KK posting for 4th year is quite different from KK posting during 3rd year as this time more focused on the quality of consultation compared to last year was to appreciate the teamwork of healthcare team from different units to run a clinic. It’s a great exposure and teaching to know the approach and consultation for both acute complaint and follow-up for chronic illness. Perceipting setting made me to realize how a doctor making decision and conducting a consultation, no longer as an observer. Besides, the role of referral letter is again emphasized through observation in consultation room. Referral letter from hospital should be complete and concise, for patient to have further management at primary care, or its hard for doctor to make adjustment according to patient’s word. We know audit is important and why should we do, however it’s too hard for us to understand the whole process in one tutorial with SVs. Indeed, it should be our initiative to approach SV if there is questions. When we don’t understand what we don’t understand, how we ask questions? I would like to suggest to have one more tutorial after data analysis then only proceed to final presentation.

I learnt a lot during this posting. It is truly an eye opener to me as Family Medicine is far not just about treating "URTI". Family medicine handles a person from womb to tomb which is very holistic. In 4th year, I get to know more managements on different conditions. Most importantly, I understand the limitations in clinics and the referral criteria for different diseases. The discrepancies between government clinic and private clinic were also noticed. The part when I enjoyed the most was during the precepting session as I felt the excitement and satisfaction in managing my patient. I want to take this opportunity to thank all the Family Medicine lecturers for giving us so much exposures and insights in Family medicine as well as guiding us to become more professional, competent doctors. - Steven Lee

To be honest. It's indeed a great kick-start for fourth year. I personally enjoyed it a lot. The best part of this posting, for me, I was able to relate to whatever that's being taught relatively easily compared to my third year in medical school. Thanks a ton, Drs and Profs.

So far this is my most favourite posting to the extend that i want to consider fam. med for post grad! I used to wonder how can a doctor manage a patient 'holistically' and 'treat not only the patient but also family member and community' in real life when a doctor only meet a patient less than 10 minutes. But now i know how it can be possible. I also learnt that i should always do self reflection and improve my patience when dealing with difficult patients. This is all thanks to the doctor that is now my real life idol (Dr. Z). I also would like to say thank you to all upm fms for educating, encouraging and always do your best for us.

A nice posting to be starting my forth year with. Genuinely, enjoyed it as I got to witness first hand on the management of patients in private gp which is quite eye opening to be honest. It totally broke the stereotype for me that private GP only focuses on certain mild cases as a whole. Nevertheless, I would suggest that there should be more session on discussing bout audit in detail so that the students will be provided a clear picture on the requirement of the audit project itself. I believe this will also improve the final audit presentation drastically. With that said, I genuinely feel thankful towards all the doctors guidance, be it our lecturers or medical officers in places we were assigned to. Your guidance played a huge part of our journey in this posting. Thank you

I have also learnt the steps to set up a private clinic and the different between private clinic and klinik kesihatan.

From this posting, it was an enjoyable learning as we got to go to kk and gp to experience ourselves the differences between govt and private clinic. In term of continuous care, it is better to have follow up in kk. From gp attachment i learnt that it requires hard work and determination to succeed and maintain in business. An eye opener. for the audit presentation, i find that it's the hardest presentation ever as we we're not so clear about what to do.
However, I'm thankful to all the lecturers. I might be interested in becoming fms

Dr Guna is a real friendly doctor that many of his patients respected him and complimented him. He performed relevant history taking and physical examination to each of his patients. Besides, all his patients’ data are stored in computer system which enables him to track the old records, making the consultation smoother and easier for him to follow up the patient. In addition, his clinic has not only medical consultation and pharmacy, but also dressing, nebuliser room, radiography service, laboratory machines for FBC and UFEME, physiotherapy and mini wards for patients needed IV drips. There is a bell in consultation room for doctor to call the nurse/staff easier. The health care staffs work together as a team. The staffs are very friendly also. Truly, dispensary of drug is quite different especially most of medication given are symptomatic relief and sometimes as defensive medicine. And I don't agree that he started antibiotic on viral fever. However, Dr Guna is kind and generous to share his experience on how his clinic is being set up. I learnt a lot and had a lot of exposure.

Family medicine is not an easy posting if you really put effort and time to discover more. I was inspired by a gp to become a doctor when i was still in kindergarten as he could solve every medical illness that i complaint about which gave me a great sense of relieve and satisfaction when i left his consultation room. However from what i observed in this posting, gp really disappoint me a lot. They prioritise money instead of the health of patients. Sometimes you can see that the patients are not satisfied with the consultation doctor but they can't do anything but just to accept whatever a doctor tell. Now i understand why antimicrobial resistant is a concern and who are the culprits of it. Personally i am still interested in primary health care and i hope that i can be a doctor with good qualities who can really contribute to the communities.☺
