ESPECIALLY THE SANDWICH GENERATION!</em></strong><br>-Nurses can assist caregivers to help them understand and cope with the stressors they are experiencing. <br>-Monitor the caregiver for declining health and emotional distress<br>-Help them acknowledge their personal feelings of stress <br>-Reinforce the importance of eating healthy and at regular times<br>-Let them know exercise can help relieve their stress<br>-Tell them to get adequate sleep<br>-Watch for lack of activity because that can indicate that the caregiver is depressed<br>-Nursing can encourage the caregiver to keep a journal to express their feelings<br>-Encourage them to continue their social activities, interests and hobbies<br>-Help them to see the positives<br>-Encourage them to seek spiritual support<br><strong><em>YOU CAN ALWAYS TRY TO CONTACT COMMUNITY RESOURCES TO HELP! SUCH AS MEALS ON WHEELS, COMMUNITY DRIVERS, ETC. SOCIAL SERVICE YOU MAYBE TO GO BACK TO IF THE PERSON WAS DC FROM THE HOSPITAL AND THINGS ARE NOT WORKING OUT.</em></strong></div>]]></description>

         <title>Medical Model:</title>

         <description><![CDATA[<div>Coordinates medical services, it was traditionally designed to be diagnosis specific. As healthcare evolved so has the medical model, as services are needed by individuals for coordination of multiple treatment and multiple conditions.<br>Benefits: Empowers patients to take responsibility for your healthcare. Ex. Mayo Model of Care (Football team-PCP: Head coach, Specialty Care: Assistant coaches, RN: Athletic Trainer).<br>Challenges: Trying to coordinate care across multiple services/places when patient is discharged from hospital. Insurance may not pay for a particular assisted living facility that the hospital recommends.<br>Financial is also an issue, as funding and financing care is not always possible. Ex. Needs to go to SNF but get home health instead due to cost.</div>]]></description>

         <description><![CDATA[<div><br>- The integrated care coordination model combines both medical and *****<strong><em>social services;***** </em></strong>coordinates various services and settings over continuous length of time. <br>- The integrative approach recognizes that patients have needs that that go beyond the healthcare system. By integrating health care alongside various forms of social and community support this type of model improves patient care, patient independence, promotes overall health, and ultimately seeks to reduce unnecessary service utilization.&nbsp; <br>- One of the biggest challenges of this model is attempting to have different service types financed from different sources. <strong><em>Miscommunication between various service types also poses potential challenges. The integrative model can be complicated even further by the scope of authority tasked with managing the various services and their levels of involvement. </em></strong><br>- Examples: Medicaid Care Coordination of Co-Morbid Health and Chronic Behavioral Health Conditions, Duval County Health Department</div>]]></description>
