         <description><![CDATA[<p>Incident reports are important in a work place environment. The purpose of an incident report is to document any accidents that may occur on the job. The incident report helps improve safety, accountability, compliance, promotes constant improvement in the work place, and serves as an important legal document in the case of an investigation. An incident report is created in any circumstance where a casualty has taken place. Common events that require incident reports are injuries, property damage, security breach, and near misses. No matter the severity of the accident (major or minor) it is a requirement to file an incident report. </p>]]></description>

         <description><![CDATA[<p>We risk public safety and/or public reputation if the report is not filed properly or at all. Some examples include e.g. 'An incident occurs and no report is filed may lead to the incident reoccuring', another would be 'An incomplete incident report has lead to the problem being partly resolved but due to a lack of detail a similar incident will likely occur again'. A fully detailed report is very important for the safety of everyone and ensuring their concerns are being heard and considered. Incidents will inevitably happen, reports are able to react to them in order to have proactive measures put in place to reduce or ultimately prevent them.</p>]]></description>

Following this, the situation also needs to be known, what was going on during the time that the incident happened. Every bit of detail needs to be included so that a proper picture can be painted and nothing is missed. If a detail is missed that can be a liability as it wasn't in the official report. </p>]]></description>

         <description><![CDATA[<p> Find out the problem</p><p><br/></p><p>incident reports are made when something goes wrong at work like an injury, property damage, or even a close call. they help recorded what happened, why it happened, and how to stop it from happening again. for the company it's important to keep everyone safe and protect us from legal or insurance problems.</p><p><br/></p><p>Ask, what at stake</p><p><br/></p><p>if we don't do reports or do them badly, it can cause a very big issue. we could get blamed unfairly, lose insurance. or keep making the same mistakes, workers could stay at risk,clients might face delays, and the company could get fined or sued. basically, accurate reports protects everyone.</p><p><br/></p><p>Consider resources/ Generate solutions </p><p><br/></p><p>Different people read reports for different reasons. management uses them to improve safety, lawyers and insurance need facts, safety officers track problems, and clients or regulators check that were doing things right. the report should include what happened who was involved, injuries or damages, and what steps we took to fix and prevent it.</p><p><br/></p><p><br/></p>]]></description>

         <description><![CDATA[<p>important is the structure of report itself, needs to be proofread to eliminate errors by the company and also the lawyers, to meet all laws and standards. in case something was edited, it needs to be still in the main frame so it is connected and no misunderstanding. every phase needs to be done in specific time, that's why we will set up a timeline, not only for all our colleagues, but also for people reporting in our new report. This will ensure that the final document is both practical and effective for real workplace use.</p>]]></description>
