10-year-old Caucasian female.
Cerebral Palsy
LIves with married Mother and Father and has an older brother who is just learning to drive at 16 years old. Older Brother is very involved with extracurricular sports that parents drive him to and from after school. 

Mother and Father supportive and attend Parent Teacher meetings and 
IEP-Individual Ed Plan

Sara needs reminders to take medication.
Sara is unable to walk and relies upon a PMD Power mobility device. She uses the PMD indep.  
Sara has speech issues with articulation. To allow others to understand 9her she uses a computer to speak for her voice. 
Sara exhibits weakened trunk and LE muscles 3/5 MMT
Sara is only able to stand with support of a device such as a walker. 

Sara toilets indep.
Sara gets in out of bed indep.
Sara is unable to get in out of car and sit in car seats without assistance.  Transfers are mod A to car with stand pivot.
Sara's family has a WC transport van. 

Sara is able to use PMD to socialize, attend school and after school functions with her brother. 

The family home has a roll in shower and 2 stories.  Sara's bedroom is on first floor along with bathroom. 

The school has ramps and elevator for second floor access.
