All the leaders agreed to demand unconditional surrender from Germany. With the surrender of Germany, they also agreed to split Grmany between there countries and France. Stalin agreed to allow free elections in Eastern Europe.
The Yalta conference became controversial after Stalin broke his promise of free elections in Eastern Europe and many people said that Roosevelt "sold out " the Soviets.
This Event and its aftermath is primarily the Soviet union's fault because they broke the promises of having free elections Eastern Europe which led to a breakdown of trust between the US and the Soviet Union. It is partly the US's fault because it caused many anti-Americans to say that the US and Roosevelt sold out the Soviet Union.

The US agreed to help South Vietnam by providing military and police support. In March of 1965, the US decided to send military troops in for a full on battle. 
This war was mostly the Soviet union's responsibility because they were trying to push to spread communism by backing North Vietnam. This invasion was fought by 1400 American trained Cubans. The invaders were heavily outnumbered and surrendered after fighting no more than 24 hours. 
This was primarily the US's fault because they decided to attempt to overthrow Castro and therefor to provoke Castro and his supporters the Soviet Union. This standoff was about the Soviet Unions nuclear missiles in Cuba. This was a huge conflict because the missiles were just 90 miles from the US and could be launched in minutes. This crisis sent the US into panic as Kenedy tried to defuse the situation. This crisis was averted by striking a deal for the US not to invade Cuba, and then the Soviet Union would in return remove the missiles from Cuba. 
This was mostly the Soviet Unions fault because they placed the missiles in Cuba and by doing that provoked the United States by having the missiles that close.
