He is 72 years old, admitted two days ago with a UTI. He has a diagnosis of moderate dementia and lives alone. His daughter mentioned he is very attached to his routine and his cat.</p><p>&nbsp;</p><p>He is disorientated, anxious, and not responding well to verbal reassurance. He has tried to leave the ward twice in the last hour. I think the unfamiliar environment is contributing to his confusion.</p><p>&nbsp;</p><p>Could you please review him to rule out any worsening delirium or infection? She is known for her strong clinical skills, but she has recently been under significant personal stress due to issues at home. Over the past few months, her colleagues have noticed that she has become increasingly impatient and short-tempered with both patients and staff.</p><p><br/></p><p><strong>Incident:</strong></p><p>One afternoon, Sarah was assigned to care for Mr Tomasz Jankowski, a 75-year-old patient, with dementia who had been admitted for a severe urinary tract infection. Mr. Jankowski was confused, agitated, and resistant to treatment. He repeatedly refused to take his medication and became verbally aggressive when Sarah tried to assist him. After several unsuccessful attempts to administer his medication, Sarah lost her temper and, in a moment of frustration, slapped Mr. Jankowski on the cheek, telling him to "stop being difficult."</p><p>The incident was witnessed by a healthcare assistant who immediately reported it to the nurse in charge. Mr. Jankowski was visibly shaken but uninjured. The nurse in charge immediately removed Sarah from patient care duties and reported the incident.</p><p><br/></p><p><strong>Investigation:</strong></p><p>A formal investigation was initiated, and Sarah admitted to slapping the patient but explained that she had been under extreme stress and had lost control in the moment. She expressed deep remorse for her actions and stated that it was entirely out of character.</p><p>The investigation revealed that there had been no previous complaints against Sarah, but it also highlighted that she had not sought any support or counselling despite her known personal issues. The hospital's policy on dealing with aggressive patients was reviewed, and it was found that Sarah had not followed the appropriate de-escalation procedures that were in place.</p><p><br/></p><p><strong>Fitness to Practice Referral:</strong></p><p>As a result of the investigation, the hospital referred Sarah to the Nursing and Midwifery Council for a fitness to practice hearing.</p><p><br/></p><p><strong>i.&nbsp; Explain what is meant by the term accountability in this scenario.</strong></p><p><strong>ii.&nbsp;&nbsp;What concerns may have been raised regarding Sarah’s behaviour and professionalism? He lives alone in a two-story house. Since his stroke, Mr Harris has been experiencing difficulty with mobility, especially climbing stairs, and has had a few near falls. He has a history of hypertension and type 2 diabetes. His mobility has declined further due to a lack of regular exercise and reluctance to use his walking aid.</p><p><strong>Initial Observations:</strong></p><ul><li><p>Mr Harris is struggling to walk steadily and refuses to use his prescribed walking stick, stating that it makes him feel "old and weak."</p></li><li><p>His home has narrow stairs with no handrails, and he frequently needs to go upstairs to access his bedroom and bathroom.</p></li><li><p>The living room is cluttered, with potential trip hazards such as loose rugs and electrical cords.</p></li><li><p>Mr Harris admits to feeling anxious about falling and has limited his movements, leading to further muscle weakness and reduced confidence in his abilities. This has left Mr Harris feeling lonely, as he has avoided his usual social activities with his friends.</p></li><li><p>Mr Harris also reports that he has been skipping meals and medication due to difficulty preparing food and managing his diabetes alone.</p></li></ul><p>&nbsp;</p><p>&nbsp;</p><p><strong>Scenario two:</strong> Mr Samoi Qai&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</p><p> <strong>Age:</strong> 75</p><p>Mr Samoi, whose first language sis not English has been prescribed two different drugs. They have been told verbally to take one of the drugs on a daily basis and the other is to be taken once a week. Mr Samoi has visual problems and struggles to read the instructions on the labels and takes both drugs every day. The result is that the patient is rushed to the emergency department suffering a severe reaction.</p>]]></description>
