Exemplar

Summary of Documentation Steps:

Login & Access Resident Profile:
Begin by logging into PointClickCare using secure credentials. Locate the resident’s profile by searching for their name or ID to ensure all information is personalized and accurate.

Daily Care Notes:
Navigate to the Progress Notes section. Use structured entries to document care activities, including vitals, nutrition, hygiene, and mobility support. Time-stamp each entry and avoid abbreviations unless standardized.

Incident Reporting:
If an incident occurs, complete the Incident Report Form immediately. Include objective observations, steps taken, and who was notified (e.g., nurse supervisor or family).

Medication Administration:
Review the eMAR (Electronic Medication Administration Record). Confirm the Five Rights (right resident, medication, dose, time, route). Record administration promptly, and flag missed doses with an explanation.

Shift Handover:
Use the handover tool to summarize key updates for the next care team. Highlight changes in condition, behavioral observations, or family communications.

Reflection:
This workflow supports consistency and accountability across shifts. It ensures residents receive personalized care while reducing errors in long-term care environments.
