In my old setting, I worked with students with moderate disabilities, and their transition assessments included college-related assessments. In general, students completed surveys through the Naviance online tool which helped identify schools they ould be interested in. Additionally, the transition coordinator used assessments of college readiness to identify areas of need for students who wanted to go to school after high school. Additionally, she would use information about student goals and areas of interest or intended majors for college to help students choose appropriate high school coursework to prepare. Although most of the students in our therapeutic program do not have a diagnosed intellectual disability, many of our students have difficulty reading and are way below grade level in all areas. They are not traditional college-bound students and many do not feel they can go to college at all.
