If you feel confident with it, you may even wish to do it with your eyes closed for some added adventure. This will give you bonus points in neuromuscular control and stability. But make sure you do it under supervision as this can make you wobbly.

This form should be completed during your follow-up visit with your rehabilitation specialist to ensure your body is ready for the next phase. All questions in this form must be answered "Yes" in order to progress to the next phase. If any answer is "No", that’s completely okay. It just means we will spend a bit more time strengthening that area before moving forward. Remember, rehab is not a race.

To calculate your score, add up your scores and divide by 10. The grading criteria is as follows (Glazer, 2009):

60 = full confidence to return

40 = moderate confidence

20 or below = low confidence, may need more time.

Don’t worry if your score is below 20. It does not penalise you. It simply helps your rehab team to support your journey with input from experienced coaches and, if needed, sports psychologists.
