Supporting Family Change by Families First Central Support Team

Supporting Family Change (SFC) brings together a Team Around the Family (TAF) for families that have multiple, complex needs and require coordination of services.

A designated support practitioner works with the family, along with other agencies, to develop a plan that focuses on the strengths of the family.

The intention is to provide early support for families with the aim of preventing problems escalating.

All local authorities in Wales have a TAF model, which recognise local variation and needs of the families in the communities.

The TAF model focussed on addressing the needs of the whole family, rather than individuals.

Supporting Family Change (SFC) delivers the TAF Model as part of the Caerphilly Families First Programme.

Caerphilly's TAF Model

Stage 1 – Initial referral & allocation

Once allocated to a TAF Worker, the worker will contact the family. An initial face to face will take place.

Stage 2 - Home visit and assessment

The TAF Worker will arrange a visit to talk with the whole family about what is going well and what they may need support with. The JAFF will be completed as an assessment tool.

Stage 3 – TAF Action Plan

The family support plan will initially look at support for approximately 12 weeks - it will include actions for everyone in the family and for other agencies. A TAF Worker will organise a TAF review meeting involving the family, current agencies working with the family and potential future agencies involved in the family plan.

Stage 4 - Review process

The TAF action plan will be reviewed regularly (every 6-8 weeks) to evaluate progress made. If the needs have not been met within the initial 12 weeks, there is a possibility of an extension.

Stage 5 - Closure

If outcomes are achieved and/or the role of the TAF model is completed then the case will be closed. If the needs of families cannot be met via TAF, family referrals to specialist services will be considered. Resilience plans will be written with the family and implemented as part of the closure process. At closure families should, if deemed appropriate and necessary, be referred to single agencies or signposted to universal provisions.

Please contact the JAFF Coordinator prior to completing a JAFF to ensure multiple copies of the document aren't created. The JAFF is designed to follow a family's journey, and therefore there should only be one 'rolling' document.

Supporting Family Change can only work with families who are NOT open to Statutory services.

Criteria for SFC is:

There are multiple needs that do not meet the criteria for statutory intervention but are likely to escalate without support
2 or more targeted services are required to meet the needs of the family, hence the need for co-ordination of a multi agency plan
There has been substantial interventions for the family from a range of services but no positive change has been achieved
The family agree to involvement and understand the process around the sharing of information
The family has children aged between 0-25 years
