At Ramsey middle school there were two seventh grade teams that met once a week for ~30 min. They had conversations about school policies and procedures (mostly revolving around ipads) So this was a good start, they stayed on the same page and looked to coordinate advisory periods well, but there wasn't a lot of time spent on outlining goals and involving families or other support staff in these meetings.
At southwest high school, the 9th graders were on a team. I'm not sure how often they met officially for a team since I was in tenth grade, but they seemed to be on the same page in coordinating assignments and keeping students in mind.
Another organizational structure was simply content collaboration. Collaboration in planning and reflection with other content members allowed the opportunity for improvement or fine tuning to better reach students for different assignments.
