         <description><![CDATA[<p><strong>Guardianship</strong> – The Eleventh Judicial Circuit's sole provider of free legal aid to indigent residents willing to serve as guardians for elderly, physically or mentally incapacitated family members, loved ones and close friends. The department is working from its dedicated offices downtown and in South Dade Courthouse to help hundreds of families each year by offering services which allow family members or close friends to become legal guardians of incapacitated and multi-generational vulnerable adults from age 21 to 110.          <description><![CDATA[<p><strong>What is the Agency for Persons with Disabilities (APD)?</strong></p><p>APD works with local communities and private providers to support people who have developmental disabilities and their families in living, learning, and working in their communities. APD also provides assistance in identifying the needs of people with developmental disabilities for supports and services so they can reach their full potential.</p><p>The agency also works to educate the public on disability issues and focuses attention on employment for people with disabilities.</p><p><br></p><p><strong>What disabilities does APD serve?</strong></p><p>In order to be eligible for services from the Agency for Persons with Disabilities (“APD”), an individual must have a developmental disability (as defined in section 393.063(12), Florida Statutes), which occurs prior to age 18 and constitutes a substantial handicap that can reasonably be expected to continue indefinitely. Disabilities served by APD include:</p><ul><li><p>Intellectual disabilities (Full Scale IQ of 70 or below)</p></li><li><p>Severe forms of autism</p></li><li><p>Spina bifida cystica or myelomeningocele</p></li><li><p>Cerebral palsy</p></li><li><p>Prader-Willi syndrome</p></li><li><p>Down syndrome</p></li><li><p>Phelan-McDermid syndrome or</p></li><li><p><a rel="noopener noreferrer nofollow" href="https://apd.myflorida.com/customers/docs/HighRiskFAQ.pdf">Individuals between the ages of 3-5 at high risk for a developmental disability</a></p></li></ul><p><br></p><p><strong>What services does APD provide?</strong></p><p>APD provides:</p><ul><li><p>Medicaid HCBS Waiver through iBudget Florida</p></li><li><p>Placement in an intermediate care facility for those with developmental disabilities</p></li><li><p>Supported employment services</p></li><li><p>Supported living services</p></li><li><p>Consumer Directed Care Plus (CDC+)</p></li><li><p>IFS (Individual and Family Supports) funding for non-waiver customer expenses</p></li></ul>]]></description>
