The team is based on the 1^st floor of Rholben Villa.

We provide assessment, treatment, and ongoing support for individuals with moderate to severe mental health needs/diagnosis.

It is a Multi-Disciplinary Team (MDT) made up of Community Psychiatric Nurses, Social Workers, Occupational Therapists, Psychologists and Healthcare Support Workers.

Our role is to help individuals with mental health needs maintain good mental health, stability, and independence in the community.

The team is dedicated to proving individualised care to each service user in a manner and environment that is acceptable and accessible for them and is not always dictated by what the service can provide.

The team uses a multi disciplinary/whole team approach to actively engage individuals, with multiple and complex needs, to prevent relapse and recurrent hospital admissions.

Service user’s families and relatives opinions are often important in formulating care for individuals.

As a team we encourage families, relatives or significant others the opportunity to participate in supporting their loved one.

We aim to successfully support, educate and empower service users in maintaining good mental health, social inclusion and engagement with services and strive to maintain the dignity and confidentiality of individuals whom we support.
