Smoking would be the confounding variable in this situation. It dictates a variable which has influences on both the dependent (pancreatic cancer) and independent ( Coffee) variables. It is also common for the confounding variable to be unknown.

Internal Validity: The extent to which an experiment supports a causal relationship between variables. Communication is an important part of internal validity.
External Validity: External validity describes the generalizability of a study to a group or situation outside the context of the study. For example, comparative psychotherapy studies often employ specific samples (e.g. volunteers, highly depressed, no comorbidity). If psychotherapy is found effective for these sample patients, will it also be effective for non-volunteers or the mildly depressed or patients with concurrent other disorders? If not, the external validity of the study would be limited.
