TIGER which stands for Technology Informatics Guiding Education Reform Initiative was designed to address skills that all nurses will need in the healthcare setting now and in the future. It is expected that all nurses are going to have to be at least competent in informatics, from basic computer skills to advanced-level information technology and literacy expertise. During the week I realized how this is considered so important. Our world and healthcare system is controlled on a widely electronical basis. I see why it is critical that nurses have a solid foundation in informatics so that they are safely documenting according to the standards of nursing practice.

Ever been in that situation where you're trying your hardest to figure out someone's made up abbreviation? I know I have! It can be super frustrating as you are trying to figure it out and in certain cases this could lead to medical errors if you interpret it for something it is not. Interoperability is in place to try and elevate some of this.

Interoperability is the ability of two or more systems to exchange health information and use the information once it is received (ex. transferring someone to a different hospital). The data get transferred in a standardized way that is safe to share. Two systems can recognize terminology and medication symbols without human intervention. This allows for the patient's information to be safely transferred without information getting lost or misconstrued.

The DIKW hierarchy is a method used in knowledge management. It is used to explain the ways we move from data, to information, knowledge, and wisdom with parts of action and decisions.

It appears as though it is a chain like the Maslow Hierarchy of Needs where you start at the base and you cannot have one without the other, so you work your way up. Data is a measurement or facts that through context provides information. With information that is organized and structured it forms meaning resulting in knowledge that through learning forms insight allowing full insight and understanding through wisdom. DIKW is always used in public health through the use for new policies and skill sets brought out, especially in nursing. Health informatics can help develop digital documentation and improve parts of it through frequent use.

It gets into lack of standardization, consent, security, and professional burdens. I have personally seen issues in regards to cyber security in my own place of work. Our system can often be attacked through emails. Every hospital I have worked at has had a short in-service with some modules to become familiar with what emails to be aware of and what to do if you feel as though you have been under attack.

These initiatives such as TIGER are in place in order to allow hospital staff to provide quality care to patients. HIMSS conducted a cross-sectional study in 2018. Data was collected using the researcher- developed questionnaire.
"The majority of participants (96%) held a bachelor’s degree. According to the participants views, 66.5% had relatively good familiarity with computers. In terms of frequency of using computers, the vast majority (59%) used computers more than once a day." This goes to show that the quality of patient care can be better ensured by communicating effectively and sharing accurate information. This in turn, will increase the continuity of care. Thorough documentation and knowledge development to support evidence-based practice is imperative to developing nursing informatics knowledge and nursing informatics competencies.

Self-Assessment of Nursing Informatics Competencies in Hospitals | HIMSS

I would actually utilize the DIKW hierarchy in any given healthcare setting. I remember when I took a travel assignment and the hospital recently switched over to EPIC. The hospital staff was lost! They did not get great education on it so the staff would always ask me how to do certain things on the new system. A new system is a HUGE transition and can be very challenging.

If I was the NIS in this situation I would first address the staff on the new EHR that is going to be implemented. I would reassure the staff that there will be in-service days where they can learn the new system that way they understand they will not just be thrown into it. We will first receive data based off a survey with the staff. I want to get an idea what is important to them and what they want to see with this implementation. I will take this information and have better knowledge of what the staff is requesting and be open to any suggestions they may have to make the EHR the most time manageable compared to paper documentation.

An experience I have had with an aspect of meaningful use is using the EMAR to administer medications. The first thing that comes to mind is IV heparin drips, these at my hospital need to be 2 nurse verified. I know some hospitals that do not need a 2 nurse verifier and this shocks me. Both Epic and the Meditech computer systems I have used require this and it truly gives me a sense of safety and security. There is no way to go around this and IV infusions should always require a second set of eyes. Myself and another nurse are able to verify the correct dose is being given or adjusted properly. Safety is key!

I believe EMRs are super useful and user friendly, especially at all the hospitals I have worked at. It is a great tool for all members of the interdispilnary team to work together and see all of the EMR at the same time. Everyone can view the same things and it allows the team to fully understand the patient and their current state of health. Epic that I used at the first hospital I worked at actually allowed us to live chat with other members of the interdisciplinary team. I was able to quickly message doctors who were off the floor at the time and ask a question about the patient I needed to and we could both see all of the assessment data right at our fingertips. I love the use of EMR in the acute care setting. It makes everything happen in a much more time efficient manner.

A group of 100 people with honest, true, accurate data is better than having 1,000 people's data that may be different or misleading. Taking the time to collect data ensures that the research in place is TRUTHFUL and SAFE and that is how it should remain. Data in medicine needs to be controlled and those researching the data need to have patience in order to keep everyone safe while still changing the lives of others each and everyday!

This app is in place to test those with Alzheimer's navigational skills specifically with a direction with an end goal and a result. A lot of people who used to play it when it was open to the public mentioned that the game actually helped them learn some of their directional and awareness skills even not having dementia. While people can benefit from playing they seem to be upset it is no longer open to the general public. I wonder if this research has moved on to the next phase of data collection possibly? Very interesting to see technology advance in this way.

It is so exhausting sometimes hearing alarms continuously go off that you truly just want to ignore them. I know it is wrong but I am honest and I know anyone who has worked in an acute care setting has suffered from this. I try to handle it by walking off the floor, even if it is for 5 minutes, it gives me a nice break to relax and not hear a bunch of random noises. When I come back I do feel better, I was able to take a breather, and can focus back on my work. In my opinion, I think we need to be better about alarm parameters and setting them to what is appropriate to the patient. It can make a huge difference.

This video is a quick and easy watch! This video explains how alert fatigue occurs. Some alerts we see on a day to day basis do not even require any clinical intervention. WHY HAVE THEM? Having these unnecessary pop ups make it extremely difficult to slow down and read a pop up you may actually should because you are so used to clicking out as fast as you can to continue what your previous tasks at hand was. This video brings the dangers to light but shows it needs to be improved on a more technology basis and how it does not fall only on doctors and nurses to fix.

HIPAA...are you following it?

I would like to say (and hope this applies to all hospitals I've ever worked at) that yes the hospital I am now at is meeting HIPAA requirements. We provide patients their rights upon admission and only discuss their personal health information with themselves or members of the interdisciplinary team. Especially with limited visitors due to covid we have ensured the person receiving updates is the Health Care Proxy so they are allowed to receive information too that way we are not violating HIPAA requirements. One thing I always try to be aware of is WHERE I am discussing patient care or specific information about patients. Some hospitals do things like "No elevator talk", just as a reminder to not in open spaces where people that are not apart of the interdisciplinary team can over hear you.

This article does a great job on addressing privacy concerns within healthcare. It also goes on to address and implement privacy controls and practices and how those can be achieved.

Patients must be confident that their privacy rights and the confidentiality of their personal information and personal health information are respected and upheld, and that the information they share is kept confidential and secure. Maintaining high standards that safeguard information privacy and security is an essential aspect of healthcare that should always be TOP PRIORITY.

Health privacy has become increasingly important due to the rapid rate of technology integration in healthcare. New technologies like mobile apps that allow you to view tests results or past medical history. You can now track substantially larger amounts of information about a patient than ever before and put it in an easy to access place such as a cloud-based service.
