         <description><![CDATA[<div><strong>Discussion 1 - Issues<br></strong>The first issue is human resource. Rad was hired without proper qualification. The human resource department should has provided a specific job description and job specification. The hiring that was done should be based on these as well as the department need.Apart from that, the head of department should implement a standard operation procedure in applying leave so that, the case of understaffing will never happened. In case of understaffing, the new worker need to be competent in handling multiple modalities. <br><br>The second issue is employee incompetency and professionalism . Rad was unable to show his professionalism when he hesitate and not willing to learn about new knowledge involving the fluoroscopic procedure by giving a lot of excuses. The management should ensure that all new employee should undergo training so that they are competent in handling all modalities present in the department. This can also happend as a result from Rad previous employer. The employer may had not prepared a proper rotation schedule of the modalities in the department so that all worker are able to perform the procedure when needed. <br><br><strong>Vignette 1<br></strong>The main issue is responsibility. The student, the radiologist and also the radiographer are at fault. All of them take the patient lightly. As a radiographer, the student is his responsibility. He should never play a fool regarding the case or patient that he handle. He should give proper instruction and example to the student. He should also be present and monitor the student during the procedure. On the other hand, the radiologist is also neglecting his responsibility by not checking the content of the syringe before injecting into the patient. The student also should not follow blindly any instruction given. He or she must equip himself with the necessary knowledge before going to clinical placement to avoid any accident. All of them may be accused for malpractice and negligence. As conclusion, every party should play their respective role and be responsible for their action. Always take the job seriously and never take thing for granted as a small mistake can cause a loss of life.</div>]]></description>

         <description><![CDATA[<div>Discussion 1</div><div>The first issue is that is evident in this scenario is understaffing in the Radiology Department in the diagnostic area. This is evident when Rad need to cover the fluoroscopic examinations eventhough he is in charge of computed tomography.&nbsp;<br><br></div><div>The second issue is poor management in work distribution. This is shown when there is understaffing at the diagnostic area while the staff at the computed tomography area might be enough as there is decrease in patient examinations in the area.&nbsp;<br><br></div><div>The third issue is the competency of the radiographer. This evident Rad mentioned that he has not work in the diagnostic area for 15 years thus making him unfamiliar with the fluoroscopic procedure and would not perform a quality radiograph.&nbsp;<br><br></div><div>The last issue is about the hiring contract. What has been agreed in the contract is Rad is only hired as the Computed Tomography technologist only. Thus he can refuse the request by Roentgen to cover the fluoroscopic work.&nbsp;<br><br><br>&nbsp;</div><div>Vignette 1&nbsp;<br><br></div><div>Question:&nbsp;<br>1.Who is / are the responsible parties in the above incident?</div><div>First is the radiographer. This is because due to the attitude of the radiographer that take lightly of his responsibility as a professional. He is not responsible as he leaved the student radiographer without supervision. He also give the wrong instruction to the student radiographer which involve the administration of the medication that can give harm to the patient.&nbsp;<br><br></div><div>Second is the radiologist. This is because lack of communication between the radiologist and the student radiographer. He does not bother to ask and check the content the syringe before administrating the medication to the patient.&nbsp;<br><br></div><div>Third is the student. The student does not inform the radiologist about the content of the syringe. The student also should have the basic knowledge of the procedure .<br><br>2.
