         <title>After certain high profile activities or meeting we hold a &quot;lesson learnt&quot; for all staff involved to share their thoughts on what worked and what didn&#39;t.  Honesty is encouraged and everyone has a chance to speak.  These sessions can influence Governance related decisions as there may be a need for a change in procedure if it is found not be working effectively.</title>

         <title>  I think rather than just hold these meeting for events high profile meetings we could have a session regarding the processes we have in place to Govern how we meet our Organisations KPIs to ensure these are working for the organisation as a whole.  These KPIs are reporting to our Governing body and shape they way certain decision are made if the process changes the outcome may change as well. </title>

         <description><![CDATA[<p>We have regular meetings to discuss regulatory compliance against Policy. These meetings involve several business units to ensure all the relevant stakeholders are present and can openly communicate their perspective.</p><p>These meetings are essential to decide whether there has been a breach or not and whether we need to remediate a customer/ore report a breach to the regulators.</p><p>Initially these meetings were very collaborative, and the outcomes were agreed to by all parties. Slowly though as the leadership changed due to restructures, the element of trust was eroded, and one particular business unit became the deciding power. This then impacted the shared goals and expectations/behaviours of the group as a whole. </p><p>This shifted the communication and negatively impacted collaboration. </p><p>To overcome this, a clear definition of roles and responsibilities needs to be revisited along with the rules of engagement.</p><p>New shared goals need to be set and confirmed with buy in from all stakeholders to foster greater communication and respect for each stakeholder and their expertise and impact to the collaboration.</p>]]></description>

These are team-led not executive-led.  We work hard to minute and disseminate agreed actions and next steps.  Ideas are encouraged and explored, or at worse parked for further examination</p>]]></description>
