Most people say that the stigma is worse that the actual illness. Let's stop that! People may not want to be treated differently if they address their mental state. They might fear the pity and the thought that they are overreacting. Some people refuse help no matter what it is, so they might be too stubborn and think that they can do it on their own.

Usually by criteria, and if you meet the criteria you will get a diagnoses. the docto has to trust that the patient is being truthful.(Its rare that someone would fake a mental illness infront of a doctor unless they were an addict looking for meds, and in that case different measures would be taken)
I don't have an answer to te second qustion, sometimes its just passed down through genetics.

Depending on the mental illness, for example, depression, if someone were to reveal something that they wanted to talk about like a breakup, for example, Will Smith who talked about his short separation with his wife Jada, some people may be mean and turn him into a meme. This may discourage people, who got turned into a joke, from ever getting help and forcing them to bottle their emotions and eventually may let it out the worst way possible through physical or verbal abuse. Another example of people not getting the help they need is sugarcoating. For example, someone with an eating disorder may ask a close friend or family about how they feel like they may need to find something to treat their large or unreasonably small eating habits, but the people they talked to may say that "it's okay" or "You're fine just the way you are," discouraging them or making them blissfully ignorant of the situation they may have.

Not only that, on a practical side, if the person in need of help is encouraged to find or seek therapy, there may be some problems with that. One of these are costs since these therapy sessions may not be available of affordable to all who, for example younger adults, are still trying to make money. Another problem could be that there is not a place to get help or therapy where they live, leading them to either becoming discouraged or having to spend more time out of their day to reach the location, seek help, then coming back which may potentially take a toll on their work or school life.
