerted us to the possibility of cervical cord involvement.&nbsp;</div>]]></description>

It delivered on both the humour and the brutal honesty of the choas, hardwork and dedication involved in training to be a doctor in the NHS.<br>I believe that osteopathy is a little calmer than this, however I do feel an afinity and a reflection in the pressure and scope of knowledge both professions are required to have inorder to be safe, effective and successful in their respective fields.<br>I would recomend this book, I would also read it again.</div>]]></description>

         <description><![CDATA[<div>The Osteopathic Practice Standards set out the standards of conduct, ethics and competence required of osteopaths to promote patients’ health and wellbeing, protect them from harm, and maintain public confidence in the profession. It brings together the Standard of Proficiency and Code of Practice required by the Osteopaths Act 1993.<mark><br></mark>The standards provide a framework to support the delivery of ethical, competent and safe osteopathic care. The standards also play a central role in the requirements for osteopathic training and gaining registration with the General Osteopathic Council for both UK and internationally qualified applicants in osteopathy.<br>The standards set out the expectations of osteopaths as regulated healthcare professionals. Patients must be able to trust osteopaths with their health. To justify that trust osteopaths must meet the standards expected in the Osteopathic Practice Standards. Osteopaths are personally accountable for their professional practice and must always be prepared to justify their decisions and actions, explaining how they have exercised their professional judgement.<br>The standards and guidance are arranged in four main themes:<br>B Knowledge, skills and performance A Communication and patient partnership<br>C Safety and quality in practice D Professionalism<br>The standards in each theme are listed at the start of each section. They are then reproduced, with, in each case, supporting guidance.<br>Within the Osteopathic Practice Standards the terms ‘must’, ‘should’ and ‘may’ are used in the following ways:<br>• ‘Must’ is used where osteopaths are expected to comply. Failure to do so may put the osteopath at risk of fitness to practise proceedings.<br>• ‘Should’ is used to indicate how the standard can be met.          <description><![CDATA[<div><strong>1. Gaining consent is a fundamental part of your practice and is both an ethical and a legal requirement. If you examine or treat a patient without consent, you may face criminal, civil or GOsC proceedings.<br><br>2. Gaining consent is an ongoing process. You must ensure that patients are able to make decisions at all stages of their treatment and care and continue to give consent.<br><br>3. For consent to be valid, it must be given:<br>3.1 voluntarily<br>3.2 by an appropriately informed person<br>3.3 with the capacity to consent to the intervention in question.<br></strong><br></div><div><strong><em>Voluntarily</em></strong></div><div><strong><br>4. For consent to be voluntary, the patient must not be under any form of pressure or undue influence to consent to osteopathic care. You must ensure that patients are given the information they need to reach their own decision and give consent.<br><br>5. Situations where you might question whether consent is voluntary might include patients being put under pressure by employers or relatives to accept osteopathic care, or where a patient might otherwise be vulnerable.<br><br></strong><strong><em>By an appropriately informed person</em></strong><strong><br><br>6. The patient needs to understand the nature, purpose, benefits and material or significant risks to them of the examination or treatment proposed. The patient must then be free to either accept or refuse the proposed examination or treatment. Some patients may need time to reflect on what you have proposed before they give their consent to it.<br><br>7. Where your diagnostic examination and treatment are carried out simultaneously, consent could be obtained by explaining your approach, describing the types of treatment methods you might like to use, setting the parameters within which you will work, and explaining how the patient may indicate that they wish the treatment to stop. If the patient consents to you proceeding on this basis, you may do so. If the patient expresses concern that you are going outside the agreed treatment plan, you must stop the treatment.<br><br>8. Before deciding that consent has been given, you should consider whether patients have been given the information they want or need, and how well they understand the details and implications of what is proposed.<br><br>9. It is particularly important to ensure that your patient understands and consents to the proposed examination or treatment of any intimate area before it is administered. Intimate areas include the groin, pubis, perineum, breast and anus, but this list is not exhaustive, and patients may regard other areas of their body as ‘intimate’. Some patients may not have come prepared for such a procedure and you should offer to conduct this at a subsequent appointment, and offer a chaperone.<br><br></strong><strong><em>Capacity</em></strong><strong><br><br>10. Capacity, in this context, relates to the ability of an individual to understand, retain and evaluate information and to make and communicate a decision regarding their health needs and treatment options.<br><br>11. You must not assume that a patient lacks capacity solely because of their age, disability, appearance, behaviour, medical condition, beliefs, or because they make a decision with which you disagree. The starting point should always be a presumption of capacity.<br><br>12. The law recognises that some patients – because of illness, mental capacity or age – are not competent to give consent for a particular examination or treatment. This is because they may not be able to absorb or weigh up the information and make an informed decision.<br><br>13. When an adult lacks mental capacity, decisions about their treatment must be taken in their best interests and in accordance with relevant legislation. Further details on the relevant legislation in England and Wales, Northern Ireland, and Scotland are provided in the GOsC guidance documents for each jurisdiction entitled Obtaining Consent.<br><br></strong><strong><em>Treatment of children and young people<br>Note that in the summary below a ‘child’ is a person under the age of 16 years and a ‘young person’ is a person aged 16 or 17 years.</em></strong><strong><br><br>14. Obtaining consent for treatment to be given to a child or young person is a complex issue. If you treat children, you must be aware of the law in this respect, which may vary depending on where you practise in the UK. Further details are provided in the GOsC guidance document Obtaining Consent.<br><br>15. A child may have the capacity to consent. You will need to apply the law, and use your professional judgement in assessing the capacity of each patient under 16 years of age, which will depend on their maturity and ability to understand what is involved. You are strongly advised to involve a person with parental responsibility for the child when seeking consent.<br><br>16. You should involve children and young people as much as possible in discussions about their care, even if they are not able to make decisions on their own.<br><br>17. A young person can be treated as an adult and can be presumed to have the ability to make decisions about their own care. Nevertheless, you will need to use your professional judgement to assess whether the young person in fact has the maturity and ability to understand what is involved in the treatment you are proposing for them because, as with adults, consent must be valid.<br><br></strong><strong><em>Records of consent</em></strong><strong><br><br>18. You must record key elements of your discussion with the patient. This should include information discussed, any particular concerns, expectations or requests for information raised by the patient, how you addressed these, and any decisions made. It is important that such issues are evidenced in the patient records.<br><br>19. The validity of consent does not depend on the form in which it is given. Written consent may serve as evidence of consent, but if the elements of voluntariness, appropriate information and capacity have not been satisfied, a signature on a form will not by itself make the consent valid.<br><br>20. Valid consent does not always have to be given in writing by the patient. However, if you are proposing a vaginal or rectal examination or technique, written consent must be obtained and kept with the patient records. You may also consider gaining written consent for other procedures, particularly those relating to areas which the patient considers intimate.<br><br></strong><strong><em>Sharing of information<br></em></strong><strong><br>21. You must obtain specific consent from patients regarding the sharing of any information about them with others.</strong></div>]]></description>

         <description><![CDATA[<div><strong>1. Patients will have different requirements for maintaining their dignity and modesty during a consultation, and you must be sensitive to these. Some of these ideas may have been shaped by a patient’s culture or religion, but it is unwise to make assumptions about any patient’s ideas of modesty.<br><br>2. You should respect your patients’ dignity and modesty by: <br>2.1 explaining to patients in advance of their first appointment that they may be asked to undress for examination and treatment<br>2.2 allowing a patient to undress, and get dressed again, without being observed<br>2.3 only observing a patient undressing if you consider this necessary for the purposes of diagnosis or treatment – this must be explained to the patient and consent obtained; if the patient does not wish to be observed, you must respect their wishes and find another way of establishing the clinical information you need<br>2.4 giving patients the option of covering areas of their body that do not need to be exposed for examination or treatment. This can be achieved by providing a suitable gown or cover, asking that they only remove such items of clothing as are necessary for the proposed examination or treatment, or providing the opportunity to get dressed again in full or part as appropriate. If you feel it is necessary for the examination or treatment that the patient is undressed to their underwear, you should explain this to the patient, and seek their consent.<br><br>3. If it becomes necessary during examination or treatment to adjust or remove items of the patient’s clothing or underwear, they should be asked to do this themselves. If it is necessary for you to assist them, then you must have their consent to do so.<br><br>4. If you need your patient to remove underwear for an examination or treatment, you should ask them to put their underwear back on at the conclusion of that particular examination or treatment and before you continue with any other procedure<br><br>5. You must always ask a patient if they would like a chaperone when: 5.1 you examine or treat an intimate area<br>5.2 you are treating a patient under 16 years of age<br>5.3 you are treating an adult who lacks capacity<br>5.4 you are treating a patient in their home.<br><br>6. A chaperone can be:<br>6.1 a relative or friend of the patient<br>6.2 a suitable person from your practice but not your spouse or personal partner.<br><br>7. If the patient wishes to have a chaperone and neither you nor the patient is able to provide one, you should offer to rearrange the appointment.<br><br>8. If a chaperone is present, you should record this in the patient records. If a patient within one of the categories in paragraph A6(5) declines the offer of a chaperone, you should record this in the patient’s notes.</strong></div>]]></description>

         <description><![CDATA[<div><strong>1. Abuse of your professional standing can take many forms. The most serious abuse of your professional standing is likely to be the failure to establish and maintain appropriate boundaries, whether sexual or otherwise.<br><br>2. Appropriate professional boundaries are essential for trust and an effective therapeutic relationship between osteopath and patient. Professional boundaries may include physical boundaries, emotional boundaries and sexual boundaries. Failure to establish and maintain sexual boundaries may, in particular, have a profoundly damaging effect on the patient, is likely to bring the profession into disrepute and could lead to your removal from the GOsC Register.<br><br>3. Not all crossing of professional boundaries will necessarily be an abuse of your professional standing. For example, sometimes it may support empathy and trust with a patient to disclose personal information or to treat a patient as an emergency outside your usual hours. However, there is a spectrum, and osteopaths must ensure that patients who may be vulnerable are protected at the time and throughout the duration of the professional relationship.<br><br>4. You should be aware of the risks to patients and to yourself of engaging in or developing social or commercial relationships with patients, and the challenges which this might present for the therapeutic relationship and to the expectations of both patient and professional. You should also be aware of the risk of patients developing an inappropriate dependency upon you, and be able to manage these situations appropriately, seeking advice from a colleague or professional body as necessary.<br><br>5. When establishing and maintaining sexual boundaries, you should bear in mind the following: <br><br>5.1 words and behaviour, as well as more overt acts, may be sexualised, or regarded as such by the patient. Examples might include:<br>5.1.1 sharing inappropriate intimate details about yourself <br>5.1.2 visiting a patient’s home without an appointment <br>5.1.3 making inappropriate sexual remarks to or about patients <br>5.1.4 unnecessary physical contact.<br><br>5.2 you should avoid any behaviour which may be construed by a patient as inviting a sexual relationship or response.<br><br>5.3 physical contact for which valid consent has not been given can amount to an assault, leading to criminal liability.<br><br>5.4 it is your responsibility not to act on feelings of sexual attraction to or from patients.<br><br>5.5 if you are sexually attracted to a patient or if a patient displays sexualised behaviour towards you, you should seek advice from, for example, a colleague or professional body on the most appropriate course of action. If you believe that you cannot remain objective and professional or that it is not possible to re-establish a professional relationship, you should refer your patient to another healthcare practitioner. If referring a patient because of your own sexual feelings towards them, you should endeavour to do so in a way that does not make the patient feel that they have done anything wrong.<br><br>5.6 you must not take advantage of your professional standing to initiate a personal relationship with a patient. This applies even when the patient is no longer in your care, as any personal relationship may be influenced by the previous professional relationship and an imbalance of power between the parties.<br><br>5.7 you must not end a professional relationship with a patient solely to pursue a personal relationship with them.<br><br>5.8 if you think that a personal relationship with a former patient might develop, you must consider whether this is appropriate. Factors that might impact on this include:<br>5.8.1 the nature of the previous professional relationship <br>5.8.2 the length of time the professional relationship lasted, and when it ended<br>5.8.3 whether the former patient was particularly vulnerable at the time of the professional relationship, and whether they might still be vulnerable.<br><br>5.9 osteopaths who practise in small communities may find themselves treating friends or family. In such cases, establishing and maintaining clear professional boundaries will help you ensure that your clinical judgement is objective and that you can provide the treatment your patients need. The same level of care should be given to all patients, whether you know them in a social or other capacity, or not.</strong></div>]]></description>

Maintaining patient confidentiality includes: <br>1.1 keeping confidential your patients’ identities and other personal information, and any opinions you form about them in the course of your work<br><br>1.2 ensuring that your staff or anyone else attending your clinic in a professional capacity (for example, students of osteopathy, potential students or peers) keep such information confidential<br><br>1.3 ensuring that the information is kept confidential even after the death of a patient<br><br>1.4 not releasing medical details or information about the care of a patient to anyone – or discussing such information with anyone – including their spouse, partner or other family members, unless you have the patient’s consent to do so (see standard D5(7) and standard D5(8) below)<br><br>1.5 taking appropriate measures to ensure that that such information is securely protected against loss, theft and improper disclosure.<br><br>2. Patients are entitled to obtain copies of their notes and, if such a request is made, you must comply with this in accordance with relevant legislation and good practice.<br><br>Management of patient information <br><br>3. You should have adequate and secure methods for storing patient information and records. Patient records should be kept:<br>3.1 for a minimum of eight years after their last consultation<br>3.2 if the patient is a child, until their 25th birthday.<br><br>4. You should have a written policy regarding retention, transfer and disposal of patient information and records, which should include whether it is your practice to retain them beyond eight years, or, in the case of a child, beyond their 25th birthday. Your patients should be made aware of this.<br><br>5. You should make arrangements for records to continue to be kept safely after you finish practising, or in the event of your death or incapacity. Patients should know how they can access their records in such circumstances.<br><br>6. You must comply with the law on data protection and associated legislation. For further information on data protection, please refer to the website of the UK Information Commissioner’s Office.<br>Disclosure of confidential information<br><br>7. There may be times when you want to ask your patient if they (or someone on their behalf) will give consent for you to disclose confidential information about them; for example, if you need to share information with another healthcare professional. In that case, you should:<br><br>7.1explain to the patient the circumstances in which you wish to disclose the information, and make sure they understand what you will be disclosing, the person you will be disclosing it to, the reasons for its disclosure, and the likely consequences<br><br>7.2 allow them to withhold permission if they wish <br><br>7.3 if they agree, ask them to provide their consent in writing or to sign a consent form<br><br>7.4 advise anyone to whom you disclose information that they must respect the patient’s confidentiality<br><br>7.5 disclose only the information you need to (for example, does the recipient need to see the patient’s entire medical history?).<br><br>Disclosure of confidential information without consent<br><br>8. In general, you should not disclose confidential information about a patient without their consent; however, there may be circumstances in which you are obliged to do so. Such circumstances might include: <br><br>8.1 If you are compelled to do so by order of a court or other legal authority. You should only disclose the information you are required to under that order<br><br>8.2 if it is necessary in the public interest. In this case, your duty to society overrides your duty to your patient. This might happen when a patient puts themselves or others at serious risk; for example, by the possibility of infection, or a violent or serious criminal act<br><br>8.3 if it is necessary, in the interests of the patient’s health, to share the information with their medical adviser, legal guardian or close relatives, and the patient is incapable of giving consent.<br><br>9. In any such circumstances, you are strongly advised to seek appropriate legal advice.<br><br>10. If you need to disclose information without your patient’s consent, you should inform the patient, unless you are specifically prohibited from doing so (for example, in a criminal investigation), or there is another good reason not to (for example, where a patient may become violent).<br><br>11.          <description><![CDATA[<div>Today was the first session of the BFPS training and focused mainly on passive listening. Before we started this task I had forgotten how hard I found this. Sometimes my brain is so concentrated on looking and being 'listening' that my ears forget to work and I don't 'hear' anything the person is telling me. After some practice I got used to it and it was helpful to listen whilst observing rather than only in the role play.&nbsp;<br>We learnt about the difference between advice and information. Before we started this task I wasn't aware how much I push advice into people in a well meaning way, especially if it is a topic I am passionate about such as breastfeeding (BF). I saw how many people can hear the same sentence differently "you should do what's best for baby and for you" was construed by some as a criticism (as in Mum hadn't made this consideration already) and by others as more caring.&nbsp;<br>Advice is owned by the person giving it "I" or "in my opinion" or "you should" whereas information is more generalised, fact driven, and often with resources to back it up "many women find" or "according to the NHS/WHO" or "here is a leaflet on".&nbsp;<br>Both of these experiences could help me develop to being a better osteopath by allowing me more time to learn to remove my ego from a consultation and be more patient centred. Over the coming weeks I will strive to be more aware of and introduce these listening practices learned today.          <description><![CDATA[<div>28th July 2020:O: I found the PREVENT &amp; FGM training very insightful and didnt realise how similar a mental health problem might be to radicailsation. I was not aware that there is a muli-team setup whereby different public services collborate to support individuals in need for either situation.<br>The FGM awareness training was quite difficult to watch at times as it is a sensitive subject and again, one which I didn't know much about.&nbsp;<br>R: To be informed that I have a duty of care to make an obligatory report of confirmed FGM/suspected FGM/potential FGM was surprising and made me once again feel the gravitas of the resposnsibility I will have once I am qualified to both my patients and the wider community. This is something I didn't factor in when making my application to enrol on the course and always takes me a little by surprise.<br>A: I feel confident knowing that there is an established process for either situation that I can refer to if I have any concerns.<br><br>OPS: A2, A5</div>]]></description>

         <description><![CDATA[<div>23/02/21 - Husband visited opticians, Dx Cataracts.<br><br>My husband visited the opticians after experiencing headaches following screen usage. The optician changed his glasses prescription, and at the end of the consultation he told him that everything is "generally fine but you do have the start of cateracts so they'll need to be operated on at some point inthe future." My husband was shocked to hear this news as it was unexpected and he has no FxHx.&nbsp;<br>He asked the optician for more information, a time line?, what does this actually mean? and the optician was quite vague - "oh don't worry, you won't need an operation for quite a while and anyway, its the most routine and successful opthalmic operation".......Quite a while turned out to be 5-10years time which from my husbands perspective is not a very long time at all.<br>When hubby asked what to look out for the optician came across as dismissive and made my hubby feel foolish for asking, as if heshould've already know the S&amp;S to look out for.<br><br>He was also shocked that the optician gave the Dx it in a way that was very blassè. He was shocked that the optician wasn't more senstive nor did he offer up any information without being pushed regarding a prognosis and S&amp;S to look out for.<br><br>This shows the importance of communicating in a sensitve way at a level that is appropriate to the patient.          <description><![CDATA[<div>Feb 2021<br><br>I have been struggling with asking for help as I feel that everyone in this new cohort is far better than I am, more knowledge, more skills, better reasoning, better application of techniques. I've missed my friends terribly and now Dissertation is out of the way I know that I need to focus on my clinical skills and knowledge.<br><br>Fear of looking foolish and "I should know all of this all ready" has been a huge blockade to me inviting others to practice, or when I do finally pluck up the courage I struggle to follow through. I'm still a lot more comfortable practicing online cases with my close friends who have already graduated.<br><br>I took the plunge in the final SAT lesson as asked another student if she wanted to stay a little longer and practice the techniques we learnt the previous day. It took so much courage for me to ask her, as I didn't want her to realise I'm rubbish (massive imposter syndrome going on at the moment) but I'm really glad that I did because we have a really effective extra 20mins practice and she was kind, giving helpful feedback on my positioning and hand placement.<br><br><br>I also took 3 weeks to ask Mr F. for some 121 feedback and help on my clinical process, questionning for CxHx taking. I'm really glad I did as he was very helpful. I've since been working on drawing my systemic questions into the intial part of the CxHx to rule out systemic DDx - something which feelsquite alien, but infact makes a lot more logical sense.&nbsp;<br><br>I have my CCA mock 25th Feb, I hope it goes well!</div>]]></description>

         <description><![CDATA[<div>25th Feb 2021<br><br>What happened?<br>I'm over the moon! I passed my CCA mock with 64%! I can do it, I'm actually not rubbish and a fraud! There is hope and light at the end of the tunnel. The case was relatively simple and a clear MSK originating pain with a clear systemic and PxHx.&nbsp;<br><br>So What does this tell me?<br>The pessimist in me acknowledges that this was a simple case, so not to get too excited. If I have a more difficult case in the CCA1 exam I will probably fail it. I still feel like my base knowledge is not there.<br><br>Now What can I do to make sure that I pass CCA1?<br>REVISE and work through all of these action plans that I am habitually making but not completing. I need to make a simple and clear revision schedule, using my time to the max.&nbsp;<br>I need to contact my Padlet/Portfolio tutor and make a plan with her re timings for submitting the essay for feedback and I need to then make a clear plan about what has to happen and for me to revise for FOPE/CCA1/CCA2.</div>]]></description>

         <description><![CDATA[<div>O: Mr K noticed that I was off my game today and pulled me into a seperate room to talk through my worries. I confided that I am feeling very tearful still since failing my CCA1 and have no idea where to being trying to make sense of everything I need to learn. I am feeling very vulnerable and alone at the moment without my core group of friends around me to support me.&nbsp;<br>He suggested that he might be able to help me take bitesize pieces of the information and set me a small task each day - focusing on the most important clinical elements that I need to learn in preparation for CCA2 and FOPE.&nbsp;<br>I jumped at the chance and am really grateful to him that he would invest that time in me.&nbsp;<br>R: It makes me feel like all is not lost and that perhaps I am capable and able to pass the course, and become and safe and competent osteopath.&nbsp;<br>It surprised me when he admitted that he understands how I'm feeling because he failed his exams too - I found that surprising as he is extemely knowledgable now.<br>A: A plan of 1-2hrs per day on focused and productive learning as directed by Mr K.          <description><![CDATA[<div>What was I trying to achieve?<br>I prepared for a difficult patient from 3rd year who had returned to me. I was concerned about the worries I had experienced with her before surrounding themes of inappropriate patient-practishioner dependence. She is hypermobile and creating a rehabilitation/strengthening program for her is beyond my compentency at the moment (OPS 2019: A2, A5, C6, D2)<br><br>So what could I have done better?<br>I arrived at the consultation, prepared. Gave all the information to my patient regarding the possible causes of the anaesthesia she was experiencing and different health professionals we can refer her to. I then conducted the assessment and discovered that my hypothesis were incorrect and no onward referral was necessary or desired by the patient.&nbsp;<br><br>Now what can I do to manage a similar situation in the future?<br>Don't get ahead of myself and gather all of the data from the patient before giving them detailed information about their options. Until I have gathered all the information how can i be certain which of the options are the most appropriate for the patient and which are unnecessary?<br>&nbsp;&nbsp;</div>]]></description>

         <description><![CDATA[<div>Only 3 people attended today's Journal Club - I felt disappointed that I couldn't persuade my friends to make the effort to join and one collegue in particular was less than kind when I suggested that we might attend.&nbsp;<br><br>It's short-sighted in my opinion,  because journal assessment will be an essential tool for when the time comes to write our dissertations.<br>I understand why people don't take the time and make the effort, when I was in my early 20's and doing my BSc I also didn't pay much attention to these ECA skill enhancers.<br>It's amazing what a little bit of life experience brings to one's outlook on the opportunities we are presented with.&nbsp;</div>]]></description>

         <description><![CDATA[<div>I spent a long time prepring the submission and preparing for the exam. It was my last day in clinic before I finished for maternity leave and I was under a lot of pressure in the weeks prior as I sat my LEPB 2/3/CPA one clinic session after the other.<br>In the exam/consulation itself, I felt extremely preppared and it was one of my most enjoyable pateitn consultations. I think this is because I felt like i really knew what I was doing, from the examination &amp; evaulation of the findings to the techniques I used to treat my patient. It would be amazing to be able to feel this prepared for each patient consulation, it really boosted my confidence and to think that I enjoyed the consultation so much and it was an EXAM!<br>I truely hope that my results reflect my perception of the session.<br>If I were able to be this prepared per consultaion (which I'm sure in the future as my knowlegde increases I will be) then I think I would find clinic far more enjoyable and less stressful. However, realistically, the time that I commited to this appointment I am unable to commit to each patietn appointment due to other academic commitments and family requirements.<br>Fingers crossed for results day!</div>]]></description>

         <description><![CDATA[<div>Having waited 2 weeks with a document that has comments but no grade, it was hugely stressful. Did I pass or not? That's all I could think about.&nbsp;<br>When I found out my grade of 55%, I was pleased to have passed but disappointed that this is my worst mark for the whole course. For such an important piece of work and something I put blood, sweat and buckts of tears into - I can't feel anything other than disappointment and loss at the likelyhood that my dreams of achieving a 1st Class Hons are now out of my reach. After achiving &gt;70% in all other years, and not considering my academic in a historical sense... I feel a deep sorrow that I will be unlikely to finish with similar success.</div>]]></description>

This will bring us into a much stringer position, commercially and financially. Dr Rolfe is a fabulous academic (something I feel the ESO has been missing) and Ian Fraser is a strong strategist and is passionate about moving the professionf orward into brighter more successful and respected times.<br><br>Although this additional position has added responsiblities to my time, I feel I am able to juggle it well. I wouldn't want to miss the opportunity to be part of something so dynamic and groundbreaking as what we are trying to create with the ICO.</div>]]></description>

There are so many new mums out there who are isolated and at home with little to no BF support and I know how essential that was to me both times - even more so the second time round as we battled a shallow latch and tongue tie.&nbsp;<br>I am keen and eager to complete the H&amp;S training once MCH release the information and we know more infomation about when the groups might start up again. In the meantime I've volunteered to man the 'inbox' and reply to Mum's email queries and also to take part in an interview for the Beside You Facebook page</div>]]></description>

         <description><![CDATA[<div>This exam went well, my relationhsip with the patient is good and the tutor I had was very supportive. I asked before hand if he would question me and make me work hard, to bring some additinoal difficulty. This is to help me to prepare for the FOPE and CCA2.<br><br>I was happy with the case history and management plan, I explained this well and was clear on what my ST and LT goals are.<br><br>Examination wise - I carried out a good assessment of the knee and went slowly as I palpated through the soft tissues, feeling for the boundaries of the H/C area's and tender spots.<br><br>Treatment - I'm trying hard to make my treatments as specific as possible now. Working slower through the tissues and trusting what I'm feeling. This is after feedback from 'treating' my friend at the weekend with another collegue observing. I have a tendency to rush because I sometimes feel like I want the experience to be over incase they realise how rubbish I am. I really need to quell the self doubt and negative self-talk....would I allow myself to speak this way to a friend? Absoultely not! So why am I talking to myself in such a negative way??<br><br>Moving forward: The plan<br>Weekly practice with 2 collegues, 1 who is a whizz at DD and the other who is a whizz at everything! I'm surrounding myself with the smart people. A friend said to me today: "if you're the smartest person in the room, you're in the wrong room".<br>Lets think about that for a minute......I don't profess to be, nor do I want to be the smartest in the room. What sage advice!&nbsp;<br>So my action plan now is to work collaboratively with key collegues who I have identified as masters of their field. Even just talking through Cranial concepts today with a collegue has got my oseopathic juices flowing - I love it! </div>]]></description>

         <description><![CDATA[<div>22/04/21 90 minutes<br><br>Fab lecture on all things LEX Radiculopathy - here are some key points:<br><br>Pain - could be <strong>Mechanical</strong> or <strong>Chemical</strong> or B<strong>oth</strong> in nature - NSAID can help with this whilst body repairs a prolapse for example<br><br>DDx - <strong>Claudication</strong> (peripheral artery disease), <strong>peripheral neuropathy,</strong> <strong>hip</strong> or <strong>SIJ</strong> (both somatic referral)<br><br>Patient expectations: Tell patients the truth! They might be in for a long journey of recovery and that means that they shouldn't 'put life on hold until they're better' but more important move and do ADL as best they can and manage the pain<br><br>Management &amp; Exercises: Neural gliding/flossing can help - but no more evidence to support this specific technique than strength/stability exercise or even exercise in general. Teach the Flossing first and then get patients to move about in the way that best serves them: light/moderate/regular! Only until they feel benefit - don't do it just because I've said so!</div>]]></description>

         <title>MAT Reflection - fighting the demons</title>

         <description><![CDATA[<div>30/07/2020</div><div>I’ve been having increasingly worrying thoughts that the new baby is going to stop breathing whenever I leave him to sleep so I must repeatedly check on him. That’s escalated now into OCD type behaviours such as kissing Zack every time I go upstairs and if I don’t follow the urge to then he’s going to die in his sleep. I’m having difficulty sleeping and the checking is becoming out of control. I’m also checking the window and door locks repeatedly in case they are open (when I know they are not).&nbsp;</div><div>&nbsp;</div><div>I have self-referred to a CBT counsellor and although I don’t think I have PPD (especially after writing my year 3 Psychology presentation on the topic (https://padlet.com/000953332ESO/Reflective_Portfolio_2016to2021/wish/1425661408)) I am aware that this behaviour isn’t healthy.&nbsp;</div><div>&nbsp;</div><div>She has Dx me with post-partum anxiety and given me some tools to help me break the habits and curb the behaviours. Acknowledging the worrying thoughts and telling my brain that that’s all they are – thoughts – is a key element. Fighting the urge will also help to break the habit.&nbsp;</div><div>&nbsp;</div><div>&nbsp;</div><div>&nbsp;</div><div>08/10/2020</div><div>I’m thankful that I’m no longer experiencing these feelings.&nbsp;</div><div>&nbsp;</div><div>I must remember the power of my mind to create my own reality, my own happiness and that anything I tell it – it will know to be true.</div>]]></description>

         <description><![CDATA[<div>This is a post and letter written to challenge the proposed change to the NICE guidelines in regards to effective treatment of pelvic girdle pain (PGP), effectively removing manual therapy as a a recommended treatment option. &nbsp;<br>The studies informing this change are limited and the attached letter highlights multiple studies NICE are encouraged to consider when updating the guidance in later this year.&nbsp;<br><br>Evidence based medicine should be just that - based on ALL the evidence available.&nbsp;<br><br>Unfortunately the proposed changes are based on a limited evidence base (3 studies, 1 of which looks at chiropractic/manal therapy as a care option.&nbsp;<br><br>https://www.facebook.com/groups/268837660424017/permalink/764933127481132</div>]]></description>

         <description><![CDATA[<div>The patient queried her diagnosis and outcome of the consultation. She arrived with LBP and we recommended she seek GP help for suspected Hypothyroidism.<br><br>She seemed happy with the outcome of the consutlation (lots of talking, digging deeper into her health history) and had all her questions answered. However upon returning home it seems she was unable to articulate to her husband what had happened and so she emailed asking for a copy of her notes.&nbsp;<br><br>We sent a summary letter (this is more appropriate than a copy of notes apparently - due to my scruffy writing and potential hyper-sensitive patient) which is attached below.<br><br>This was an interesting experience because although we did everything right at the time the patient still requires furhter written guidance. The other thing that's interesting is that I wrote a 1st draft, summary of the 10 points of pain and testing outcomes (basically justifying the DDx and the reason for excluding other DDx) but the tutor cut all of that out and kept the information succinct and to the point.&nbsp;<br><br>I wonder if more or less information is helpful to a patient - in this instance he siad les helpful as the patient is a worrier and more information allows patients to catastrophise - so we must be mindful of how we manage them. I've always believed that knowledge is power so it's interesting to me to consider this other view piont - and essential in future cirtical thinking and making decisions appropriate to each individual case I encounter. </div>]]></description>

         <description><![CDATA[<div>I'm pleased with the achievements I've made so far and find this LAP a helpful tool in keeping me on track with the important aspects of learning at the moment. It would be easy to become overwhelmed and frantic, the stress is really building up and can be felt, almost palpable amongst the year 4's.</div>]]></description>
