Growing up, my life has always been filled with science, medicine and hospitals given that my mother is a doctor and, additionally, her brothers and sisters are all in the medical field. My aunt (her oldest sister) has been the nursing officer of the paediatric ward in my hometown’s hospital for almost 25 years, before retiring in 2015.  
Surely, she’s been partially responsible for my choice.  
Furthermore, I consider myself an empathetic and caring girl: my dream, for the longest time, has always been to travel the world and to help the less fortunate (i.e. Emergency, Save The Children etc.) and, last year, I took the opportunity to fulfill this dream and I enrolled in nursing school. Mom: Thank you, but he can walk slowly. Do you know when can I find the sister? I have some questions for her. 
 
Nurse: You can find her from 8am to 2pm in the office in front of the doctor's one. If you can’t find her, I'm sure that the paediatricians will be more than happy to answer in her place. Oh, here we are: this is the paediatric ward.

“Scrubs” is a tv show set in the early 00’s that follows the journey of J.D., a new intern at Sacred Heart Hospital, and his colleagues (and at some point, friends) during the 9 years of his internship and first work as a consultant in the hospital. 
Thanks to some really good music, witty jokes, dress-ups, iconic quotes but also sad and eye-opening stories, it has reached, at least in my opinion, the status of best tv show ever. 
Apart from the above qualities, I found really interesting to watch how the daily life of the ward unfolds: from the stoic Dr. Cox, to the down to earth and amazing nurse Carla and her husband Turk, JD’s best friend, you can empathize with all the characters, because you can feel their love for the patients and for their job.

  • Stretcher -> The patient had been in a car crash so, to avoid any unnecessary movement, he was transported into the ER on a stretcher. 
  • Night shift -> I’m working a night shift tomorrow: I know that many people hate it but I really like the freedom that it gives me during the day and I don’t find it too tiresome either. 
  • Charge nurse -> In our ward, we don’t have a sister but a charge nurse: Robert’s really competent and clearly loves his job!
  • Cardiology -> The cardiology ward is right next to the main entrance: you just have to go through the swing doors on the left.
  • Microscope -> I've always dreamt of working as a pathologist: I'd love to analyse samples at the microscope and study many illnesses.
  • Porter -> My friend works as a porter in the nearby hospital: he walks a lot (too much, sometimes!) but he really loves to help patients and the entire medical equipe. 
  • Wheelchair -> When I was young, I broke my leg and had to use the wheelchair for the entire summer and I realized how many things we, able-bodied people, take for granted every day. 
  • Triage nurse -> The triage nurse in our hospital is amazing at her job because she can almost immediately spot a life-threatening condition and acts as quickly as possible. 
  • Blister -> I can’t believe I managed to wear those uncomfortable high heels for the entire day: they even gave me the worst blisters ever.
  • CPR -> My boyfriend is a paramedic and the first thing they learn about is the importance of CPR during a cardiac arrest and how much the chest compressions can dramatically improve the chances of survival.
  • Local anaesthetic -> The surgery only requires a local anaesthetic because the patient has to undergo a simple procedure and can return to his daily activities after just a couple of hours. 
  • Waste -> In the hospital, the general waste is disposed by the city’s waste company while the medical waste is disposed, at first, by the porters and then by a specific medical waste company. 
  • Migraine -> The dog has been barking for hours and this gave me a terrible migraine: my head is spinning and I feel a throbbing pain near my temples. 
  • Painkillers -> The doctor prescribed my mom some painkillers for her recurring back pain but he strongly recommended her to see a physiotherapist. 
  • Cough -> One of the first and most common symptoms of Covid-19 is cough associated with fever, fatigue and difficulty in breathing.

    • Empathy, a basic but extremely important skill to help communicate with patients and to understand their needs, even when they don’t communicate them directly 
    • Scientific knowledge, because science and evidence based nursing are fundamental during hospitalization and during every clinical treatment
    • Organization and co-operation: it’s important to be organised and to co-operate with the equipe to take a better care of the patient and to reduce clinical risks (such as giving the wrong drugs to a patient) 
    • Strong will, because you will have to work long hours, you will be tired and also you could get mistreated by rude patients 
     
    I don’t know if I'll be a good nurse, but i am a strong believer of working hard to become, eventually, who you want to be. Even though some of these skills are an innate talent, no one prevents me from learning and improving them with time. So, I hope to become a good nurse, with time and experience.

    Typhoid Mary
    The title of this article,"Typhoid Mary", has a double meaning: not only it describes the protagonist of the story, Mary Mallon, but it is also used nowadays to indicate the carrier of a disease that does not want to co-operate with hospitals and autorities. 

    Mary was an irish immigrant that, probably, contracted a mild case of typhoid, but was asymptomatic: typhoid is an infection of the digestive system that causes many symptoms such as weakness, fever and delirium. 

    Typhoid is transmitted through orofecal transmission, mainly due to contaminated food and/or water: it does not surprise us that Mary was a cook and that’s how she infected at least 40-50 people. Through a precise investigation, George Soper found out that Mary was the “healthy” carrier of typhoid but she never wanted to help autorities and was forced to stay in isolation for more than 20 years and eventually died in 1938 of pneumonia.

    The nurse is doing an assessment of a new patient: a 16-year-old girl named Anita. 
    She was admitted for a 48 hours observation because she fainted and she appears to be very very thin: she weights less than 40 kg. They are almost sure that she is anorexic and that she is obsessed with dieting: she doesn’t eat and gets stomachache and constipation. 
    Anita doesn’t know why she feels miserable and doesn’t understand that she has a weight problem because she thinks that she eats as everyone else. However, she noticed some changes in her health and body: sleep problems, losing hair and not having her periods.

    I didn't know the origin of "typhoid Mary".  I was vaguely familiar with her.  The summary is so timely considering the pandemic period in which we have been living for almost 2 years.  I guess today she would be considered no vaxxer/masker .     :(

I was really impressed about his method of teaching: since I was his first, I think he was trying to experiment new ways of relating and interacting with students. Nonetheless, he was amazing: he helped me to understand my clinical priorities of the day by letting me make mistakes and think clinically. At the start of the shift, he would let me read the nursing handover notes from the earlier shift and let me assess what we both had to do for the day: he wouldn’t comment on it until much later. At first, I wasted a lot of time doing things that weren’t useful or were not prioritary but, by the end of my clinical practice, I was able to independently care for my “assigned” patients of the day, with his supervision, of course.
