Believe it or not The US Dept. of Agriculture has a whole page devoted to explaining what a needs assessment is and when to conduct one. They also give some example forms and examples of when conducting a comprehensive needs example would be appropriate.  Even though Humanitarian Emergencies is not necessarily an interest of mine, I think this resource is an excellent resource and is actually a current concern due to the issues with illegal immigration.
