The Care Certificate is a programme of theory based education and practical based work that enable you to meet the required standard and be awarded your certificate.

The Care Certificate is a requirement for all patient facing unregistered members of staff within the Trust.

If you have an identified learning or support need, please contact us so we can arrange a suitable plan to support your completion in a way which works for you.

As a team, we work Monday - Friday and will be able to assist with any questions or additional support you may require. We encourage to contact us as soon as your identify an area of support required so we can support you as best we can.

Once you have completed each module, there will be an online certificate attached to your account. This will need to seen by a Registered member of staff in your clinical area and the Sign off Document completed for the relevant section.

The staff in your clinical area also need to assess your practical skills and complete the Sign Off Document once you have demonstrated each practical element within your clinical area. This can be by a Registered professional or by an unregistered member of the team, provided they have completed the Care Certificate themselves already.

The final page of the sign off document (page 52 - 'Declaration of Completion') needs to be completed by your line manager.

Once completed, please send a copy of the final page of the sign off document signed by your line manager to us. Please ensure your line manager is copied into this email. A scanned copy or photograph would be sufficient.

If you not completed and have not engaged in the programme within the 12 deadline, we would need to discuss initiating an action plan with your line managers involvement. If this action plan is failed then it would progress to formal performance management with your line manager.

Remember your submission requires you to send a photo or scanned copy of the final page of this document to us via email. We will also require your line manager to be cc'd into the email.

Please be mindful that failure to respond or engage, may lead to a member of the team visiting your clinical area unannounced to carry out a pastoral visit.

We will also email at week 11 to remind you that submission date is only one week away.

I have been a registered adult nurse for nearly 20 years. Throughout my career I have worked in acute nephrology, haemodialysis, paediatric haemodialysis, and critical care. I became lead nurse in August 2022 to learn strategic as well as operational ways of working and I have a passion for staff development at all career levels. In order to submit, please send us a scanned copy or photograph of the 'Declaration of Completion' (the final page of the sign of document, page 52) which will be signed by your line manager. Please ensure you have cc'd your line manager into this email also.

Once submitted, the team will update the records, ESR and issue a certificate of completion for your records.
