Dynamic DUO- InPen smart insulin pen and Simplera CGM

I wanted to share some exciting information about new technology that is revolutionizing diabetes management. The "dynamic duo" of continuous glucose monitoring (CGM) systems and smart insulin pens is making diabetes management simpler and more intuitive. The constant finger sticks, logging, and calculations traditionally required for blood glucose monitoring are quickly becoming a thing of the past.

One such innovation is the Medtronic Simplera, a CGM sensor that you can place on the back of your arm to monitor glucose levels in real time. This eliminates the need for lancets, glucose strips, and traditional blood glucose meters. The Simplera CGM is suitable for individuals with gestational, prediabetes, Type 1, or Type 2 diabetes. It combines the sensor and transmitter into a single, fully disposable, and single-use device. Its flat, square design features an anti-lint adhesive engineered to resist sweat and water, protecting the sensor. The device requires a two-hour warm-up period and lasts for seven days. Additionally, it is compatible with the InPen smart insulin pen. The InPen provides personalized insulin dosing guidance for individuals on multiple daily injections through its integrated app, which features a bolus calculator that considers factors such as carbohydrate ratios, meal estimations, insulin on board, and fixed dosing regimens (Hetzel, P., 2025).

According to Medtronic, the Smart MDI system is allowing users to achieve better glucose levels. Real-world data collected from 1,852 Smart MDI system users across Europe who self-reported living with Type 1 diabetes showed a mean percentage of time spent in the range of 70-180 mg/dl (3.9-10.0 mmol/l) of 55.7%. Individuals who used the Smart MDI system as intended, reacting to >75% of "Missed Dose" or "Correct High Glucose" alerts with a bolus, had a higher average Time in Range (TIR) of 67.2% and 71.5%, respectively (Medtronic, 2025). This indicates that the actionable alerts help users achieve better glycemic outcomes. As more data becomes available, I am confident that Medtronic will continue to advance its system, further benefiting the diabetic community.

I wanted to share some of the key benefits of using the InPen and Simpera CGM system:

No continuous finger sticks: This system eliminates the need for constant finger pricks throughout the day.

Real-time data: Readings are sent directly to the mobile app every five minutes, allowing for real-time viewing of glucose levels, dosage recommendations, and glucose trends.

Helpful reminders: The InPen app provides reminders for long-acting insulin, ensuring you never forget a dose. It also alerts you for insulin that is too high or too low, and provides insulin dose suggestions.

Convenient and durable components: The sensor lasts for seven days, the insulin pen lasts up to 12 months with refillable cartridges, and the transmitter is rechargeable.

Storage alert: The pen will send alerts if it gets too hot, ensuring the insulin remains at the required storage temperature to work properly.

Easy data sharing: Data can be easily shared with a healthcare provider or family members directly through the InPen app.

Improved glucose control: Overall, the system can lead to better glucose management.

Here are some cons I've identified for Simplera and InPen:

Phone Dependency: Both Simplera and InPen require a connected Android or Apple iOS phone to monitor glucose levels. Your phone must be within a certain range, and if it dies, you will lose the ability to monitor your glucose.

Simplera CGM Warm-up Time: The Simplera Continuous Glucose Monitor (CGM) requires a two-hour warm-up period.

Visible Simplera CGM: The Simplera CGM is visible when worn, typically on the back of the arm, which may be a concern for some users.

Smart Insulin Pen Limitations: The smart insulin pen only administers short-acting insulin. If you are prescribed both short- and long-acting insulin, you will still need to carry a separate, traditional pen for the long-acting dosage.

Opinion and Rationale

I want to express my strong belief in the transformative potential of the Simplera CGM and InPen smart insulin pen in the diabetes industry. I am confident these technologies will empower users with better, tighter control over their insulin management, provide crucial assistance during forgetful moments, and offer timely alerts for both high and low blood sugar levels.

My personal experience with Gestational Diabetes during pregnancy highlighted the challenges of traditional glucose monitoring. The constant logging, cleaning, pricking, and checking of blood sugar were burdensome, and I frequently missed documenting readings in my log journal, leading to incomplete information for my doctor. Due to lapses in self-care, I experienced multiple hypoglycemic episodes at work. Fortunately, I never became hyperglycemic, but I recognize that I put both myself and my baby at risk, as well as my patients. I firmly believe that if Simplera had been available to me then, I would have been able to monitor my glucose levels more effectively and provide more accurate documentation to my physician.

My brief time as a diabetic opened my eyes to a deeper level of empathy for individuals living with this condition. The daily routine of multiple finger pricks and strict dietary management is a significant chore. While we often speak to our patients about diabetes management as an "easy life-changing style," it is far more complex. Medtronic's "dynamic duo" of Simplera and InPen offers a comprehensive solution, streamlining logging, reducing the frequency of finger pricks to once a week, and providing essential alerts. I believe these innovations will enable the diabetic community to achieve better control over their glucose levels and significantly improve their quality of life.
