1.non perishable foods-canned foods as they don't have a due past date, allowing much to be stored for a long time
2.electrical generator-preferably a wind turbine or solar panel for infinite energy-used to power radios and machines
3.spare materials-due to the un certain nature of the catastrophy it is highly likely you'll need materials to repair your shelter and/or trade with other humans for more resources

4.weapons-now weather or not you believe in guns or not you will need weapons, either to fight off bandits or animals to defend your resources-for this I would suggest a clip fed weapon that is simple enough to be able to be repaired by you or a friend
5.radio-after the great melt most forms of communications will probably be unusable, and radios are incredibly sturdy and easy to use once you get used to them, they are priceless to communicate to other survivors
6.books-get books that teach you how to survive, any survival books will certainly help, as well as some educational books, so the next generation will have the same knowledge and skills as the pre melt ones have
7.flares-when radios don't work or you don't have enough time, flares can be used to signal to other survivors that you are under attack or in danger
8.water filters-with the fall most water sources will probably be either to salty to drink or dangerous to drink, so water filters shall help you, get as many as you can so you will always have a source of fresh water
9.clothes-you will need as many pieces of clothes and the materials to repair said clothes, as they will get worn down incredibly quickly what with the work you will have to do as well as all the dangers you could face
10.a good pair of boots or 3-as in most places walking with sturdy shoes will be a death sentence, as your feet will probably be destroyed either due to poison, or tripping or scratching something, so a good pair of boots that allow you to go over hazardous terrain is incredibly important

if you pack and prepare sufficiently, you could survive for many years off of that food, but it will eventually run out, no matter how much you pack, so an easy solution to it is to farm. That is why you will need to pack some seeds of many different plants, like carrots, lettuce, and (most importantly) potatoes.
it has been proven that as long as a human has supplements of vitamins, a human can survive indefinitely on potatoes, so pack a lot of them.
But one of the major problems of farming in this brave new world, is the fact that with the weather being as screwed as it is, you will need to keep it covered or even underground, Eg using lamps to grow it underground, but this will be a huge toll on your resources and energy so it is recommended if you can to farm outside.

Now as anyone familiar with any post apokolyps scenario knows, clothes are very important for you, if you wear the right ones you can survive for longer and make yourself look awesome. But that is not what I am here to tell you about, because looking cool is the last thing you want in your clothes line, simple things that are easy to repair and thick or thin enough to survive your local temperature, as well as strong enough to be able to protect you.
Any armour that you can get(weather that be military grade kevlar or plastic toy armour) will help, but again, it should be the best armour and clothes you can get.
(NOTE:gas masks will probably not be needed if you don't live next to the following, military base, nuclear power plant, normal power plant, etc)
boots are also incredibly important, as good pairs can last for years and whether incredible things that most other shoes will not.
