- Follow Mr Hadisyam with Mr Awie to KLINIK KESIHATAN BEDONG to shoot a video on the cooking demo of ‘Less Sodium Program'. 

 

- The program was conducted by Ms Zalikha Amirah Zainol (Pegawai Sains Pemakanan (PSP)) and assisted by SN Ommi Salmah Murat (Health Nurse) and JM Rawati Binti Ahmad (Community Nurse). Dishes served are Spaghetti Bolognese Chicken Balls and Nachos. The program undergoing for 4 hours.

- Sharimila and I were taken to the registration office by Mrs Anis to create a ‘Fingerprint ID’.

- Attend afternoon meeting at Flat Paya Nahu. Ms Azila Azmi from the Health Promotion Unit at JABATAN KESIHATAN NEGERI (JKN). The meeting discussion on the factors of increasing dengue cases and find the solution to the problem. Finally, they decided to have a ‘Gotong-Royong Program’ at Flat Paya Nahu on Saturday, 20 March 2021 around 8.30 am.

-  At 5.00 pm we start fitness activities for 'I-Fiter Program' conducted by Mrs Anis. This program helps the community to lose weight healthily through proper exercise activities such as seat up, stretching and squad. The program ended at 6:00 pm.

- Mr Awie and Mr Nazri showed us about the condition of the poster room that needs to be improved. They ask us to classify and updated the tagging of the flyers. 

- The poster room has various types of disease information, prevention measures and an exhibition of awards that have been obtained by the UNIT PROMOSI KESIHATAN. The purpose of this arrangement is to facilitate the distribution of posters during visits and the implementation of programs in places at risk to local residents.

- Sharimila and I were reminded by Madam Juita to report on duty to KLINIK KESIHATAN MERBOK at Physiotherapy Unit and meet with Mrs Husna on 21/3/2021. At KLINIK KESIHATAN MERBOK, we have one week internship training program.

- We went out for lunch with Mr Hadisyam, Mr Awie and Mrs Anis.

- Before returning to the office we made an announcement in Jalan Sekerat about the Conditional Movement Control Order (PKPB). This is because, yesterday there was a positive case of Covid-19 related to the Indian community association in the area known as the ‘Jalan Sekerat Cluster’.

- We resume tidy up and arranging flyers in the poster room until working hours ended.

- Sharimila and I were given a project that needs to be implemented to attract more people to visit the UNIT PROMOSI KESIHATAN. This paperwork should be completed within two weeks and should be presented to Madam Juita.

 - Then, we went to the poster room to resume the work of arranging flyers and posters. Record each type of disease during the arrangement of flyers on each shelve. Next, we typed and printed the sentence types of diseases while being monitored and corrected spelling errors by Mr Hadisyam and Mr Awie.
- After cutting the types of diseases that have been printed, we returned to the poster room to remove the ‘old tagging’ and paste the ‘new tagging’. 

- Then we changed into sports clothes for the ‘i-Fiter’ physical activity program.

- Set up for ‘i-Fiter’ program. I, Mrs Anis and Sharimila prepared the place by arranging equipment in each station for physical activity. Mr Awie, Mr Hadisyam and Mr Nazri also help to install the speakers.

- We wait for participants and record the latest weight using electronic scales. The purpose was to measure the differences of participants' current weight during the ‘I-Fiter’ program.

- 5.00 pm - The program ended and all equipment was well kept. Then a soft reminder from Madam Juita asked us to attend Gotong-Royong Program at Flat Paya Nahu on Saturday 20/3/2021 conducted by Mr Hadisyam.

- 8.30 am-The program began with the registration of residents and shortlisted representatives as well as cooperation from the health clinic, Angkatan Pertahanan Awam (APM), Bomba dan Penyelamat. YB also participated to make this gotong-royong program going well.

 

- Residents seen were busy cleaning their yards and disposing of rubbish that clogged drains. Fire force also helped by releasing strong splashes of water to wash the silt -filled ditches.

- At 11.00 am the program ended. Then we moved to the morning market near the Taman Ria and industrial area to make announcements about social distance care and a friendly reminder of the new norms regarding the Covid-19 outbreak.

-This is a last day for I and Sharimila internship at KLINIK KESIHATAN MERBOK. Mrs Huda and Mrs Husna create a small farewell ceremony. At 3.30 pm we go back to home. 

- Today, we focused on ‘Pasar Pagi Taman Intan’.

- There are almost 8 person have registered today at booth.

- Registration are using smart phone and laptop through app ‘My Sejahtera’.

 

- At 9.00 am we go to AIMST UNIVERSITY for second day ‘Vaccine Registration Program’. This event held from 9.00 am until 3.00 pm. There are around 60 people have registered today.

- We categorized the ‘Covid-19 Poster’ to be distributed to the other department.  

-  Next, we go to FLAT PAYA NAHU and met Mrs Noriza to give some document for COMBI volunteer information filling form.

-  Then meet Mr Shukri at JABATAN PERPADUAN NEGARA to pick up bags for the 'COMBI Program'

-  Before returning to the office, we followed Mr Awie for shooting video of 'PROGRAM PIRAMID MAKANAN MALAYSIA 2020'. The program   ended at 3.30pm.

-  At 4.30 pm, we helped Mrs Anis for the 'I-Fiter' program.

-  Join with us 2 practical students from the PEJABAT DAERAH. The program runs from 10.00 am to 5.00 pm. A total of 44 people have registered today.

-  At 9.30, we go to TESCO UTARA, SUNGAI PETANI with Mrs Anis to open booth for vaccine registration. This is the third day we are here.

-  There are 2 practical student from PEJABAT DAERAH come and help us do registration for vaccination.

-  There are 20 people have been registered at our booth from 9.30 am until 1.30 pm.

-  At 11.00 am, we go for second meeting at SURAU GURUN JAYA. We meet with AJK GURUN JAYA which is Mr Abdul Aziz. 

-  At 2.30 pm, we go meeting at TAMAN RIA MESRA II. We meet with Mr Zabir and 2 others AJK. 

The details in the meeting discuss about ‘DASAR PECAH DAN PERINTAH’ for every zone and area. Then, answer and fill the Google form, selection on temporary committee and searching for COMBI volunteers. 

- Then, we help Mrs Anis and Mr Nazri to paste hand sanitizer sticker on the bottles. This hand sanitizer bottles we are going to distribute to UITM students for vaccine registration program.

- Then, at 9.00 am I and Sharimila follow Mrs Anis to UITM, Merbok. We are going to set up booth for Vaccination Registration Program.

- From the data collection, there are 20 people have been registered. 

- Today, we got work from Madam Juita to fill up online registration through website for those whose had done paper register form. This work should be finished up on Sunday, 11/4/2021.

- Then, I and Sharimila meet Madam Juita to discuss about Vaccination Registration Program that will held at Tesco Selatan on Saturday, 10/11/2021.

- Madam Juita also give some advice and tips during we undergoing internship and what will happen in workplace when we start working in the future.

- At 9.00 am, we go to UITM, Merbok for second day vaccination registration program. We continue to finish up the registration through website and encourage the student to come to our booth.

- Today, together with us there are 2 student practical from PEJABAT DAERAH come and help us encourage people to register vaccine via My Sejahtera app.

- Then, at 9.15 am I, Sharimila and Mrs Anis go to LOTUS STORE SOUTH, BAKAR ARANG to open vaccine registration booth. The program ended at 5.00 pm.

- After that, I, Sharimila, Mrs Anis and Mr Hadisyam go to ECO SHOP, DIY and IKA shop to buy tools for Sauna Room.

- At 9.00 am, we go to KOLEJ KOMUNITI to open vaccine registration booth. Today, we encourage student with 18 age above to take an action to register themselves as vaccine achiever. Almost 60 peoples have been registered. The program ended at 12.30 pm.

- At 2.00 pm until 5.00 pm, I and Sharimila go to Sauna Room to continue the room decoration. We do task like paste white paper at wall and door, covering bulb with yellow plastic, sweep and mop, and set up place for massage section. 

- Today we don’t have too much work.

- This morning there was FB Live about the sharing about "PUASA & UBAT" by Mrs Fauziah Binti Ismail (PEGAWAI FARMASI KLINIK KESIHATAN MERBOK) and Mrs Nazarina ABD Rahman (PEGAWAI FARMASI KLINIK KESIHATAN TAMAN INTAN).

- In the afternoon, I met with Madam Juita and she informed me that there are 2 programs this Saturday, which are 'Gotong-Royong' program and vaccine registration program at C-Mart.

- Today our booth was attended by 2 practical students from PEJABAT DAERAH.

- A total of 8 people have registered as vaccine recipients.

- Then, Madam Juita asked for help to tidy up the prayer room, pantry room and sauna room as there was a director’s visit to WELLNESS HUB. Mrs Suraya who is the Director of Health will be coming to WELLNESS HUB on Tuesday 20/4/2021.

- At 10.30 am, we go to open vaccine registration booth at C-Mart.

- At 3.00 pm there a visit from Ms Azila Azmi at WELLNESS HUB.

- At 4 pm, Sharimila and I meet with Madam Fazalani to give some exposure on the NUTRITION UNIT. Then, we were given one project to run a cooking demo program on next Monday.

- Before punch out fingerprint, we prepared info graphic for sauna room such as a massage technique.

- At 10.30 am there was a visit from the Mrs Suraya (PENGARAH BAHAGIAN PENDIDIKAN KESIHATAN).

- In afternoon, Sharimila and I were searching for nutrition related reference materials for Cooking Demo Program that will held on next Monday.

- In the afternoon, I went to WELLNESS HUB to meet Sharimila and discuss about the menu selection suggested by Ms Zalikha. We were asked to provide menu and script.

- Our focus in this program is to increase appetite among children with growth retardation. Focus group of children among 1 until 5 years old.

- Our focus today is to prepare flyer, menu and script on the NUGGET KENTANG AYAM Cooking Demo.

- After that, we presented flyer to Madam Fazalani and made some copies of the flyer that will be used for tomorrow's program.

- 8.30 am, Sharimila and I moved to KLINIK DESA BUJANG to meet with Ms Zalikha and run a cooking demo at the clinic.

- 9.30 am, cooking demo program is conducted. There are about 12 audience among mothers with children in the range of 1 to 4 years old. The program ends at 10.30 am.

- Then we met Ms Zalikha and listened how she consult patients then she give some comments on our cooking demos. At 1.30 pm Sharimila and I arrived at WELLNESS HUB.

- At 3.00 pm, Sharimila and I had a training with Mrs Anis for tomorrow program which is Active In Ramadhan Program (Chair Workout).

- At 11.00 am, our program has been started and there are 322 views and got 3 positive comments from community.

- After the program, we do info graphic slide for our second project in NUTRITION UNIT. I choose ‘5 TIP JAGA KALORI DALAM PEMAKANAN’. Our work will be evaluated and monitored by Madam Fazalani.

- First slot of the COMBI meeting started at 10.00 am with Mr Muhamad Nazri Bin Ariffin from Taman Melor.

- Second slot of the COMBI meeting at 12.00 pm with Mr MA Zukhi from Taman Arkid.

- The purpose of this meeting is to find volunteers who are committed in carrying out COMBI program activities.

- On afternoon, we go to sauna room to paste info graphic that lay down on the floor and tidy up the room.

- Then, Madam Juita asked us to help Mr Awie create a ‘Google Form’ to record attendance of ‘Mesyuarat dan Taklimat Pengukuhan kepada Guru Penasihat Kelab Doktor Muda Sekolah Rendah’.

- I and Sharimila also attended and heard the point of the meeting.

- We arrange the scattered items and separate them according to the function of each item.

- We rearrange items that are still in good condition and dispose of items that have been damaged. 

- So today, I, Sharimila, Madam Juita, Madam Fazalani, Mr Awie and Mr Nazri working together to help remove damaged items. 

- The affected areas are the screening room (Bilik Saringan), counseling room (Bilik Kaunseling) and meeting room (Ruangan Mesyuarat).

- The work we did was moving chairs and tables, moving cupboards, pushing water out and mopping the floor. 

- In the afternoon, we help Madam Juita, Mrs Anis, Mr Awie and Mr Nazri to arrange and divide ‘Sampul Duit Raya’ which will be distributed to many agencies and departments. 

- Then, at 10.00 am we follow Madam Juita and Mr Hadisyam to PEJABAT DAERAH and MAJLIS PERBANDARAN SUNGAI PETANI KEDAH (MPSPK). At there, I and Sharimila need to distribute ‘Sampul Duit Raya’ to the public who receive vaccinations, especially for Malays. 

- Then, at 10.30 am I and Sharimila help Madam Juita, Mr Awie and Mr Nazri to distribute ‘Sampul Duit Raya’ to the public that come to get services at PEJABAT KESIHATAN DAERAH KUALA MUDA. 

- In this morning, I and Sharimila help Madam Juita clean the refrigerator and throw out the thing that had already expired and rotten.

- At 1.00 pm, we move out to fingerprint. 

- I and Sharimila do planning on how to decorate and clean the living room (waiting area).

- At afternoon, we remove all info and pictures from the notice board. Then, we paste white paper as a background and then divide it into 2 parts, namely the “WELLNESS HUB Info Corner” and the “WELLNESS HUB Activity Corner”.

Info and pictures that are easily torn we laminated to look neater. Thumbtacks and ribbons are used to stick and decorate notice boards. 

- Today, Madam Juita informed us that next week we would be transfer to another department. I and Sharimila will be placed under the care of CC Ina better known as Haslina in the Finance Unit.

- In the evening at 3.00 pm, we had a meeting with Madam Juita. Madam Juita asked us to make 8 copies of the information to be discussed in the meeting. This meeting is about a “Summary of What Need to Know and Do in a COVID19 Case Investigation”.

- We were exposed to the categories that had close contact with COVID19 positive patients. They are category CC1 (must do SWAB test and wrist) and CC2 (need to undergo SWAB test and wrist). The meeting ended at 4.45 pm. 

- At 10.30 am, I and Sharimila have programme with Mrs Anis. We need to do an exercise video about Cardio Training that include high knee, butt kick, burpees, mountain climbers and plank to push-up.

- After that, we help Mrs Anis to edit the video as Mr Hadisyam has not come yet because he need to settle the other matter. This video will be uploaded through Facebook page Unit Promosi Kesihatan PKD Kuala Muda. 

- Then, we helped Mrs Anis make a video.

- This exercise video is about ‘Core Body Workout’. The movements we do include Leg Raise, Russian Twist, Curl Up, Heel Touch and Elbow Plank.

- Mrs Anis divided these types of exercises for me and Sharimila to do. Meanwhile, Mrs Anis recorded a video.

- Once the video was finished recording, we edited using the Capcut app.

-  At 9.00 am, we are given some briefing about what undergoing in financial unit.

- After that, CC Haslina given some task for me and Sharimila. The task need to complete within one week.

- In financial unit, CC NorHaslina share the flow of finance organisation chart. From the upper section was conducted by Dr Suziana Bt Redzuan (Pegawai Kesihatan Daerah UD56), follow by Mrs Hafsah Bt Kadir (Penolong Pegawai Tadbir N29), follow by Mrs Haslina Bt Hashim (Pembantu Tadbir W22) who is our incharge on this week. Under her follow by her staffs.

- We do stamping the document that have been certified by the superior officer of each department and the financial officer with ‘TELAH BAYAR’ stamp. 

- But this time, we need to stamp the document according to financial officer recorded. Today, we finished stamp Mrs Sharifah document.

- Madam Juita ask our help to do 10 copies of document for her meeting.

- Then, I update to my faculty supervisor about the appointment meeting with Madam Juita.

- Madam Juita inform us that this week we have been placed at management department.

- So, tomorrow I and Sharimila need to meet with EO Hafsah Binti Kadir (Penolong Pegawai Tadbir N29) at the office. 

- Madam Juita briefing to us about the task of what we need to do. We were directed to help investigator of COVID-19 key in the details that they collected through phone call with the person that had close contact with Positive Covid-19.

- Then, I and Sharimila go to WELLNESS HUB to get a letter of permission to come to office during Movement Control Order (MCO) for the duration of COVID-19.

- Then, Madam Juita inform us through phone call for whose that need to print should email the document through upkkualamuda@gmail.com. 

- At afternoon, we need to do 20 copies of document and give to the person at Melati Meeting Room. 

- Then, we check the email of upkkualamuda@gmail.com time by time if there are document that needs to print. 

- Then, at 9.00 am we are already at MELATI Meeting Room.

- We had task to do. I and Sharimila typing for investigation document and create Excel. 

- We help nurses to type investigation document and create Excel for close contact family members of Positive COVID-19.

- We had done for 4 documents of Microsoft Word and 3 Excel for this day investigation report. 

- We help nurses doing investigate the cases and key in the personal details of Positive COVID-19 and their close contact.

- From what we understand, the high risk to be infected will be called to do SWAB Test. 

- After break, we’re at WELLNESS HUB and have been called by Madam Juita. She give us a new project about “Norma Baharu”.

- We need to create a short video about 30 seconds. The topic is “Gantikan Bersalaman Dengan Meletakkan Tangan Di Dada”.

- Then, we do a script about the story line and submit it on her desk. 

- She give a positive comments and she also informed that we get approved from her boss (Dr. Suziana) to upload the video in their Facebook page.

- We just need to do a simple correction on our video and resend it to Madam Juita.

- Then, we create a script on second video about “Pakai Mask”.

- I and Sharimila discuss and brainstorm on what action that we want to do. 

- I and Sharimila are shooting a video. We prepare the stuff and clothes for shoot. 

- We retake the video until it going smoothly. It takes a lot of time to edit and searching the meme sounds that can be attachable with our video. 

- But a few minutes later, she inform that her boss disagree with video and we need to improve the content and the action. So, I and Sharimila need to brainstorm another action that suitable with the video. 

- We have been informed by Madam Juita that tomorrow we need to set up the place for “Majlis Bacaan Yaasin”. 

- Then, we set up the place by placing the tables and chairs for today event. The event will be started at 9.00 a.m. 

- We get ready with 3^rd episode script about “Sembang Dekat” and submit it to Madam Juita. 

- Then, we discuss scene on episode 3 about “Sembang Dekat”.

- At afternoon, we create the storyline on notebook.

- Then, we print it and show the script to Madam Juita.

- After that, we start our first shoot on scene of Episode 3.

- Then, we complete the animation video.

- I and Sharimila discuss and download the memes sound and music that suitable to be added on our video.

- Then, We continue editing the video and submit the video to Madam Juita.

- Next, I type the script through Word Document and printed it.

- Help mr Hadisyam untuk set up meja for Majlis Bacaan Yaasin.

- Then, we prepare the thing and start shoot the scene for episode 4 about “Pembelian Secara Online”.

- We spend time on searching the memes sound effect, cartoon, animation, some other information and background music to add in video.

- Then, we ask Mr Hadisyam to help us to create certificate and fill in our detail. 

- Log book and the document that needed by FHPK faculty also done submitted at Madam Juita for her to do observation and commented on our work during internship.
