Hypotes:

In human-centred design (HCD), researchers and designers attempt to cooperate with and learn from potential users of the products or services which they are developing. Their goal is to develop products or services that match users’ practices, needs and preferences. In this position paper it is argued that HCD practitioners need to deal with two tensions that are inherent in HCD: they need to combine and balance users’ knowledge and ideas with their own knowledge and ideas; and they need to combine and balance a concern for understanding current or past practices with a concern for envisioning alternative or future practices. Six HCD approaches – participatory design, ethnography, the lead user approach, contextual design, codesign and empathic design – are discussed in order to argue that these different approaches are different ways to cope with the two tensions. In addition, several examples from practice are provided to illustrate these tensions. Moreover, it is advocated that HCD practitioners critically reflect on their practices, their methods and their own involvement, so that they can more consciously follow specific HCD approaches and more mindfully cope with the two tensions.

Keywords: human-centred design; participatory design; methods; critical; reflection; reflexivity

AbstrAct re ections upon the meaning
of the word ‘design’ are made and a relatively complete de nition of the paradigm of human centred design is formulated. Aspects of both the background and the current practice of the paradigm are presented, as is a basic structural model of the design questions addressed. Examples are provided of the economic bene t of human centred design in business settings as an approach for designing products, systems and services which are physically, perceptually, cognitively and emotionally intuitive. Examples are further provided of the coherence of the paradigm with the logic and structure of several currently popular marketing and banding frameworks. Finally, some strategic implications of adopting human centred design as a business strategy are suggested.

KEYWORDS: people centred design, human centred design, design process, innovation model

Joseph Giacomin

Human Centred Design Institute, Brunel University, UK

" Unlike the humanities with its interest in under- standing the past, informatics is interest- ed in designing the future." (Dahlbom, 96:42).

Use of Information and Communication Technologies Among Older People With and Without Frailty: A Population-Based Survey

Background

Use of information and communication technologies (ICT) among seniors is increasing; however, studies on the use of ICT by seniors at the highest risk of health impairment are lacking. Frail and prefrail seniors are a group that would likely benefit from preventive nutrition and exercise interventions, both of which can take advantage of ICT.

Objective

The objective of the study was to quantify the differences in ICT use, attitudes, and reasons for nonuse among physically frail, prefrail, and nonfrail home-dwelling seniors.

Methods

This was a population-based questionnaire study on people aged 65-98 years living in Northern Finland. A total of 794 eligible individuals responded out of a contacted random sample of 1500.

Results

In this study, 29.8% (237/794) of the respondents were classified as frail or prefrail. The ICT use of frail persons was lower than that of the nonfrail ones. In multivariable logistic regression analysis, age and education level were associated with both the use of Internet and advanced mobile ICT such as smartphones or tablets. Controlling for age and education, frailty or prefrailty was independently related to the nonuse of advanced mobile ICT (odds ratio, OR=0.61, P=.01), and frailty with use of the Internet (OR=0.45, P=.03). The frail or prefrail ICT nonusers also held the most negative opinions on the usefulness or usability of mobile ICT. When opinion variables were included in the model, frailty status remained a significant predictor of ICT use.

Conclusions

Physical frailty status is associated with older peoples’ ICT use independent of age, education, and opinions on ICT use.
Using Smartphones and Health Apps to Change and Manage Health Behaviors: A Population-Based Survey

Chronic conditions are an increasing challenge for individuals and the health care system. Smartphones and health apps are potentially promising tools to change health-related behaviors and manage chronic conditions.

Objective

The aim of this study was to explore (1) the extent of smartphone and health app use, (2) sociodemographic, medical, and behavioral correlates of smartphone and health app use, and (3) associations of the use of apps and app characteristics with actual health behaviors.

Methods

A population-based survey (N=4144) among Germans, aged 35 years and older, was conducted. Sociodemographics, presence of chronic conditions, health behaviors, quality of life, and health literacy, as well as the use of the Internet, smartphone, and health apps were assessed by questionnaire at home visit. Binary logistic regression models were applied.

Results

It was found that 61.25% (2538/4144) of participants used a smartphone. Compared with nonusers, smartphone users were younger, did more research on the Internet, were more likely to work full-time and more likely to have a university degree, engaged more in physical activity, and less in low fat diet, and had a higher health-related quality of life and health literacy. Among smartphone users, 20.53% (521/2538) used health apps. App users were younger, less likely to be native German speakers, did more research on the Internet, were more likely to report chronic conditions, engaged more in physical activity, and low fat diet, and were more health literate compared with nonusers who had a smartphone. Health apps focused on smoking cessation (232/521, 44.5%), healthy diet (201/521, 38.6%), and weight loss (121/521, 23.2%). The most common app characteristics were planning (264/521, 50.7%), reminding (188/521, 36.1%), prompting motivation (179/521 34.4%), and the provision of information (175/521, 33.6%). Significant associations were found between planning and the health behavior physical activity, between feedback or monitoring and physical activity, and between feedback or monitoring and adherence to doctor’s advice.

Conclusions

Although there were many smartphone and health app users, a substantial proportion of the population was not engaged. Findings suggest age-related, socioeconomic-related, literacy-related, and health-related disparities in the use of mobile technologies. Health app use may reflect a user’s motivation to change or maintain health behaviors. App developers and researchers should take account of the needs of older people, people with low health literacy, and chronic conditions.

Keywords: telemedicine, eHealth, mHealth, smartphone, mobile apps, health promotion, chronic disease, health literacy, quality of life

Clemens Ernsting,

1 Stephan U Dombrowski, PhD,2 Monika Oedekoven,1 Julie L O´Sullivan,1 Melanie Kanzler,3Adelheid Kuhlmey, PhD,1 and Paul Gellert, PhD1

The number of smartphone users is growing rapidly, including among healthcare professionals. The purpose of this study was to classify smartphone-based healthcare technologies as discussed in academic literature according to their functionalities, and summarize articles in each category.

Methods

In April 2011, MEDLINE was searched to identify articles that discussed the design, development, evaluation, or use of smartphone-based software for healthcare professionals, medical or nursing students, or patients. A total of 55 articles discussing 83 applications were selected for this study from 2,894 articles initially obtained from the MEDLINE searches.

Results

A total of 83 applications were documented: 57 applications for healthcare professionals focusing on disease diagnosis (21), drug reference (6), medical calculators (8), literature search (6), clinical communication (3), Hospital Information System (HIS) client applications (4), medical training (2) and general healthcare applications (7); 11 applications for medical or nursing students focusing on medical education; and 15 applications for patients focusing on disease management with chronic illness (6), ENT-related (4), fall-related (3), and two other conditions (2). The disease diagnosis, drug reference, and medical calculator applications were reported as most useful by healthcare professionals and medical or nursing students.

Conclusions

Many medical applications for smartphones have been developed and widely used by health professionals and patients. The use of smartphones is getting more attention in healthcare day by day. Medical applications make smartphones useful tools in the practice of evidence-based medicine at the point of care, in addition to their use in mobile clinical communication. Also, smartphones can play a very important role in patient education, disease self-management, and remote monitoring of patients.

Just lIke computers and the Internet, mobile phones are becoming common personal items for older persons. In early 2006, 60 percent of people aged 65 to 74 years and 36 percent of peo- ple aged 75 years and over owned and used mobile phones in the UK [1]. Actually, older people have a much higher adoption rate of mobile phones than of Internet usage [2]. Many older persons use mobile phones in both leisure and work contexts.

However, while for some older persons, com- municating using mobile phones seems to be an enjoyable experience, most mobile phones had not been designed with older people in mind and are usually difficult for them to use [3]. Most complaints are related to displays that are too small and difficult to see; buttons and characters that are too small, causing users to push wrong numbers frequently; functions that are too many and too complex, non- user-friendly menu arrangement; unclear instruction on how to find and use some functions; and services that are too expensive.

If the problems related to the use of mobile phones can be solved, mobile phones can potentially play an important role in maintaining quality of life for older people in many ways. They provide a sense of security and safety for elders: By carrying mobile phones, other people can reach them practically any- time and anywhere [4]. Past studies show that when asked, older people were quite vocal and creative in specifying desired innovations, such as a health-mon- itor function, speech activation, or a security-bracelet function that can send calls for help [5].

In a research project commissioned by the British Society of Gerontology, people 60 years old and above were actively involved in exploring issues related to their mobile phone use with the intent of translating the findings into a set of requirements for the design of a senior-friendly phone, through a combined approach of qualitative and quantitative methods (content analysis of expert interviews and

focus group discussions, as well as statistical analysis of an online questionnaire designed in collaboration with the focus group) to arrive at a more nuanced understanding of these issues.

the Issues

Through a combination of the investigation into usage patterns, problems and concerns, perceived benefits, and desired and unwanted features, we uncovered the following issues:

The most-called person: partners.

The three important benefits of having mobile phones for older persons are: 1. They know
they can always call somebody when they are in trouble; 2. They feel more confident to go out by themselves; 3. They feel safer when they are alone.

The three most-desired functions are address book, diary, and alarm clock (arguably, these functions share a common characteristic of being memory aids).

The top three functions considered unnecessary are music player, camera, and videophone (these are entertainment-related functions, which many older persons would not associate with a commu- nication device).

The three most difficult aspects of using mobile phones are learning to use, navigating menus, and choosing the right option.

the desIgn

Shifting through the problems with the current design of the phones used by the study participants and the participants’ opinions on the various aspect of physical design, a set of requirements for the physical design elements that were perceived to be senior-friendly are:

Mapping mHealth research: a decade of evolution

In the last decade, mobile health (mHealth), the branch of eHealth broadly defined as “the use of mobile computing and communication technologies in health care and public health” [1], has been constantly expanding. Mobile applications for health can target heterogeneous audiences such as doctors, nurses, patients, or even healthy people [1]. Different features of mobile phones may address specific needs in different situations. Available literature suggests that the use of mobile phones serves a wide variety of purposes [2], such as smoking cessation, weight loss, diet and physical activity, treatment adherence, and disease management. The biggest advantages of using mobile devices, and in particular mobile phones, for health are that these devices are personal, intelligent, connected, and always with people [3,4]. Therefore, they can serve patients both in everyday life and during hospitalization or rehabilitation, as well as health care providers during emergency or routine visits. Current evidence suggests that the use of mobile technology can improve diagnosis and compliance with treatment guidelines, as well as patient information, and can increase administrative efficiency [5]. In particular, short message service (SMS) text messaging reminders have been shown to be a simple and efficient option for health services to use in order to improve service delivery, resulting in health benefits for the patients who receive them [6]. Mobile phone technologies have also been shown to be effective in smoking cessation, weight loss, physical activity, diabetes management, STD prevention and treatment, and hypertension [7].

The mobile phone market is constantly evolving. The first digital mobile phones appeared in the early 1990s, and since then, mobile technology has continued to be refined thanks to the development of new features and better networks. Current smartphones have been defined as “mobile telephones with computer features that may enable them to interact with computerized systems, send e-mails, and access the web” [8]. Over a third of US mobile phone users own a smartphone [3,9], and it is estimated that 67.6% of adults worldwide own a mobile phone [2,10], making it the most equitable communication technology [1]. It has been argued that mobile phones could be a solution to overcome the traditional digital divide derived from the introduction of the Internet because they provide new opportunities to reach underserved and previously unreachable parts of the population worldwide, especially in developing countries [2].

Mobile technology, with its diffusion and characteristics, holds a great potential for health care applications. However the use of mobile phones in health care delivery has not been fully explored, and the diverse outcomes of mHealth have barely been documented. Although some literature reviews cover one part or the other of the field [6,11,12], an overall picture is still missing, possibly due to the field’s constant evolution. A recent methodological review sought to map the domain of mobile phone health interventions [13], but it relied on describing the design of the interventions, with a clear focus on technology, rather than the outcomes. As the authors stated, their motivation lay in the fact that “effectiveness reviews can be best done at the level of a particular pathology”, while they wanted to draw a more comprehensive taxonomy of the field.

The main objective of this paper, as stated in the title, is to map the field, but without omitting the outcome measures. This means that our intention is to investigate how the impact of mobile phones on health has been assessed in peer-reviewed scientific literature. In particular, we are interested in understanding the evolution over the past decade, how the interventions have been developed, the main health care delivery areas where the impact of mobile technologies has been assessed, the methodology and features used, and finally, the type of outcome measures and general impact of the intervention.

The second objective of this review is to understand, after the 5 years since the introduction of the new generation of smartphones (eg, the iPhone in 2007), whether and how these devices have triggered research. The appeal of these new devices resides in the fact that they include several computer-like built-in features (eg, the GPS or the accelerometer) allowing the monitoring of a whole series of behaviors. Additionally, new mobile operating systems allow users to customize their devices according to their needs, by downloading apps available for free or for a low price from a central store. Klasnja and Pratt named this kind of feature “native application” [13], which is a typical complex and sophisticated application that can be implemented on major smartphone platforms (iOS, Android, Symbian, BlackBerry, webOS, and Windows Phone). In 2012, smartphone users spent US $8 billion for paid apps in the top 5 app platforms, and the European mobile app market size reached €1.68 billion [14]. Therefore, iPhones and similar devices are potentially very interesting for application in health care—they already integrate most of the features that researchers previously had to add to traditional mobile phones in order to use them for health-related purposes and monitoring [15,16].

http://zl7zn5jl8u.search.serialssolutions.com/?ctx_ver=Z39.88-2004&ctx_enc=info%3Aofi%2Fenc%3AUTF-8&rfr_id=info%3Asid%2Fsummon.serialssolutions.com&rft_val_fmt=info%3Aofi%2Ffmt%3Akev%3Amtx%3Ajournal&rft.genre=article&rft.atitle=Mapping+mHealth+research%3A+a+decade+of+evolution&rft.jtitle=Journal+of+medical+Internet+research&rft.au=Fiordelli%2C+Maddalena&rft.au=Diviani%2C+Nicola&rft.au=Schulz%2C+Peter+J&rft.date=2013&rft.eissn=1438-8871&rft.volume=15&rft.issue=5&rft.spage=e95&rft_id=info%3Apmid%2F23697600&rft.externalDocID=23697600&paramdict=sv-SE
