I will send home a note with information about the group for their parent or guardian to sign and return. 

If any student is not returning the note I will visit with the student and then contact home so students are able to join the group.

During these interviews I will explain the purpose of the group, assess their level of commitment by asking questions, and then choose group members who I think will participate in the group. 

Since the goal of the group is to change their behavior around attendance, some students who are invited may be reluctant to come. The counselor on my campus said she has a simple form that she sends out at the beginning of the year so students and families understand confidentiality. She keeps those on file as a blanket statement for informed consent. 

I used the provided template and created an informed consent specifically for this group. Stage 3
During the last group we will review data from last semester and the current six weeks to compare. Each group member will set goals for the next six weeks. I will explain that I am still there even though our group is ending.
