Example for Child:
Current Nap Observations: Gianna is new to napping on the floor bed but typically falls asleep quickly. She sleeps with a blanket. She has a tendency not to move once on the bed and may need gentle help/encouragement to get into a comfortable position. She prefers to sleep on her stomach but occasionally sleeps on her back. Gianna takes 2 naps per day usually 45 minutes to an hour long (occasionally longer). She fusses when she wakes and needs a lot of encouragement to get up from the floor bed on her own. Adult proximity is usually the only support Gianna needs to fall asleep. 
Nap Action Plan: Gianna will develop the confidence to move on and off the floor bed. They started when I was on PTO so I did not get to welcome them to my community. Since they are older (12 months) I want to observe them closely to make sure I am planning meaningful presentations. 

I also have had a hard time finding the time to sit down for lesson planning, so I want to be more mindful with that. As my children are getting older they are outgrowing the materials in the classroom, so I want to work on improving my observations and lesson planning.
